This paper presents a case of urosepsis caused by Escherichia coli in an 11-year-old girl with a neurogenic bladder. The patient had not previously been supervised by a nephrologist or urologist. On admission, significantly elevated serum markers of inflammation (C-reactive protein 32.4 mg/dl, procalcitonin 64.2 ng/ml), kidney failure (creatinine 6.9 mg/dl, pH 7.22, HCO3 6.3 mmol/l), and increased serum urea (399 mg/dl) were detected. Antibiotic therapy with rehydration was started and improved renal function without dialysis. However, a 9-cm-long perirenal abscess site was diagnosed. It required surgical treatment, which was complicated by an anaphylactic reaction to latex. Colitis caused by Clostridium difficile was also diagnosed. Hospital treatment lasted 38 days. After 4 months the patient’s glomerular filtration rate was 66 ml/min/1.73 m 2 and indicated stage 2 chronic kidney disease. Introduction: Vedolizumab (VDZ) is effective in the induction and maintenance of remission in adults with inflammatory bowel disease (IBD). Pediatric data are still limited. This retrospective cohort study aimed to assess the safety and efficacy of VDZ in pediatric IBD including very-early-onset IBD (VEO-IBD). Material and methods: A review of pediatric IBD patients receiving VDZ was conducted. Laboratory parameters, nutritional status, and disease activity scores were compared between each follow-up visit and between two groups divided by age of disease onset – VEO-IBD (age of onset < 6-years-old) and non-VEO-IBD (age of onset ≥ 6 years < 17 years). The primary outcome was clinical response after induction therapy (at 4 th dose visit). The secondary outcome was clinical remission after induction (at 4th dose visit) and maintenance phase (at 10th dose visit). Statistical considerations were included. Results: Seventy-two patients with pediatric IBD were included: 12 with Crohn disease (CD), 60 with ulcerative colitis (UC). The definition of VEO-IBD was met by 21 patients. After the induction phase, a clinical response was observed in 60/72 (83.3%) patients (51/60 with UC and 9/12 with CD) and clinical remission in 44/72 (61.1%) patients (40/60 with UC and 4/12 with CD). Clinical remission after the maintenance phase was achieved by 22/72 (30.6%) patients (16/60 with UC and 6/12 with CD). Improvement in the patients’ laboratory parameters and nutritional status was observed. No significant differences were observed in VDZ response between VEO-IBD and non-VEO-IBD. Conclusions: Vedolizumab was safe and effective in the treatment of pediatric IBD irrespective of age of disease onset.

There are many types of foreign bodies found in soft tissues in children. Ultrasound is a very useful tool in diagnosis. The article presents a series of cases – children with foreign bodies located in soft tissues – in which the diagnosis was confirmed with the use of ultrasound. Different features of different foreign bodies can provide information about the foreign body type (gravel, glass, wooden splinter, metal). The foreign bodies could be successfully removed under local anesthesia. Ultrasound can be used to confirm the presence of soft tissue foreign bodies in suspected patients. Moreover, the features of the image can provide information about the foreign body type. In the pediatric population, the main advantage of ultrasound is the reduction of X-rays performed.

Reversible pulmonary artery banding (rPAB) was proposed as an innovative therapy for young children with end-stage heart failure due to dilated cardiomyopathy (DCM). We report our early experience with rPAB in 2 infants with DCM and preserved right ventricle (RV) function to evaluate the effectiveness of this treatment. Pre-and postoperative echocardiography included standard evaluation of right (RV) and left (LV) ventricle global systolic function. Moreover, we applied the LV global longitudinal strain for better analysis of LV systolic function. During 15 months of follow-up, clinical and echocardiographic improvement was observed. Our early experience indicates that rPAB may be an effective alternative to heart transplantation or recovery in selected children with DCM
