Understanding weight management in adult secure mental health services: findings from a mixed-methods study in Northern England 了解成人安全心理健康服务中的体重管理:英格兰北部一项混合方法研究的结果

Purpose

Obesity and associated morbidity and mortality are major challenges for people with severe mental illness, particularly in secure (forensic) mental health care (patients who have committed a crime or have threatening behaviour). This study aims to explore experiences of weight management in secure mental health settings.

Design/methodology/approach

This study used a mixed-methods approach, involving thematic analysis. A survey was delivered to secure mental health-care staff in a National Health Service (NHS) mental health trust in Northern England. Focus groups were conducted with current and former patients, carers and staff in the same trust and semi-structured interviews were undertaken with staff in a second NHS mental health trust.

Findings

The survey received 79 responses and nine focus groups and 11 interviews were undertaken. Two overarching topics were identified: the contrasting perspectives expressed by different stakeholder groups, and the importance of a whole system approach. In addition, seven themes were highlighted, namely: medication, sedentary behaviour, patient motivation, catered food and alternatives, role of staff, and service delivery.

Practical implications

Secure care delivers a potentially “obesogenic environment", conducive to excessive weight gain. In future, complex interventions engaging wide-ranging stakeholders are likely to be needed, with linked longitudinal studies to evaluate feasibility and impact.

Originality/value

To the best of the authors’ knowledge, this is the first study to involve current patients, former patients, carers and multidisciplinary staff across two large NHS trusts, in a mixed-methods approach investigating weight management in secure mental health services. People with lived experience of secure services are under-represented in research and their contribution is therefore of particular importance.

Self-harm and aggression in women who committed a criminal offence: a scoping review 刑事犯罪妇女的自我伤害和攻击行为:范围界定审查

Purpose

The purpose of this paper is to examine the current state of the literature on the variables associated with self-harm and aggression in women who committed a criminal offence.

Design/methodology/approach

Studies were identified through online databases, namely, PsycINFO, PubMed, ERIC and EBSCOhost, as well as manual searches of reference lists of the selected studies. The target population included women who committed a criminal offence and have engaged in self-harm and aggressive behaviors during their incarceration, either in correctional institutions or in forensic psychiatric settings.

Findings

Of the 1,178 studies identified, nine met inclusion criteria. The studies were conducted in six different countries and included data from 6360 female participants. Few studies examine self-harm and aggression in women who committed a criminal offence which speaks to the still sparse literature on this topic. This review of the association between self-harm and aggression in women offenders highlights the finding that a small group of women is often involved in both self-harm and aggression. The authors have identified possible psychological factors associated with women engaging in both self-harm and aggression. The findings also reveal a possible connection between types of aggressive behaviors and specific time periods during sentences or stays in forensic psychiatry.

Practical implications

The findings of this scoping review have clinical implications which may be considered by both researchers and the case management teams of women involved in both self-harm and aggression.

Originality/value

Despite the limited number of studies examining self-harm and aggression in women, this scoping review highlights gaps in the literature as well as notable psychological correlates of women who engage in self-harm and aggression.

Purpose

The purpose of this study is to explore the experiences of those volunteering within a restorative justice service thus enabling an insight into their perceptions of the different methods used, their beliefs about restorative justice effectiveness, and its place within the criminal justice system. The study also sought to identify any challenges and positive experiences the participants encountered during their role as volunteers, with volunteering during the COVID-19 pandemic explored specifically.

Design/methodology/approach

Data was collected from the participants (n = 5) via semi-structured interviews and analysed using thematic analysis, thus enabling patterns within the experience of the volunteers to be identified.

Findings

A prior understanding and interest in restorative justice was evident within the data, with participants demonstrating a preference for direct, face-to-face mediation. The perceived lack of support from external agencies was discussed along with the role of education in their volunteering experience. Finally, it was acknowledged that although face-to-face practice was deemed the most effective overall, certain practices adopted during COVID-19 enabled aspects of the role to be carried out more efficiently and equally as effectively.

Practical implications

The findings from this study draw out real-world implications, producing tangible action points for restorative justice services. Some tentative suggestions for future practice are outlined.

Originality/value

The volunteers’ role within restorative justice is often overlooked within the literature (Paul and Borton, 2013) and time constraints can add additional barriers to a hard-to-reach population. However, volunteers play a vital role in restorative justice. By exploring and listening to the volunteers’ experience, this study expands an additional strand within the literature in terms of what makes restorative justice effective and the challenges that are faced from a volunteer perspective.

An exploration of mock juror experience during the deliberations of a defendant diagnosed with a personality disorder 探讨模拟陪审员在审议被诊断患有人格障碍的被告时的体验

Purpose

The purpose of this study is to investigate how mock jurors’ experiences of deliberations are impacted by the defendant having a personality disorder.

