Exploring the potential of nurses in the delivery of care for people living with chronic conditions 探索护士在为慢性病患者提供护理方面的潜力

People living with one or more chronic conditions is becoming an everyday occurrence. According to a recent report by the World Health Organization (WHO, 2013), 65% of all global deaths are attributable to chronic illness, with nearly 40% of those deaths being in people between the ages of 30 and 70 years old. Concomitantly, the cost of caring for people living with chronic conditions has placed an ever-increasing burden on health care budgets. This is because long term care requires the ongoing support of multiple services. The challenge for health services is to provide the optimum care possible for the least cost. To do this, one must understand that care is no longer just disease focused. Many health services lack the capacity to deal with such complex patients, with fragmentation of care commonplace, resulting in increased admissions and reduced capacity for patients to manage their condition, or have input into their care (Sheridan et al., 2015). The objective is to find new models of care that support an integrated approach to care that is communitybased and addresses the whole-person needs (Boehmer et al., 2018).

Pain assessment in surgical patients in the South Bohemia region 南波希米亚地区外科患者的疼痛评估

Aim: The aim of the study was to describe the current state of pain assessment in surgical patients in the South Bohemia region. Design: A quantitative descriptive study. Methods: Two sets of questionnaires were used to determine the state of pain assessment. The sample included 253 nurses and 205 patients from the South Bohemia region. Results: Nurses use visual analog scales (129) and verbal pain assessment (89) to assess pain. A more precise verbal description of pain assessment would be appreciated by 44 of the respondents, and 42 would appreciate confirmation of assessments by doctors. According to nurses, the main pain manifestations in patients are: limited mobility (156), verbal expressions (134), lack of cooperation (129) and limited independence (128). Direct physical manifestations of pain mentioned by patients include: limited mobility (140), limited independence (97), and mimic manifestation (46). Patients would like pain assessments complemented by more accurate verbal assessments (84), use of pictures to illustrate pain (40), and increased frequency of assessment by doctors (29). Conclusion: The data obtained suggest some shortcomings in pain assessment, and indicate the spheres in which provision of care could be improved.

Factors affecting nursing students' attitudes towards violence against women: a cross-sectional study 影响护生对妇女暴力态度的因素:一项横断面研究

Aim: The aim of this study was to determine nursing studentsʼ attitudes towards violence against women (VAW) and factors that might influence these attitudes, in particular, those related to their sociodemographic characteristics, previous exposure to violence, and knowledge of violence. Design: A cross-sectional study. Methods: The study sample consisted of 284 nursing students. The data were collected using a student information form and a standardized tool: the Attitude towards Violence against Women Scale (ATVAWS). Results: Students who were female, students who had graduated from vocational schools of health, students whose mothers had had a medium-level of education, and students who were interested in receiving information on VAW had attitudes to VAW that were more progressive (p < 0.05). In contrast, students who had received education on VAW, students who had witnessed or been subjected to domestic violence, and students who had experienced violence from their partners had attitudes to VAW that were more conservative (p < 0.05). Conclusion: Factors affecting students’ ATVAWS mean scores were gender, type of school graduated from, mother’s level of education, education on VAW, willingness to learn about VAW, witnessing of domestic violence, and previous exposure to domestic violence and to violence from partners. Courses on VAW should be led by instructors with sufficient knowledge and experience of the subject.

Socio-demographic and pregnancy-related characteristics associated with prenatal distress: a Turkish study 与产前窘迫相关的社会人口统计学和妊娠相关特征:一项土耳其研究

Aim: This study aimed to determine socio-demographic and pregnancy-related characteristics associated with prenatal distress. Design: Cross-sectional study. Methods: This cross-sectional study was carried out at the obstetrics and gynecology clinics of the Maternity and Children’s Hospital, Giresun, Northern Turkey (n=230). The data were collected using the Demographic Information Form, Insomnia Severity Index, and Prenatal Distress Scale. The Kruskal Wallis test and the Mann Whitney U test were used to compare characteristics. Bivariate correlation analysis was conducted to investigate the influencing factors of prenatal distress. Results: Prenatal distress levels of pregnant women were low (10.27 ± 5.02). Prenatal distress was affected at the level of statistical significance by employment status (Z = -1.976; p = 0.048), income status (X2 = 6.568; p = 0.037), and history of stillbirth (Z = -2.398; p = 0.016). Nausea, vomiting, and insomnia during pregnancy were determined as significant variables that increase prenatal distress level. Conclusion: The results for this Turkish population were consistent with the previous literature on prenatal distress and influencing factors of pregnant women. Evaluation of prenatal distress level in pregnancy follow-up is necessary to prevent the development of mental illness. Measures to prevent medication errors in intensive care units 预防重症监护室用药错误的措施

