Adipose tissue — derived mesenchymal stem: a role in the pathogenesis of obesity and type 2 diabetes mellitus 脂肪组织衍生间充质干:在肥胖症和 2 型糖尿病发病机制中的作用

Adipose tissue-derived mesenchymal stem are adult stem cells endowed with multipotent abilities and immunomodulatory properties, like mesenchymal stem cells of other origins. Numerous studies show that adipose tissue stem cells are involved in the pathological process and can exhibit pro-inflammatory properties and attract inflammatory immune cells in the neighborhood. Subsequently, inflammation creates a microenvironment leading to adipose tissue dysfunction. Examples of such a process are obesity and type 2 diabetes mellitus, in which adipogenesis is disrupted and insulin resistance is initiated. The aim of this review is to understand the role of adipose tissue stem cells in the pathogenesis of obesity and type 2 diabetes mellitus.

Influence of surgical treatment of obesity on the course of nonalcoholic fatty liver disease 肥胖症手术治疗对非酒精性脂肪肝病程的影响

Non-alcoholic fatty liver disease (NAFLD) is a liver disease with a characteristic accumulation of fatty inclusions in hepatocytes and includes a spectrum of liver lesions from steatosis to cirrhosis. In view of the increasing incidence of obesity and associated NAFLD, a search is underway for optimal treatments. The authors analyzed published clinical studies on the effect of bariatric surgery on morphological changes in the liver. The literature was searched for the keywords «non-alcoholic fatty liver disease», «obesity», «bariatric surgery» in Pubmed databases and еLibrary.ru for the period from 1990 to 2022 . Both the positive effects of surgical treatment of obesity in the form of a decrease in steatosis, steatohepatitis of the liver, and adverse effects in the form of aggravation of cirrhosis in patients after surgery are considered. Despite the obvious effectiveness of bariatric surgery in relation to the components of the metabolic syndrome associated with obesity, there is still no unambiguous opinion about their effectiveness in relation to NAFLD.

Diagnosis and treatment of central diabetes insipidus 中枢性尿崩症的诊断与治疗

Diabetes insipidus represents a serious disease that dramatically interferes with the everyday life of patients due to the need to constantly replenish of fluid lost in the urine, which comes amid shortage of synthesis, secretion or action of pituitary hormone vasopressin. The main difficulty is the differential diagnosis of types of diabetes insipidus in patients with the syndrome of polydipsia-polyuria as the correct differential diagnosis of these forms predetermine the safety and efficacy of further treatment. This lecture presents the current concepts of etiology, diagnosis and treatment of central diabetes insipidus (CDI). We give the comparative characteristics of various preparations of desmopressin for the treatment of the central form of the disease. We also consider the features of the management of selected patient populations with CDI: during pregnancy and lactation, pathology of the thirst sensation, after traumatic brain injury and neurosurgery.

Growth hormone and insulin-like growth factor-1 in prognosis coronary artery disease in patients with obesity 生长激素和胰岛素样生长因子-1与肥胖患者冠状动脉疾病预后的关系

Introduction.  In patients with obesity coronary atherosclerosis and chronic heart failure (CHF) progress rapidly and have a worse long-term prognosis than those with normal weight. Objective:  To investigate the prognostic significance of GH and IGF-1 in the evaluation of cardiovascular risk in patients with obesity. Materials and Methods.  The study included 75 men (mean age 55.31±6.32 years), which are overweight or have mild obesity (body mass index (BMI) 28.69±3.6 kg/m 2 ). Group 1 included 45 patients (age 56.4±6.29 years, BMI 28.69±3.69 kg/m 2 , blood pressure 124±10.18/80±4.59 mm Hg) who underwent coronary angiography. Group 2 included 30 patients (mean age 53.6 ± 6.1 years, BMI 28.68±3.52 kg/m 2 , blood pressure 128±9/83±6.81 mm Hg), CAD who are excluded by treadmill test. The coronary artery calcium score was assessed in group 2. All participants were evaluated impaired glucose tolerance (IGT), triglycerides (TG), IGF-1 and GH, LPHD, LPLD, geometry of the heart chambers was assessed by echocardiography. Results.  Patients in both groups did not differ in age, BMI, blood pressure. IGF-1 levels were not significantly different among the study groups. High circulating IGF-1 levels were frequently observed in group 1 (р=0.018). A statistically significant association of high IGF-1 observed with obesity (p=0.033), smoking (p=0.049), hypertension (p=0.002), end-diastolic dimension (p=0.045). GH was lower in group 1 compared with group 2 (p=0.046). Serum levels of GH are positively associated with EF (p=0.023) and E/A (p=0.043) and negatively associated with left atrial wall thickness (p=0.025) and coronary artery calcium score (p=0.005). Conclusion:  1. IGF-1 may be a useful indicator to assess the prognosis of CAD and CHF in patients with obesity. 2. Relative GH deficiency was more often associated with severe CAD in patients with obesity.

