Prophylactic Use of Levosimendan in High-Risk Coronary Artery Disease: Retrospective Clinical Research 左西孟旦预防高危冠状动脉疾病的回顾性临床研究

ABS TRACT Objective: To investigate the perioperative and postop- erative effects of preoperative levosimendan use in coronary artery patients with low ejection fraction (EF) (<40%). Material and Methods: A total of 6,571 coronary bypass patients who were treated at Kartal Ko ş uyolu High Specialization Training and Research Hospital between 2016 and 2019 were scanned, and 132 patients with preoperative EF below 40% were retrospectively analyzed. The patients were divided into 2 groups as those who received preoperative levosimendan (Group 1: 90 patients) and the control group, who did not receive it at all (Group 2: 42 patients). All of the patients included in the study were on- pump coronary bypass patients and excluded off-pump bypass, emergency bypass, and other concomitant procedures. Results: In terms of EF values, improvements over time were significantly different in favor of Group 1 (p<0.043). The duration of the intensive care unit stay in Group 1 was longer (p<0.047). The changes in patients in terms of postoperative venous oxygen saturation over time were found to be statis- tically positive in favor of Group 1 (p<0.042). Mortality was observed in 5 patients in the postoperative early period (first 30 days), all of whom were from Group 1. However, no statistical difference was observed. Conclusion: Preoperative use of levosimendan in coronary artery patients with low EF improved some hemodynamic parameters; however, it did not make a significant hemodynamic and clinical contribution to surgery success in high-risk patients.

Determination of Spiritual Well-Being and Hope Level of Patients Undergoing Open Heart Surgery: A Descriptive Study 心内直视手术患者精神幸福感和希望水平的测定:一项描述性研究

ÖZET Amaç: Araştırma, açık kalp ameliyatı olacak hastaların spiritüel iyi oluş ve umut düzeylerinin belirlenmesi amacıyla tanımlayıcı türde yapılmıştır. Gereç ve Yöntemler: Araştırmanın örneklemini Sağlık Bilimleri Üniversitesi Kartal Koşuyolu Yüksek İhtisas Eğitim ve Araştırma Hastanesinde açık kalp ameliyatı olmak için yatan 166 hasta oluşturmuştur. Araştırmanın verileri; hasta tanıtım formu, Herth Umut İndeksi, Spiritüel İyi Oluş Ölçeği ile toplanmıştır. Verilerin analizinde tanımlayıcı veriler sayı yüzdelik hesaplamaları, Kolmogorov-Smirnov testi, tek yönlü ANOVA, bağımsız gruplar için t-testi, Kruskal-Wallis testi ve Pearson korelasyon analizi kullanılmıştır. Bulgular: Araştırmaya katılan hastaların %72,3’ünün 51 yaş ve üzeri yaş grubunda, %69,3’ünün erkek, %45,3’ü ilkokul mezunu, %84,9’unun evli, %51,2’sinin baypas ameliyatı geçireceği, %84,3’ünün daha önce kalp ameliyatı geçirmediği, %41’inin 4-6 gündür hastanede kaldığı belirlenmiştir. Hastaların Spiritüel İyi Oluş Ölçeği toplam puanının 115,93±10 ve Herth Umut İndeksi toplam puanının 39,11±5,40 olduğu saptanmıştır. Araştırmaya katılmaya gönüllü hastaların tanıtıcı özelliklerine göre Spiritüel İyi Oluş Ölçeği’nden aldıkları puanlar arasında istatistiksel olarak anlamlı farklılık bulunmamıştır (p>0,05). Araştırma kapsamındaki hastaların eğitim durumu, daha önce kalp ameliyatı geçirme durumu ve hastanede kalış süresine göre Herth Umut İndexi’nden aldıkları puanlar arasında istatistiksel olarak anlamlı farklılık belirlenmiştir (p<0,05). Sonuç: Araştırmada, hastaların spiritüel iyi oluşlarının ve umut düzeylerinin yüksek olduğu, spiritüel iyi oluş ile umut düzeyi arasında ilişki olmadığı sonucuna ulaşıldı.

Percutaneous Aortic Stent Implantation in an Adult Patient with Interrupted Aortic Arch 成人主动脉弓中断患者经皮主动脉支架植入术1例

30 Interrupted aortic arch (IAA), an uncommon malformation of the aortic arch, is defined as a loss of luminal continuity among the ascending and descending portions of the aorta. The incidence of IAA among congenital heart defects is around 1%.1 There are 3 types of the IAA, and they are classified according to the site of the interruption by Celoria and Patton.2 Type A is interrupted distal to the left subclavian artery (LSCA); Type B is interrupted between the left subclavian and left carotid artery (LCA), and Type C is interrupted proximal to the LCA. The frequency of occurrence is listed as B, A, and C, respectively. Survival into adulthood with the isolated anomaly is very rare and depends on the development of collateral circulation. Extra anatomic bypass procedures are the most preferred reconstruction method in adult patients with IAAs.3

Objective: Coronavirus disease-2019 (COVID-19), caused by a novel coronavirus, has become a worldwide pandemic. In order to control the global spread of this contagious disease, elective surgeries including cardiac and vascular procedures were postponed. In this article we present our experience during the initial phase of the pandemic in view of safety protocols. Material(s) and Method(s): The study included all elective, urgent, and emergent procedures that were performed from March 11, 2020 to June 30, 2021 at the department of cardiovascular surgery. Patients' demographics, preoperative COVID-19 (reverse transcription-polymerase chain reaction) test results, surgical procedures, complications and outcomes were prospectively collected. We also developed our own protocol to proceed with our surgical activity without delay. Result(s): A total of 86 cardiac and vascular procedures were performed during the study period. The median age was 64 years (range 23-79), 59 (69%) were males. There were 42 (48%) elective procedures, and 13 (15%) patients had emergency procedures. 57 (66%) patients underwent coronary artery bypass graft surgery. 10 (12%) patients underwent valvular procedures. Peripheral vascular surgeries were performed in 18 (21%) patients. The median length of stay in the hospital was 9 days (range 1-60). A positive COVID-19 test was identified in 4 patients, two of them were detected in the immediate postoperative period and died. In-hospital mortality was 9%. Conclusion(s): Cardiovascular procedures could be performed safely with a relatively low risk during the outbreak, particularly for elective patients with proper management and strict infection control and isolation protocols.Copyright © 2022 by Turkiye Klinikleri.