Aim: The aims were to summarize studies on the effectiveness of interventions to reduce adverse events of the medication error type and, based on the studies, to identify recommendations for preventing medication errors in intensive care units (ICUs). Design: A descriptive review. Methods: To find relevant resources, the SCOPUS and EBSCO electronic databases were searched using the following search words: prevention, medication errors, intensive care unit. Both primary and secondary studies on prevention of medication errors in ICUs were selected. Results: A total of eight primary studies and three systematic reviews were included. The studies showed considerable variability and differed in methods, numbers of monitored events or ways of data collection. The assessed interventions were pharmacist involvement, automated infusion devices, reporting medication errors, strategies to limit interruptions during drug administration, electronic health records together with support systems for clinical decision making, nurse education in drug administration and creating checklists. Conclusion: The assessment of selected studies suggests that to a certain extent, all of them showed certain medication error reduction. Due to numerous limitations, however, it is impossible to select and recommend a single approach.

The effect of educational intervention by nurses on home environmental risk factors for falls 护士教育干预对跌倒家庭环境危险因素的影响

Aim: The purpose of this study was to assess the incidence of environmental safety hazards in the homes of people aged 65 years and above, and to evaluate the effect of an educational intervention by nurses on the incidence of home environmental risk factors for falls, and on the perception of the safety of the home environment. Design: The study was a non-randomized single-arm pre-post intervention study. Methods: The sample consisted of 106 elderly patients living in their homes in the Czech Republic. The intervention involved individual education about home hazards, including the provision of a free educational leaflet and emergency call sticker, and advice regarding home environment modification and safety devices. Results: A statistically significant difference was found in the number of risk factors before and after the intervention (p < 0.001). After the intervention the elderly subjects perceived their home environment as being less safe than they had before. Conclusion: This study demonstrates that home environmental risk factors for falls can be reduced by a well-prepared educational intervention.

Results: A statistically significant difference was found in the number of risk factors before and after the intervention (p < 0.001). After the intervention the elderly subjects perceived their home environment as being less safe than they had before. Mental health nursing in Europe: a global challenge, in Czech Republic too 欧洲的心理健康护理:捷克共和国面临的全球性挑战

The European Mental Health Action Plan 2013–2020 summarizes in a very practical way the goals, values and vision that incorporate the major challenges for mental health in Europe (justice, empowerment of patients and families). This commitment to de-institutionalization and the development of community-based mental health services remains alive across Europe. (WHO, 2015) Reality also shows that the social stigma associated with mental illness is a monster that is difficult to fight and that the resulting discrimination reduces the demand for mental health care.

Compliance with the principles of the perioperative safety process in the context of the work of perioperative nurses 在围手术期护士工作的背景下遵守围手术期安全流程的原则

Aim: The main aim was to describe the current state of perioperative safety processes with reference to perioperative nurses' work. Design: To achieve the stated goal, a custom-built "ad hocˮ research design was created, using a combination of qualitative and quantitative methods. The findings of the exploratory qualitative survey were verified by the quantitative survey focused on in this article. Methods: The exploratory survey and Surgical Safety Checklist were the basis for a questionnaire which was designed to be used as a tool for investigation among a set of perioperative nurses in Czech hospitals. The target group of respondents was operating room managers, one representative per healthcare institution. The obtained data were analysed by exploratory statistical methods. Results: More than 96% of the healthcare institutions included in the research sample have formally established procedures: patient identification, verification of the type and side of the operation, checks for patient's history of allergies, and checks of the number of medical devices (n = 68). The survey showed that the respondents perceive safety checks to be of utmost importance in the perioperative procedure (modal value of seven on a scale from one to seven). Conclusion: Using the Surgical Safety Checklist should lead to safety improvements in nursing perioperative care. In most cases, the nursing safety practices formally introduced at respondents' workplaces correspond to WHO recommendations in the form of this Checklist.