Design/methodology/approach

This study used a qualitative approach to explore mock jurors’ experiences during the deliberations of a fictional defendant, Sarah Priest. Ten participants formed two mock juries, and each mock jury were given two case studies to deliberate. Case study one described Priest as having “Severe Personality Disorder, Borderline Pattern” whereas case study two described Priest as having “Complex Mental Health Problems”. There were no changes to the content of the case studies aside from the change in language used to describe the defendant.

Findings

An inductive thematic analysis identified two main themes relating to juror experience: “Interaction with Other Mock Jurors” and “Language as a Barrier to a Verdict”. Participants constructed that prosocial interactions with other mock jurors in the deliberations helped them make a verdict decision, but some of these interactions led to disagreements between participants due to a wide variation of opinion. Second, the different description of the defendant in each case study were constructed to have made the deliberations and decision-making difficult, but for different reasons. In case study one, a lack of knowledge surrounding BPD was the reason for this difficulty, and in case study two, participants thought that the applicability of diminished responsibility criteria were unclear, making it hard to reach a verdict.

Practical implications

The findings have key implications for the judicial system; common experiences can be identified and recorded to implement procedures to protect jurors from adverse experiences.

Originality/value

There is a lack of studies that have investigated juror experience in the UK, and the few studies available have used a quantitative methodology. The approach taken in the current study is, therefore, unique in a UK context. Children and adolescents as perpetrators of stalking: an overview 儿童和青少年作为跟踪骚扰的实施者:概述

Purpose

Stalking is considered a public health priority with a range of adverse outcomes. This paper aims to explore existing literature on children and adolescents as perpetrators of stalking, with a focus on rates of stalking and victim and perpetrator characteristics associated with stalking.

Design/methodology/approach

A review of research where stalking was engaged in by those 18 years old and younger was conducted, where 17 manuscripts met criteria for review.

Findings

This review found a prevalence of young people engaging in stalking of between 5.3% and 36%. Considerations including demographics, typologies, prior relationship characteristics, stalking and pursuit tactics, cyberstalking and a brief consideration of the impact are given.

Practical implications

Literature considering perpetration remains thin, and future research should seek to move towards a widely acceptable definition of stalking, as well as considering effective interventions for early intervention, and to consider the role of mental health services in supporting perpetrators and victims, who may not always be mutually exclusive groups.

Originality/value

This paper extends previous literature reviews; the authors understand this to be original work that contributes to a gap in the literature.

Animal abuse recidivism: a narrative review 动物虐待累犯:叙述性回顾

Purpose

The purpose of this paper is to consolidate and evaluate the available research on animal abuse recidivism.

Design/methodology/approach

A narrative review of the animal abuse literature was conducted. Articles were included if they provided data/estimates of the rates of recidivism, findings regarding the static, dynamic and/or protective factors associated with animal abuse recidivism and available risk assessment tools, specifically for use with individuals who have a history of animal abuse.

Findings

The literature review highlighted high rates of reoffending amongst those who have harmed animals. Many risk and protective factors associated with animal abuse were common to the wider offending behaviour literature (e.g. antisocial attitudes, relationship issues), but more robust research is needed to highlight any distinct characteristics. Lastly, the review reports two risk assessment tools designed specifically for this offending group.

Practical implications

Clinicians and criminal justice personnel base their sentencing, detention and treatment decisions, at least in part, on the recidivism literature. This review provides a consolidation of the evidence base as an aide memoire for practitioners.

Originality/value

History of animal abuse is a risk factor for future animal harm specifically, and interpersonal violence more broadly. To the best of the authors’ knowledge, this is the first literature review that presents the key research evidence on risk/protective factors and relevant risk assessment tools that can inform intervention planning to reduce risk of reoffending towards animals and humans alike when practitioners encounter clients who have a history of harming animals.

Group-based mindfulness interventions in prisons: a selective critical review 监狱群体正念干预:选择性评论

Purpose The use of mindfulness interventions (MIs) as means to manage unwanted or undesirable affect and behaviour has increased in popularity recently. This has resulted in the trial of MIs as a means to benefit well-being within prisons, with some intention of reducing recidivism. This study aims to examine current research regarding the use of MIs within prisons. Design/methodology/approach A total of 14 studies were manually selected for review inclusion according to their determined eligibility. All studies were required to have taken place in a prison and to include an explicit mindfulness intervention, including mindfulness theory and practice. Findings Most interventions reviewed demonstrated success, particularly when tailored to the unique needs of a prison environment. Positive changes reported included improvements in participant mental health symptoms, as well as reduced stress and increased mindfulness skills. Some preliminary evidence was provided to indicate the potential longevity of some of these benefits. This review further highlighted some significant questions regarding the generalisability of the proposed suitability of MIs within prison environments including practical delivery constraints and the lack of acknowledgment of the unique challenges and circumstances faced by prisoners. Practical implications The findings of this review show positive potential for the use of MIs within prison populations, where these are well-linked to mindfulness theory and to the needs of inmates. The review also suggest that MIs should be investigated thoroughly, examining potential negative implications alongside positive changes. There needs to be further review of the role of MIs within prison populations to ensure their suitability in the custodial environment. Originality/value MIs are somewhat inflated in terms of predicted outcomes and sometimes applied to potentially unsuitable groups. A significant gap within the literature also persists regarding potentially negative implications associated with MIs, particularly within unique prison populations. Prison treatment programmes should be developed with careful consideration of unique prisoner needs in comparison to a general population.

