Botulinum Neurotoxin A for Hand Tremors in Parkinson’s Disease: A Meta-Analytic Study 肉毒杆菌神经毒素A治疗帕金森病患者手部震颤:一项荟萃分析研究

Background: Resting tremor is a prominent cardinal motor symptom of Parkinson’s disease (PD). In some cases, the tremor may be refractory to dopaminergic and anticholinergic treatment. Multiple studies were previously done to evaluate the effectiveness of Botulinum Neurotoxin A (BoNT/A) with essential tremors and dystonia, but data regarding its use on tremors of PD is still lacking. Objective: This meta-analytic study aims to determine the effectiveness of BoNT/A in treating tremors of patients with PD. Data Sources: Researches were searched at PubMed, ScienceDirect and EBSCO Host. Review Methods: Articles on the effect of BoNT/A on PD hand tremors were searched. Studies and data pertaining to non-PD tremors like essential tremors excluded in the analysis due to difference in pathophysiology. Standardized mean difference was used as the effect measure and was computed with Review Manager version 5.4 software. Results: Three open label studies were used for final analysis in this study. Studies included are those pertaining to tremors due to PD. Pooled estimates showed a significant change in decreasing tremor score after BoNT/A injection. Telemedicine has been an emerging method for delivering healthcare services due to the challenges brought by the COVID-19 pandemic. This descriptive quantitative correlational study is aimed at assessing the knowledge, attitudes and perceptions (KAP), and their relationship among adults aged 18-34 without prior experience in the use of telemedicine in Manila, Philippines during the COVID-19 pandemic. The study had 322 eligible respondents who answered an online survey questionnaire of three parts that asked for their socio-demographic profile, KAP on telemedicine. Descriptive statistics, profile analysis and Spearman’s rho were utilized as statistical tools. The respondents’ knowledge was on an average to low level, and attitude and perception towards telemedicine were both neutral. Results showed no significant difference between the KAP of the respondents when analyzed according to socio-demographic variables. Knowledge and attitude have a weak positive linear relationship while knowledge and perception have a moderate positive linear relationship. A strong positive linear relationship was indicated between attitude and perception. A positive attitude can be attributed to a positive perception towards telemedicine but both do not consequently come from high knowledge levels of it. No pain in academic and research endeavours, no gain in university rankings. Let us look at the gain first. Any university worth its salt has a high caliber faculty who are experts in their fields of study and are effective teachers or mentors to their students. Their effectiveness in teaching are mostly informed by their continuing research activities and strong academic performance of students. As more faculty publish their studies, the higher the probability of citations. Reputation and university rankings elevate with academic and research excellence of its faculty and students. Improved university rankings are likely to translate into opportunities for larger international research collaborations and funding. As more resources become available for research, higher is the chance to participate in ground-breaking studies.

Safe, Nourishing, Accessible Community Kitchen (S.N.A.C.K.): A Community Kitchen Manual for Public Health Emergencies 安全、营养、方便的社区厨房(S.N.A.C.K.):紧急公共卫生事件的社区厨房手册

Objective: The project aims to develop a community kitchen manual for public health emergencies. Participants: The officers and members of the Samahan ng Nagkakaisang Kababaihan ng BASECO served as community respondents for this project. These eight women were in charge of the community kitchen operations, particularly the feeding program during the COVID-19 pandemic. Implementation: The whole project comes in three phases. The design stage was executed in phase one of this project. The phase one also focused on the development of manual content based on available literatures and considered the interviews conducted with respondents. Discussion: The manual contains guidelines on food safety and meal preparations. Based on the interview, the proponents identified specific contents that must be included in the guidelines, as compared with the need to improve current operations of the group’s community kitchen. Looking at the activities of the community, the proponents indicated the food flow guidelines to ensure a safe, nourishing and accessible community kitchen (S.N.A.C.K). Conclusion: The designed community kitchen manual may be of use to lay groups putting up community kitchens, since there is no existing manual in the Philippines. However, phases two and three must be conducted to validate its usefulness and accuracy in the field. Strengthening Nutrition Knowledge of Pregnant Women Through Nutrition Education During Public Health Emergencies 突发公共卫生事件中通过营养教育加强孕妇营养知识

