Endotype Conformity with Japanese Epidemiological Survey of Refractory Eosinophilic Chronic Rhinosinusitis (JESREC) Score in Chronic Rhinosinusitis 慢性鼻窦炎患者难治性嗜酸性粒细胞慢性鼻窦炎(JESREC)评分的内型与日本流行病学调查相符

Introduction: Chronic rhinosinusitis (CRS) is one of the most prevalent disease entities in ORL-HNS field. Management strategy of CRS remains challenging among otorhinolaryngologist. Currently, CRS was known as a heterogeneous disease with various endotypes and phenotypes. By identifying endotypes, comprehensive management strategy in CRS was expected. The Japanese Epidemiological Survey of Refractory Eosinophilic Chronic Rhinosinusitis (JESREC) score was a categorical score developed from Japan to diagnose eosinophilic chronic rhinosinusitis (eCRS) and non-eosinophilic chronic rhinosinusitis (non-eCRS). The assessment of JESREC score was expected to have conformity with CRS endotypes so that it can be used as future efficient diagnostic modality. Objective: This study aims to determine conformity and accuracy of endotype with JESREC score. Patients and Methods: An analytic study with a cross-sectional design involving 33 samples of primary CRS. Biomarker for specific endotypes measured by ELISA. Results: Statistical analysis with Mc. Nemar showed no difference/ a conformity between JESREC score and CRS endotypes ( p>0.05 ) with sensitivity 61,5% and specificity 90%. Conclusion: JESREC score is considered a screening modality for CRS endotype.

Vocal Tract Abnormalities in Patients with Two Genodermatoses: Lipoid Proteinosis and Neurofibromatosis Type 1 两种遗传性皮肤病患者的声道异常:脂质蛋白沉积症和1型神经纤维瘤病

Objectives: We aimed to investigate the abnormalities of the oral cavity, pharynx, and larynx of patients with two Genodermatoses: lipoid proteinosis (LP) and Neurofibromatosis type 1 (NF1) Study Design: Cross-sectional study. Patients and Methods: Twenty-five patients with genodermatoses were recruited from the Genodermatoses clinic, NRC, eighteen cases with NF1 and seven cases with LP, where precise cutaneous and oro-dental examinations were done for all patients followed by Naso-pharyngo-laryngeoscopic examination done in the phoniatric unit, faculty of medicine, Cairo university. Results: Oral, pharyngeal, and laryngeal abnormalities were detected in twenty-three, twenty, and seventeen patients respectively. Malocclusion was the main oro-dental abnormality in patients with NF1, while in patients with LP lingual and labial abnormalities were the most common oro-dental abnormalities. Reddish and white-yellowish oropharyngeal swellings were the chief pharyngeal abnormality in patients with NF1 and LP respectively. Congested and thick hypertrophic laryngeal structures are the main laryngeal abnormalities in patients with NF1 and LP respectively. Vocal fold paralysis is not uncommon finding in patients with NF1. Conclusion: Oral, pharyngeal, and laryngeal abnormalities are very common in NF1 and LP. Precise Oro-dental examination and Naso-pharyngo-laryngeoscopic evaluation are recommended for all patients with genodermatoses, especially NF1 and LP. Periodic follow-up by a multidisciplinary team is essential to detect any newly developed lesions or changes in current detected lesions.

Intracapsular versus extracapsular coblation tonsillectomy 囊内与囊外消融扁桃体切除术

Objectives: The aim of study is to evaluate and compare between using coblation in intracapsular tonsillectomy (preserving the capsule) and extracapsular tonsillectomy (total). Patients and Methods: The study was conducted on 50 patients with symptomatic tonsillar hypertrophy such as recurrent sore throat, snoring, difficulty of swallowing and recurrent follicular tonsillitis proved by ENT specialist. The study was conducted from February 2022 to December 2022 at the Department of Otorhinolaryngology, Al-Azhar University Hospitals, Cairo, Egypt. Group A formed of 25 patients underwent intracapsular coblation tonsillectomy, and Group B formed of 25 patients underwent extracapsular coblation tonsillectomy. Intraoperative and postoperative blood loss was measured plus time of surgery and postoperative pain. Results: Fifty patient included in our study underwent adenotonsillectomy, 22 patients were males and 28 patients were female. In two groups, (Group A = 25 patients, Group B = 25 patients). Our results show highly statistically significant differences ( p-value < 0.001 ) between studied groups as regard blood loss. According to pain score, VAS in the first 4 days related to Group A was (5.0 ± 2.5) and in Group B was (7.5 ± 1.5) without statistically significant difference. The percentage of normal activity after one week related to Group A was (2.0 ± 1.0) and in Group B was (4.0 ± 2.0) with statistically significant difference. Conclusion: Although the intracapsular coblation tonsillectomy consumes more operative time and intraoperative blood loss than the extracapsular technique, but it had superiority in the diminishing the postoperative pain especially after one week and the complications especially postoperative bleeding and infection.
目的:研究目的是评价和比较在囊内扁桃体切除术(保留囊)和囊外扁桃体切除术(全切除)中使用消融术的效果。患者与方法:对50例经耳鼻喉科专科医生证实为复发性咽痛、打鼾、吞咽困难、复发性滤泡性扁桃体炎等症状性扁桃体肥大患者进行研究。该研究于2022年2月至2022年12月在埃及开罗爱资哈尔大学医院耳鼻咽喉科进行。A组25例患者行囊内消融扁桃体切除术,B组25例患者行囊外消融扁桃体切除术。测量术中和术后出血量,加上手术时间和术后疼痛。结果:50例患者行腺扁桃体切除术,其中男性22例,女性28例。两组(A组25例,B组25例)。我们的研究结果显示,在研究组之间的失血量有高度的统计学显著差异(p值< 0.001)。根据疼痛评分,A组与B组的前4 d VAS相关评分为(5.0±2.5)分,B组为(7.5±1.5)分,差异无统计学意义。1周后正常活动百分率A组为(2.0±1.0),B组为(4.0±2.0),差异有统计学意义。结论:虽然囊内消融扁桃体切除术比囊外技术耗费更多的手术时间和术中出血量,但在减少术后疼痛特别是1周后的疼痛和并发症特别是术后出血和感染方面具有优势。

