Gastrointestinal symptoms of COVID-19 in children 儿童COVID-19胃肠道症状

Introduction: The COVID-19 pandemic progresses. The clinical manifestation of the disease and the severity of its course vary significantly. There are considerable differences between symptoms of SARS-CoV-2 infection in the child and adult populations. The gastrointestinal (GI) symptoms are an essential element in understanding the pathophysiology of the disease and in drawing conclusions concerning the diagnostic, therapeutic, and epidemiological management of COVID-19. The aim of the study was to characterize the gastroenterological symptoms of COVID-19 in the paediatric population and to find differences in the course of the disease between paediatric patients with and without GI symptoms of COVID-19. Material(s) and Method(s): We report the clinical characteristics of 321 children with COVID-19 (age 0-215 months) hospitalized between March 2020 and April 2021. The following division was used when processing the data: the first wave of cases in Poland lasted from the beginning of the pandemic to June 2020, the 2nd wave September-November 2020, and the 3rd wave February-May 2021. We specifically compared the differences between patients with and without GI symptoms. Result(s): Among all included patients, 95 (29.5%) had GI symptoms - the most common included abdominal pain (15.27%) and diarrhoea (14%). Approximately 3% of patients with GI symptoms required surgical intervention. As the pandemic progressed, GI symptoms were reported with increasing frequency - during the first wave 9%, the second wave 25%, and the third wave 38%. Patients with GI symptoms had more frequent and statistically significantly higher inflammatory parameters. During treatment, GI patients more often required the administration of antibiotics. The most common abdominal ultrasound abnormalities were liver enlargement, a slight amount of free fluid in the peritoneal cavity, and moderately enlarged individual lymph nodes. Conclusion(s): Gastrointestinal symptoms form an image of COVID-19, which is a possible prognostic risk factor for severe course of the disease. Skeletal and dental abnormalities in patients with sex chromosome aberrations: a systematic case-based review 性染色体异常患者的骨骼和牙齿异常:一项系统的基于病例的回顾

Sex chromosome aneuploidies (SCAs) are common chromosomal disorders characterised by an atypical number of sex chromosomes. Turner syndrome (TS), Klinefelter syndrome (KS), and Jacobs syndrome (JS) are associated with a wide spectrum of skeletal manifestations, including craniofacial and limb anomalies. This systematic review aimed to analyse the incidence of skeletal abnormalities in selected SCAs based on case reports. In this review, 55 articles were included from the MEDLINE/PubMed and Google Scholar databases, according to PRISMA guidelines. High-arched palate, skeletal class II, and cubitus valgus were most frequently demonstrated among TS patients. Patients with KS and JS most often presented micrognathia, hypertelorism, and flat nasal bridge in the craniofacial region. In contrast, radioulnar synostosis, clinodactyly, and pes planus could be observed in the limbs of KS patients. The presence of dysmorphic facial features and limb malformations may indicate SCAs, which are underdiagnosed in the general population due to a variety of phenotypes.

A 15-year-old boy with travel-imported dengue haemorrhagic fever 一名患有旅行输入性登革出血热的15岁男童

Dengue is a viral, mosquito-spread disease with a high prevalence in tropical and subtropical areas. We report a case of travel-imported dengue haemorrhagic fever in a 15-year-old Polish boy. The patient presented with fever, headache, and muscle pain. He was initially diagnosed with influenza. He gradually developed skin rash, mild hepatosplenomegaly, episodic gingival bleeding, and mild symptoms of plasma leakage in the form of palms swelling. The laboratory results revealed leukopaenia and thrombocytopaenia with increasing haematocrit. Dengue virus infection was confirmed by simultaneous detection of NS1, IgM, and IgG antibodies. The patient was treated empirically in the critical phase with satisfactory outcome. The symptoms subsided and the platelet and leukocyte levels gradually reached the normal range for his age.

Desirable and undesirable lifestyle changes in Polish children resulting from the COVID-19 pandemic COVID-19大流行导致波兰儿童生活方式的理想和不理想改变

Introduction: This study hypothesized that the lockdown due to the COVID-19 pandemic implemented in Poland on 11 March 2020 negatively influenced the lifestyle, dietary and sleeping habits, and physical activity of Polish children. The study aimed to assess the change of children's lifestyle habits, and screen and sleep time due to measures taken during this lockdown. Material(s) and Method(s): The study was cross-sectional and involved 486 parents of children aged 3-18 years, who completed the questionnaire containing 37 questions related to sociodemographic factors, nutrition, physical activity, and screen and sleep time regarding the child before and during lockdown. Result(s): During the lockdown, the number of meals consumed daily increased significantly in children between 7 and 11 years old (0.19 +/-0.68, p < 0.001) and 12 and 18 years old (0.24 +/-0.81, p = 0.005), whereas it decreased in the youngest group (3-6 years old) (-0.11 +/-0.66, p < 0.001). The declared consumption of fast food decreased in all groups (p < 0.001), whereas the number of responders who prepared meals at home increased significantly (95.9% vs. 98.4%, p = 0.02). The time of physical activity decreased significantly during the lockdown in the groups of older children (p < 0.001), especially in children who live in blocks of flats. In the youngest group of children, the declared time of physical activity did not change significantly. In all groups of children, the screen time increased significantly during the pandemic. The amount of sleep also increased in all age groups but its quality worsened. Conclusion(s): It can be concluded that the lockdown due to the COVID-19 pandemic generated mainly undesirable lifestyle changes (decreased physical activity, increased screen time);however, desirable effects (increase in meals eaten at home and amount of sleep) were also observed. Breastfeeding during the COVID-19 pandemic COVID-19大流行期间的母乳喂养

The SARS-CoV-2 coronavirus pandemic significantly influenced aspects of children's nutrition, in particular breastfeeding and the percentage of breastfeeding mothers. During the pandemic, changes in postnatal care have occurred and may result in a global decline of the breastfeeding initiation rate and problems with longterm maintenance of lactation. Mother's milk is not a source of viral infection for a child;on the contrary, due to the presence of antibodies specific to the SARS-CoV-2 virus, it gives the possibility of acquiring passive immune protection. Currently, breastfeeding requires special care and protection. Emphasizing the benefits of human milk should become a priority in obstetric, neonatal and pediatric departments.
SARS-CoV-2冠状病毒大流行严重影响了儿童营养的各个方面,特别是母乳喂养和母乳喂养母亲的百分比。在大流行期间,产后护理发生了变化,可能导致全球开始母乳喂养率下降,并出现长期维持哺乳的问题。母乳不是儿童感染病毒的来源;相反,由于存在针对SARS-CoV-2病毒的抗体,母乳有可能获得被动免疫保护。目前,母乳喂养需要特别的照顾和保护。强调母乳的好处应该成为产科,新生儿和儿科部门的优先事项。