Proper maternal nutrition evidently results in good pregnancy outcomes both for the mother and born infant. However, the process leading to good nutrition during pregnancy remains a challenge most especially during public health emergencies, such as the COVID-19 pandemic, which brought limitations on access to a variety of foods as well as nutrition services. Thus, nutrition education through the provision of contextualized information, education and communication (IEC) materials was explored as an alternative delivery strategy. The delivery strategy aims to increase the number of pregnant women in Barangay Palanan, Makati City, Philippines who have adequate nutrition knowledge through IEC materials-based nutrition education that is contextualized during public health emergencies. As an outcome, the delivery strategy intends to increase the number of pregnant women who have normal nutritional status as determined by weight per month of pregnancy. Results in determining the adequacy of nutrition knowledge through pre- and post-testing show that there is a 10% increase in the passing rate from 59% at baseline to 69% at endline. From the number of pregnant women who have taken the post-test, a remarkable 52% have either post-test scores that are maintained or improved. Meanwhile, for pregnant women who have normal nutritional status, an increase of 5% was seen given by 46% at baseline to 51% at endline. Ultimately, IEC materials-based nutrition education may serve as a supplementary approach in the continuous provision of nutrition services to attain good nutrition of pregnant women amid the COVID-19 pandemic.

Effectiveness of Bacille Calmette-Guerin Vaccination Policies in Reducing Infection and Severity of COVID-19: a Systematic Review Protocol 卡介苗- guerin疫苗接种政策对降低COVID-19感染和严重程度的有效性:一项系统评价方案

Introduction: The bacille Calmette-Guerin (BCG) vaccine is used for the prevention of tuberculosis (TB) worldwide. Evidence reports a much lower incidence of COVID-19 in TB-endemic areas implying a possible protective mechanism of BCG in countries with mandated BCG policies. The objective of the study is to synthesize and critically evaluate the effectiveness of national BCG vaccination policies in reducing infection and severity of COVID-19 in their native population. Methods: Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, a comprehensive search using pre-identified keywords will be done in PubMed, Cochrane, HERDIN Plus, WPRIM, Web of Science and EBSCO databases. After the initial selection of studies based on eligibility criteria, methodological appraisal will be conducted using the Joanna Briggs Institute appraisal instruments and an adapted quality assessment checklist for ecologic studies. Relevant data will be extracted and synthesized including reporting descriptive and inferential statistics to interpret results. Results: The study will generate a systematic review synthesizing evidence regarding the effects of BCG in curtailing the spread of the COVID-19 pandemic. Discussion: The battle against the COVID-19 pandemic is far from over, and as such, further studies must be undertaken to verify the evidence behind initial strategies in battling it. This includes the use of BCG in decreasing COVID-19 incidence and mortality. The results of the review can ultimately guide health authorities and policy makers create evidence-based decisions regarding BCG vaccination policies and clinical trials related to COVID-19 control and prevention. The objective of the study is to synthesize and critically evaluate the effectiveness of national BCG vaccination policies in reducing infection and severity of COVID-19 in their native population. Methods: Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, a comprehensive search using pre-identified keywords will be done in PubMed, Cochrane, HERDIN Plus, WPRIM, Web of Science and EBSCO databases. After the initial selection of studies based on eligibility criteria, methodological appraisal will be conducted using the Joanna Briggs Institute appraisal instruments and an adapted quality assessment checklist for ecologic studies. Relevant data will be extracted and synthesized including reporting descriptive and inferential statistics to interpret results. Results: The study will generate a systematic review synthesizing evidence regarding the effects of BCG in curtailing the spread of the COVID-19 pandemic. Discussion: The battle against the COVID-19 pandemic is far from over, and as such, further studies must be undertaken to verify the evidence behind initial strategies in battling it. This includes the use of BCG in decreasing COVID-19 incidence and mortality. The results of the review can ultimately guide health authorities and policy makers create evidence-based decisions regarding BCG vaccination policies and clinical trials related to COVID-19 control and prevention. IsiPAN (Isinusulong ang Psychiatry At Neurology): Kalusugan ng Diwa, Utak at Kaisipan tungo sa malusog na Pamayanan at Kinabukasan. A Community-Based Mental Health Program

