Effect of chlorhexidine gel on the prevention of alveolar osteitis and post‐operative pain following transalveolar extraction of impacted mandibular third molars: A randomized study 氯己定凝胶对下颌阻生第三磨牙经牙槽拔牙后牙槽骨炎和术后疼痛的预防作用:一项随机研究

Abstract Objective This study aimed to compare the efficacy of 0.2% chlorhexidine gel with warm saline mouth rinse (WSMR) in the prevention of alveolar osteitis (AO) following transalveolar impacted mandibular third molar (IMTM) extraction after accounting for surgical operative confounding factors. Materials and Methods We conducted a parallel design randomized study. Participants were consenting adults randomly assigned to 0.9% WSMR and 0.2% chlorhexidine gel (administered twice daily post‐operatively) groups. All participants were followed up daily post‐operatively for one week. The primary outcome was an incidence of AO, assessed using Blum's criteria. Fischer's exact tests and regression models were used to investigate the effect of interventions on AO. Results One hundred participants were enrolled in the study, with 50 participants in each group. A higher incidence of AO was observed in the WSMR group than the chlorhexidine group (16% vs. 2%, p = 0.01). However, after adjusting for duration of extraction (aOR: 0.18; 95% CI: 0.02, 1.71) and bone operating time (aOR: 0.16; 95% CI: 0.02, 1.47), the difference was not statistically significant. Conclusions In conclusion, the study demonstrates that the application of the 0.2% chlorhexidine gel was not significantly more effective in preventing AO compared with the use of WSMR following transalveolar IMTM extraction after controlling for operative factors.

Comparative evaluation of use of apexo elevators versus crane pick elevators in extraction of mandibular posterior root stumps 下颌后根残端拔除中使用尖部吊具与起重机吊具的比较评价

Abstract Purpose The crane pick elevator is a solid version of the pick‐type dental elevator that rolls the root out of the socket using the wheel and axle method. The traditional apexo elevator works on the wedge principle, wherein it helps to retrieve root stumps below the gingival line. Although most of the extractions can be performed with routine techniques and instruments, there is a constant urge to improve these conventional procedures with newer instruments and techniques. Thus, we intend to do a comparative study, evaluating the efficiency of apexo elevators versus crane pick elevators in the extraction of mandibular root stumps. Patients and Methods This was a comparative in vivo study conducted on a total of 68 patients aged 18 years and above. They were divided into two groups of 34 each using a random number generator. The patients were evaluated on the basis of their gender, whether extraction was successful, time taken for extraction, operators' difficulty, root length and the tooth that was extracted. Results Majority of the participants were males in the apexo group (61.7%) and females in the crane pick group (55.8%). InAU terms of successful extraction, it was seen to be more with apexo elevators compared to crane pick elevators and is statistically significant (0.04*). The operator encountered greater difficulty during the removal of root stumps with crane pick elevators. Conclusion We concluded that crane pick elevators are useful in luxating apical one third of the root when fractured. However, apexo elevators are better when it comes to delivering the tooth out of the socket.

An audit of post‐operative complications following oral surgery procedures over a 3‐month period at the Eastman dental hospital, London 对伦敦伊士曼牙科医院3个月口腔手术后并发症的审计

Abstract Objective The objective of this paper was to evaluate the rate of post‐operative complications following Oral Surgery procedures at the Eastman Dental Hospital. Dento‐alveolar surgery procedures can lead to complications including pain, swelling, bleeding, infection and numbness amongst others. Although many are considered minor and resolve swiftly, it is important as clinicians to have an understanding of the incidence of complications prior to undertaking surgical treatment to allow evaluation of the risks and benefits. Materials and Methods Electronic patient records were reviewed across a 3‐month period from February to April 2022 for every patient receiving treatment under local or general anaesthetic and intravenous sedation. Patients returning for an unplanned appointment with a problem related to their treatment was classified as a “post‐operative complication”. Information was recorded regarding patient demographics, treatment type and modality, complications, management and outcome. Results were analysed using Microsoft Excel. Results Nine hundred and ninety four patients received treatment in total. The overall complication rate was 6.33%. The treatment modality with the highest incidence of complications was general anaesthetic. Third molar extractions resulted in the highest proportion of complications out of the procedures considered. The complication with the highest incidence was post‐operative pain and swelling (2%), followed by dry socket (1.5%) and post‐operative infection (1.4%). Conclusion Post‐operative complications can occur following a range of procedures and can lead to significant morbidity. Clinicians should consider the likelihood of complications occurring when planning treatment as well as early recognition. Future auditing could consider medical co‐morbidities of patients and potential influences of this on developing complications.

Abstract Aim and objectives A prospective split mouth study was done to compare the efficacy of transdermal ketoprofen versus diclofenac patch after therapeutic orthodontic tooth extraction based on four parameters – pain intensity (VAS), number of rescue analgesia intakes, local allergic reactions and patient tolerability for immediate and 3 consecutive postoperative days. Materials and methods In total, 33 (66 extraction sites) patients were divided into the ketoprofen patch group and the diclofenac patch group. A single ketoprofen patch (70 cm 2 30 mg) was applied following extraction of premolars of the 1st and 4th quadrants and patients were recalled after 1 week for extractions of premolars of the 2nd and 3rd quadrants and diclofenac patch (50 cm 2 100 mg) was applied on the deltoid region of the right arm, respectively. Patients were then evaluated under the above‐mentioned parameters. Results Ketoprofen transdermal patch was found to be more effective in terms of controlling postoperative pain and the minimum number of rescue analgesia tablets taken as compared to diclofenac (statistically significant on 1st and 2nd day) though the former showed the incidence of a local allergic reaction ( p ‐value 0.002, 0.002 and 0.012), respectively, in few cases which were not seen in case of diclofenac. Conclusion Overall, both the transdermal patches are effective in terms of safety and providing analgesia, though more research are needed in proving the safeness and efficiency of these transdermal patches with large sample size.