The growth of obesity and type 2 diabetes incidence has made bariatric surgery a widespread method of treatment. The effectiveness of bariatric operations in the treatment of obesity and related metabolic diseases is thoroughly highlighted in medical literature. However, the results of surgery do not always correlate with type of operation. As before, the mechanisms have not been fully studied of how the bariatric surgery influence on insulinresistance, entero-insulin axes, adipokines. Understanding such mechanisms will allow us to determine more precisely the indications relating to surgical treatment, and enhance the effectiveness of surgery in specific patient. The review is focusing on the influence of various types of bariatric surgery on the level of adipokines and incretines that participate in regulation of appetite and of fat and carbohydrate metabolism. The article elaborates modern concepts related to the impact of bariatric operations on metabolic disorders in obesity.

This review article discusses the similarities and differences in the research, production and use in clinical practice, original and generic drugs. Reproduced drugs (generics) present on the Russian pharmaceutical market that have the bioequivalence data are not always equal in therapeutic equivalence of the original drug. The article presents evidence base of efficacy and safety of the original drug Xenical in the complex treatment of patients with overweight and obesity.

Nutrition is one of the fundamental factors that influence the effectiveness of physical activity, increase efficiency and replenishment of muscle mass, balances the ratio of energy consumed and restored. The diet of an athlete can and should be built on common foods available and prepared in accordance with generally accepted principles of healthy eating. The need for major macronutrients and micronutrients is determined by the need for energy, the intensity of sweating and the goals for building muscle mass. Depending on the intensity of the proposed load including competition, there are individual nutritional needs and, if necessary, various food supplements may be used. The basic principles of sport nutrition are described in this article

Granins as biochemical, immunohistochemical and metabolic markers of nonfunctioning pituitary adenomas 颗粒蛋白作为无功能垂体腺瘤的生化、免疫组织化学和代谢标志物

Granins are glycoproteins presented in granules of neuroendocrine cells, they regulate secretory pathway that is responsible for controlled delivery of peptides, hormones and neurotransmitters, and growth factors, organization of the granular matrix, and the mechanisms of administration after hormone secretion The highest accuracy has been observed in neuroendocrine tumors characterized by an intense secretory activity, but its specificity and sensitivity remain very high also in nonfunctioning tumors. The granins application as the markers of nonfunctioning pituitary adenomas would allow the early disease detection, assessment the secretory capacity of tumor, and differential diagnosis with autoimmune forms of pituitary lesions. Furthermore, granins can be used as markers of related metabolic disorders, especially of cardiovascular diseases.

Effect of metformin on the bioavailability of nitric oxide in patients with type 2 diabetes 二甲双胍对2型糖尿病患者一氧化氮生物利用度的影响

The aim of our study was to evaluate the levels of nitrite in plasma and methemoglobin in whole blood using electron paramagnetic resonance (EPR) in patients with type 2 diabetes mellitus receiving metformin. After 3 months of continuous use of the drug at a dose of 1700 mg in parallel with the improvement of carbohydrate and lipid metabolism, reduction in waist circumference there was a statistically significant increase in nitrite concentration in plasma and increased methemoglobin in whole blood. Increased bioavailability of nitric oxide during treatment with metformin may be an additional explanation for the mechanism of drug action.