Currently, mental health issues are of greatest importance to society as it is considered to be a growing invisible pandemic. It is strongly suggested to address concerns in the field of neurology and psychiatry, especially at the community level. One of the most popular interventions that the World Health Organization (WHO) recommends is to bring health promotion to the periphery. With this in mind, a public health campaign in one of the University of Santo Tomas Simbahayan partner communities, Kasiglahan Village Rodriguez, Rizal was born. This project aims to bring health awareness to the community, especially at the grassroots levels involving the municipal rural health unit, barangay health care workers and allied specialist stakeholders. They were taught symptom recognition, proper referral to specialists and prevention measures that the community may benefit from, which is much needed at this day and age. Further, this program can serve as a precursor to more public health initiatives regarding mental health in the community to spread awareness and aid in the prevention of mental health illnesses. Teaching Clinical Medicine During the Pandemic: Experiences and Insights 大流行期间的临床医学教学:经验和见解

The Covid pandemic nearly brought our lives into a different dimension beyond our imagination. Measures to ensure the safety of everyone have never been stricter. The education system had to deal with similar restrictions. Everyone shifted to online virtual classes, which has never been easy to both the teachers and students. Not only are we concerned of our responsibilities as educators but responsive to the needs of students and patients as well during these times of uncertainties. This article briefly describes my personal insight on teaching medicine and how patients continue to take part in the learning process of students despite restrictions.

Viewpoint: Physician as a Clinician, Researcher, and Academician 观点:作为临床医生、研究员和院士的医师

The daily work life of physicians who function as clinicians, researchers, and academicians is focused on improving public health and patient outcomes in three usually compartmentalized settings: clinics, classrooms, and centers for research. In these settings, physicians are addressing patients’ medical concerns based on evidence, and at the same time, considering if new information could lead to research and discovery for better patient outcomes. The physician then proceeds to mentor, teach, and train medical students in quality evidence-based clinical practice and the conduct of scientific research. The importance of operationally blending these three disciplines with research as a common thread is not only a big challenge for these physicians but also an opportunity to innovate for better public health.

This capstone project of the Master in Public Health International of the Faculty of Medicine and Surgery initiated the development of community health programs. Phase 1K, Kasiglahan Village, Barangay San Jose, Rodriguez, Rizal, a partner community of the University of Santo Tomas - SIMBAHAYAN Community Development Office was chosen as the primary stakeholder for the implementation of this capstone project. Addressing the identified priority problem of lacking a community-based strategy in the community, the project aimed to initiate community-based prevention of cardiovascular diseases (CVDs) through risk factor reduction in Phase 1K-Kasiglahan Village, Barangay San Jose, Rodriguez, Rizal at the end of the 6-week project timeline. Specifically, the project aimed (1) to increase the mobilization of the community through a consultative workshop, (2) to raise awareness of the community through Information, education, and communication (IEC) materials, and (3) to increase the baseline knowledge of the community through public health education lay forum. The project outputs include (1) mobilization of the community leaders in identifying their own health problems and in formulating their interventions, (2) posting and distribution of IECs in the community, and (3) increased baseline knowledge after the public health education lay forum. With the potential of influencing local policies to improve the health of the community, this project was formally and officially endorsed to the community through its local government leaders, rural health unit officials, and community leaders.