Purpose The purpose of this study is to build on previous literature in this area thus, the views of professionals working with those who report institutional abuse was sought using a Delphi method. Design/methodology/approach Professionals working with those who report institutional abuse, such as psychologists, social workers and personal injury lawyers, were invited to engage in the Delphi study. Sixteen professionals completed the final round (with four rounds in total). This method was used to gain professional consensus on the considered impacts of institutional child abuse and what factors influence impacts. Findings Eight superordinate themes were developed, as follows: institutional abuse has lasting negative effects on well-being, functioning and behaviour; loss of trust in others and the system is a potential outcome of institutional abuse; negative impacts on future life chances; negative impacts of institutional abuse are exacerbated by numerous factors; protective factors reduced negative impacts; psychological intervention is useful for survivors; positive and negative impacts of disclosure – the response of others as important; and keep impacts individualised. Practical implications The need for an individualised approach when working with those reporting institutional abuse was a salient finding. Originality/value Institutional abuse is known to result in several negative impacts, although research into this area is limited with a need to better understand what may protect or exacerbate impacts.

Purpose Whilst a growing body of support exists for the use of prison animal programmes (PAPs), research within UK prisons is sparse. Opportunity to add to this body of literature is also limited due to the practical barriers around safely embedding animals within forensic settings. This study aims to capitalise on an opportunity to explore a visitation model of a prison-based dog programme (PBDP), recently implemented within a UK prison. The intervention requires staff members to volunteer to bring their pets into the prison on a weekly basis and to accompany them as they complete their normal day-to-day prison roles. Design/methodology/approach Data was collected within a Category B adult prison for males convicted of a sexual offence. Semi-structured interviews were carried out with six prisoners who had spent time with the visitation dogs and six staff members who had been part of those interactions. Reflexive thematic analysis was used to analyse the data, and three themes were developed that focused specifically on the value of this model of PBDP (staff members bringing their pets into the prison). Findings Three themes (reconnecting men to their outside lives; legitimising pro-social narrative; and “a safe space for them to open up”) were developed from participants’ lived experience of the intervention and its perceived value. Practical implications The potential for a low-level PAP to contribute to a rehabilitative prison culture was supported. Originality/value To the best of the author’s knowledge, this is the first study to explore the value of this particular model of PBDP within a UK prison.

Purpose Remand prisoners (RPs) are known to be in a more vulnerable situation than those already convicted. Beyond the difficulties to adapt to the prison, RP also tend to experience tough circumstances due to the uncertainty of their future. This study aims to further test the psychometric properties of the Suicide Concerns for Offenders in the Prison Environment (SCOPE-2) in a sample of RP. Design/methodology/approach The authors have carried out a confirmatory factor analysis to test the psychometric qualities of the SCOPE-2. The authors tested the originally proposed two-factor structure composed of two subscales: optimism and protective self-worth. Also, the authors examined internal consistency through Cronbach’s alphas. Convergent validity was tested by correlational analyses between SCOPE-2 subscales and the Suicide Behaviors Questionnaire-Revised and Brief Symptom Inventory (BSI) total scores. Finally, the authors have tested known-groups validity by carrying out discriminant analysis by testing the SCOPE-2 subscales’ ability to predict belonging to a group with previous suicide attempts. Findings The confirmatory factor analysis showed an acceptable model fit, even though the subscale “Protective self-worth” presented a less acceptable fit. The correlation analysis supported the convergent validity of the SCOPE-2. Both the “Optimism” and “Protective self-worth” subscales showed a positive correlation with the total scores of BSI. Finally, the Portuguese version of SCOPE-2 also showed known groups validity. Concretely, the “Optimism” scores revealed an acceptable predictive accuracy. Practical implications This study embraces important contributions to the practice as it was the first study to validate a measure to assess vulnerability for suicide and self-harm in male and female RP. Originality/value To the best of the authors’ knowledge, this was the first study to validate an instrument to assess vulnerability for suicide and self-harm in both male and female pretrial detainees. Knowing their particular case, as well as the lack of responses for these individuals, it is particularly important to access suicide concerns that could precede suicide attempts.

Design/methodology/approach The authors have carried out a confirmatory factor analysis to test the psychometric qualities of the SCOPE-2. The authors tested the originally proposed two-factor structure composed of two subscales: optimism and protective self-worth. Also, the authors examined internal consistency through Cronbach’s alphas.
