Hallux deformity influence - quality of life 拇趾畸形影响生活质量

Background: Hallux valgus is a very common deformity of the big toe and can befamilial or acquired. Knowledge of risk factors of hallux valgus is important in order to prevent or minimize progression of this deformity, since a surgical intervention is the only treatment to correct this deformity. The objective of this study was to determine the risk factors of hallux valgus deformity and the influence in quality of life Material and Methods: This was a cross-sectional study was conducted in Gadarif state, eastern Sudan from January to June 2021.Structured questionnaires were used to collect data from participants. An informed consent was obtained before data collection. Results: A total of 59 subjects from Gadarif state with HA were enrolled in this study. The females were 32 (54.2%).The most common affected age group was > 35 years accounted 44 (74.6%).33 (55.9 %) of study subjects had normal body weight(statistically not significant p=0. 957).41(69.5%) subjects had a family history of HV (statistically significant p=0. 000). 20/32 (62.5%) female use high heelShoe.44 (74.6 %) subjects had a weakness hallux plantar flexion action and abduction(statistically significant p=0. 000). Conclusion: This study found the prevalence of HV increased with age and was higher in females. There was significant correlation between the family history and hallux deformity. Also the study observed significant association between the severity of hallux deformity and impairment of functions of hallux plantar muscles. There is a reduction in health in general and particularly in foot health with increasing severity pain of hallux deformity that appears to be associated with increase in age. Central nervous system Tuberculomas in a patient with disseminated multi-drug resistant tuberculosis; A case report 播散性耐多药结核病患者的中枢神经系统结核瘤病例报告

Tuberculosis (TB) is one of the leading causes of death worldwide, particularly in low- and middle-income countries. Although Mycobacterium tuberculosis can involve any organ, most commonly the lung, central nervous system (CNS) tuberculosis is the most devastating form of the disease. Tuberculoma is the most common parenchymal lesion in CNS tuberculosis which could be found in any portion of the intracranial space. The global rates and numbers of drug resistant TB are rising. With increasing globalization, the spread of drug-resistant strains of TB has become a mounting global public health concern. We present a case of 27-year-old male with disseminated multi-drug resistant (MDR) TB who presented with neurological symptoms and multiple CNS Tuberculomas. The patient was started on regimen for Multi-drug resistant tuberculosis (MDR-TB), which allowed the serial resolution of intracranial tuberculomas. Study of histomorphological patterns of ductal carcinoma in situ (DCIS) occurring simultaneously with infiltrating duct carcinoma (IDC) of breast in menopausal females 绝经期女性乳腺导管原位癌(DCIS)与浸润性导管癌(IDC)同时发生的组织形态学特征研究

Breast cancer ranks second according to the incidence of cancers among women in the world. The peak incidence of breast cancer in developed countries is above the age of 50 years whereas in India it is above the age of 40. Ductal carcinoma in situ lies along a spectrum of preinvasive lesions originating within normal breast tissue, with histological progression from atypical hyperplasia to invasive breast cancer. DCIS is generally categorized by architectural description into five groups, comedo, solid, cribriform, papillary and micropapillary. Studies have found a significant association between nuclear grade of DCIS and histological grade of infiltrating carcinoma. In the present study, out of 60 cases, 33 (55%) show a single growth pattern while 27 (45%) showed a mixed growth pattern. In the present study, comedo pattern is most commonly observed in DCIS lesions with a single growth pattern (48%), followed by solid pattern (42%), and cribriform pattern (06%). The micropapillary pattern was the least common pattern (04%). In the present study, a highly significant correlation was found between the nuclear grade of DCIS and Bloom Richardson grade of infiltrating ductal carcinoma. The present study has validated the concept that nuclear grade of DCIS, as well as architectural pattern, are significantly correlated with the histological grade of invasive carcinoma.

Allergic Bronchopulmonary Aspergillosis (ABPA) misdiagnosed as Pulmonary Tuberculosis 过敏性支气管肺曲霉病(ABPA)误诊为肺结核

Allergic Bronchopulmonary Aspergillosis (ABPA) is a syndrome associated with asthma, manifesting as transient pulmonary infiltrates and eosinophilia which can progress to severe proximal bronchiectasis, and pulmonary fibrosis. Clinicoradiologically it is similar to pulmonary tuberculosis hence most cases of ABPA are usually misdiagnosed as pulmonary tuberculosis resulting in delay in diagnosis as well as treatment which predisposes them to complications and potentially toxic anti–tubercular therapy (ATT). Here we report a case of ABPA masquerading as tuberculosis in a 67 year old female .
