Objective: To investigate the prevalence of first dorsal interosseous myofascial trigger point (MTrP) in patients with thumb carpometacarpal osteoarthritis (CMC OA). Methods: Fifty subjects participated in the study, 25 with thumb CMC OA and 25 without thumb CMC OA. Participants were examined to identify first dorsal interosseous MTrP through palpation. Visual analog scale and Pressure Pain Threshold were the primary outcomes. Results: In patients with thumb CMC OA, the prevalence of MTrPs was higher when compared with the nondominant hand (88% vs 64%) and higher when compared with control group (48% vs 44%). Conclusions: Prevalence of MTrP showed to be higher in the dominant hand of patients with thumb CMC OA than in the dominant hand of people without thumb CMC OA.

Background: The David Spine Concept (DSC) device–based physical therapy for low back pain involves the quantitative assessment of lumbar muscle strength and range of motion. The study aims to determine the effectiveness of the DSC device–based therapy for low back pain. Methods: In this retrospective observational study, clinical data of 103 patients who underwent DSC device–based physiotherapy were reviewed. Pre- and posttreatment outcome measures for pain and disability were assessed. Results: Our results showed statistically significant improvement in terms of disability, pain, and functional status after physiotherapy. Conclusion: The DSC device–based therapy effectively relieved pain, improved function, and reduced disability in patients with low back pain.

Background: To perform a systematic review of the literature to investigate the influence of psychosocial factors on pain and functional outcomes after knee arthroplasty from 6 months after surgery. Methods: Studies were included if they were prospective cohort observational studies. The subjects had to be middle aged or aged (mean age: 45 years) and have undergone total or unilateral knee arthroplasty. Studies should have recorded the influence of different psychosocial factors and the surgery outcomes had to be evaluated according to pain and/or function variables regardless of the tools used to measure them. In addition, outcome measures had to be recorded in the medium term (6 months) or the long term (12 months). Two reviewers assessed independently the MEDLINE, PsycINFO, and CINAHL databases to select observational studies. Results: Twenty-two studies with a total of 7156 patients (5349 females) were included in this review and the mean age was 67.92 years. Twenty-two studies included in this review showed a good average methodological quality (mean ± SD: 7.22 ± 0.92) on the Newcastle—Ottawa Scale for cohort studies. Conclusion: The evidence suggests that catastrophizing, kinesiophobia, anxiety, self-efficacy, and mental health are predictors of postoperative functional outcomes at 6 and 12 months after surgery. There is conflicting evidence on whether or not catastrophizing, anxiety, and depression are predictors of postoperative pain at 6 and 12 months after surgery.

Gait Alterations in Knee Osteoarthritis 膝骨关节炎步态的改变

Objective: People with knee osteoarthritis suffer from many different symptoms and signs, including gait deviations. Studying in deep these alterations through precise instruments like optoelectronic systems allows clinicians to identify alterations on the different body planes during gait. Methods: This study highlights the different alterations found by literature, analyzing them according to the single gait phase. Results: The spatiotemporal parameters are among the most altered parameters; with stride length and speed reduction, on the kinetic and kinematic level, the knee range of motion is reduced while the moments affecting the knee, hip, and ankle joint are altered in the different phases of stance. Conclusions: Preliminary studies have been developed on the possible quantification of these alterations through scoring systems like the Gait Deviation Index. Further studies should use this scoring system also to solve the issue between the possible correlation of pain and gait deviation that literature is still trying to solve.

Background: Force sense is described as the ability to reproduce a desired level of strength. An adequate sensation of the muscular thumb effort allows us to be able to carry out activities of daily life. Objective: The primary objective of the present study was to evaluate the impact of a proprioceptive exercise program on the force sense for carpometacarpal joint. Methods: A total of 17 elderly women with a diagnosis of carpometacarpal joint osteoarthritis in their dominant hand participated in the study. Mean pinch force errors were measured at basal, 4, and 12 weeks of treatment. Results: Statistically significant differences in force sense testing scores after the proprioceptive exercise program were observed. Conclusion: The proprioceptive exercise program could improve precision on force sense of the thumb in patients with carpometacarpal joint osteoarthritis.

Physical Activity in Older Adults With Visual Impairment 老年视力障碍患者的体育活动

Visual impairment presents challenges for older adults to participate in physical activity or exercise. The purpose of this collective case study is to explore how older adults with visual impairment participate in physical activity. Four older adults with visual impairment were recruited to participate in the study. Three out of the 4 participants were able to complete all rounds of the study prior to COVID-19 pandemic halting the study. Five themes emerged from the study: exercise to enhance health and well-being, occupations as activity, loss of participation, adaptation to vision loss, and motivation to stay healthy. The findings support the use physical activity to enhance overall health and well-being.

Maintenance Physical Therapy 维持性物理治疗

Background: Medicare recipients historically have been discharged from rehabilitation when progress plateaus. This improvement standard continues to shape episodic practice of physical therapy (PT). The Jimmo v Sebelius decision of 2013 has the potential to open physical therapist practice with the adoption of community-based maintenance PT into everyday practice. Conclusion: Prevention and maintenance of function are supported by the American Physical Therapy Association. Evidence suggests that intervention over time combats frailty, reduces fall risk, improves functional outcomes, and prevents movement disability. Community-based maintenance PT as a method of moving from the episodic restorative model to a continuous paradigm of restoration, prevention, and maintenance is proposed.

Depression Screening by the Interprofessional Team for Community-Dwelling Older Adults 社区居住老年人跨专业团队的抑郁症筛查

Objective: A miniature semisystematic review to determine health professionals' current practices when screening for depression in older adult community dwellers. Evidence Review: A review of research studies published between April 2010 and April 2020 using a university library system and key words specific to factors essential to this review. Findings: Health professionals inconsistently perform depression screening to rule out or determine the risk of diagnosis or after a new diagnosis. Conclusions and Relevance: Health professionals are not routinely screening for depression despite possible impacts on outcomes. More evidence is needed to promote depression screening in health care for improved patient outcomes.