The Effects of Cueing in the Facilitation of Word Retrieval in Patients with Aphasia 线索对失语症患者词语检索的促进作用

Introduction: Anomia is one of the hallmarks of aphasia. Word retrieval can be facilitated by using phonological and semantic cueing strategies. Phonological cueing may be phonemically based consisting of the first consonant or first consonant plus vowel in a target word, while semantic cueing provides description of target words. Purpose of the study: To determine the effect of using phonological and semantic cueing on the facilitation of word retrieval in patients with aphasia and to compare the effect of phonological and semantic cues on picture naming accuracy. Patients and Methods: The study includes 30 participants that had suffered a left hemisphere cerebrovascular accident (CVA) at least 3 months prior to the start of the study. Each patient has carried out language assessment using the Arabic version of the Comprehensive Aphasia test (CAT) prior to treatment and post treatment. Naming tests scores (using phonemic and semantic cues) obtained in pretreatment baseline phase were compared to post-treatment scores. Results: Both phonological cueing and semantic cueing treatments improve word retrieval in participants. Naming tests scores has significantly improved for all participants. The results revealed beneficial effects of both phonological and semantic cues on word retrieval with the therapy sessions. Conclusion: Responsiveness to cues in picture naming assessment might provide a positive prognostic indicator for treatment. There was an improvement in the pre & post rehabilitation scores regarding naming objects, naming actions and spontaneous picture description among the two groups using phonemic & semantic cueing therapy.

Behavioral Temporal Auditory Processing Tests in Adult Stutterers 成人口吃者行为时间听觉加工测试

Purpose: To assess the temporal processing function in adults stutterers and compare the results with non-stutterers. Patients and Methods: Eighty adults: forty with stuttering as the study group and forty without stuttering as controls, whose age ranged between 18-45 (27.7±7.53) and 18-55 (29.88±10.56) respectively, were compared regarding selective behavioral temporal auditory processing tests: Pitch Pattern Sequence Test (PPST), Duration Pattern Test (DPT), Time Compressed Sentences Test (TCST) for adults and Auditory Fusion Test-Revised (AFT-R). The severity of stuttering was evaluated by using Stuttering Severity Instrument (SSI-3). Results: Scores of the AFT-R was statistically significantly higher (worse) in the study group than the control group in both ears and at all frequencies. PPST, DPT and TCST showed statistically significant lower values (worse) in the study group than the control group in both ears. No correlation was found between stuttering severity assessed by (worse) index score and either AFT-R, PPST, or DPT, while there was a statistically significant negative correlation between stuttering severity and TCST at 40%, and 60%

Endoscopic versus microscopic stapedecomy: Advantage and outcome 内镜下与显微下镫骨切除术:优势和结果

Objective: To evaluate the surgical outcomes of endoscopic stapedectomy, comparing the results with a conventional stapes surgery under microscopic approach. Patients and Methods: This study was carried out in Otorhinolaryngology Head and Neck Surgery department of both Benha University Hospitals and Hearing & Speech Institute upon 30 patients divided into two groups, each consists of 15 patients, Group A underwent endoscopic stapedectomy, Group B underwent microscopic stapedectomy from June 2019 till March 2020 to evaluate the advantages of endoscopic stapedectomy in comparison to microscopic one. Results: We reported that 14 patients (93.3%) of the endoscopic group showed improvement in hearing, while only one patient (6.7%) complained of recurrent hearing impairment at the 6 th month follow up after lifting heavy object while hearing was very good at 1 st , and 3rd months follow up. All patients (100%) of the microscopic group showed improvement in hearing at 1 st , 3 rd and 6 th months post-operative with nearly closed ABG. Conclusion: Endoscopic surgery is particularly suitable for stapedial disease. Endoscopic stapes surgery can be performed despite a curved and narrow external auditory canal and is minimally invasive, being characterized by little drilling and almost no postoperative pain.
目的:评价内镜下镫骨切除术的手术效果,并与常规镫骨镜下手术效果进行比较。患者与方法:本研究于2019年6月至2020年3月在滨海大学附属医院耳鼻喉头颈外科和听力语言研究所开展,将30例患者分为两组,每组15例,A组行内镜下镫骨切除术,B组行内镜下镫骨切除术,比较内镜下镫骨切除术与显微镜下镫骨切除术的优势。结果:内镜组14例(93.3%)患者听力改善,仅1例(6.7%)患者在举重后随访6个月听力复发,随访1、3个月听力均良好。显微镜组所有患者(100%)术后1、3、6个月听力均有改善,ABG接近闭合。结论:内镜手术特别适用于镫骨疾病。尽管外耳道弯曲狭窄,内窥镜镫骨手术仍然可以进行,并且微创,其特点是钻孔少,几乎没有术后疼痛。
