A Toolbox for Implementing Community-Based Physical Activity Programs for Older Adults and Adults With Disabilities 实施以社区为基础的老年人和残疾人体育活动计划的工具箱

The article describes the role of rehabilitation professionals in promoting physical activity for older adults and older adults with disabilities, including tools for implementing evidence-based programs. Using the plethora of resources articulated in this article, therapists will be able to enhance their wellness, fitness, and physical activity programming in their practice. Two innovative programs in which a university collaborated with a local YMCA to provide physical activity programming and 2 clinic-based programs developed by rehabilitation professionals are described. Conducting needs assessments, measuring outcomes, and being aware of facilitators and barriers to program participation are critical to program success.

Movement, Music, and Service Learning 运动、音乐和服务学习

Importance: Lack of geriatric health care professionals is a growing concern. Negative attitudes toward older adults and aging bias in students can impact quality of care, and lack of evidence-informed practice has been linked to early training. Students must be nurtured to provide quality-appropriate care for older adults. Objective: To explore the development of an evidence-based program for older adults, specifically for balance training, to be used with students in a service-learning capacity. Design Setting, and Participants: A qualitative case study was conducted with physical and occupational therapy students, led by faculty, engaging in service-learning experiences. Students were from 3 consecutive cohorts attending a health sciences gerontology class. One-third of the students were randomly selected to participate in the developed movement and music workshop at senior centers, assisted living facilities, and memory care units. The remaining students participated in general activities in the same or similar locations. Surveys were completed by the students as well as a reflection paper. Exposures: Students participated in a 1-hour movement and music workshop alongside volunteer older adults. The workshop was developed using motivational theories and evidence-based recommendations for balance interventions for older adults. Main Outcome: Student surveys demonstrated a progressive ability to recommend advanced balance interventions for older adults. Student surveys and reflective papers showed that the service-learning workshop had specifically enhanced their comprehension of motivation and meaningful activities to push older adults. Results: The ability for students to recommend intermediate-level balance activities increased by 15% with a class only, but then advanced activity recommendations improved by 68% in those attending the workshop. Psychosocial aspects only emerged after the workshop. Service learning influenced all students, but concepts of motivation and meaningful activities were more understood by those participating in the movement workshop. Conclusion and Relevance: Service learning is an excellent opportunity for faculty to demonstrate to students how to apply evidence-informed interventions in a manner appropriate and motivational for older adults.

Incorporating evidence-based community programs into clinical care recommendations and goals may help bridge the clinic-to-community transition for older adults. Engagement in evidence-based programs can help older adults manage chronic conditions and reduce fall risk through behavior change and self-management following a clinical episode of care. This paper describes evidence-based fall prevention and physical activity programs, provides resources to locate programs, and strategies to match older adults to the right programs.

Context: The purpose of this article is to describe the typical aging process and compare this to mild cognitive impairment and early symptoms of dementia, specifically cognitive-communicative and swallowing deficits, to help professionals better identify dementia in the care of community-based adults. Objective: To evaluate the role and importance of the speech-language pathologist in early identification and management of dementia. Suggested interprofessional considerations for best practice for early identification, management, and referral are also addressed. Conclusion: Speech-language pathologists are critical for assessing and treating those with early stages of dementia and mild cognitive impairment. It is critical for medical professionals to recognize the speech pathologist's role in treating those with cognitive impairments and swallowing disorders.

Causes of social isolation and loneliness in older adults are multifactorial and can have significant effects on their health and quality of life. Older adults who have good social support exhibit lower levels of depression, higher levels of life satisfaction, lower stress levels, and lower health care costs. By assessing and addressing social isolation and loneliness in community-dwelling older adults, rehabilitation therapists can have a positive impact on their physical and mental health.

Rehabilitation therapists have traditionally been key players in the development and delivery of community-based programs, and the value of those programs is well documented. The onset of COVID-19 necessitated the closure or shift of many of these programs to virtual delivery, resulting in a change in the role of the rehabilitation therapist. The challenges and opportunities of traditional and virtual programming are explored. Factors influencing the participation of seniors in these programs are examined. The impact of COVID-19 and the resulting digital resources on future community-based practice is discussed.

Purpose: To determine effects of a fall risk educational program on fall risk awareness. Methods: Twenty-five community-dwelling older adults 65 years and older attended a single-session intervention. Investigators held focus groups 1 month post-intervention. Investigators administered the Short Falls Efficacy Scale-International (S-FES-I) pre- and post-intervention. Results: Raw S-FES-I scores trended toward reduced fear of falling. Older adults reported heightened awareness of fall risks and reinforcement of fall prevention behaviors. Social support and narrative storytelling helped motivate participants. Conclusion: A single-session educational program reinforced existing fall risk reduction knowledge and behaviors but was limited in its ability to prompt behavioral change.

There is evidence to suggest that assisted cycling can effectively improve the motor function of patients with Parkinson's disease (PD) and an assisted cycling intervention is more beneficial for those with more advanced PD. Therefore, the aim of this study was to investigate the influence of an intervention with a coordination program at MOTOmed Viva 2 Parkinson on dynamic balance, muscle parameters, and symptoms in elderly people diagnosed with advanced stages of PD. We studied 4 elderly people with PD in advanced stages. We evaluated PD symptomatology using the adapted Spanish version of the MDS-UPDRS (Movement Disorder Society Unified Parkinson's Disease Rating Scale); dynamic balance was evaluated by means of the Timed Up and Go (TUG) and the MOTOmed Viva 2 Parkinson system stores data about muscle tone, symmetry, coordination, and spasms, in each of the sessions undertaken. We found that a 21-minute coordination training in 13 sessions with the MOTOmed device demonstrates positive effects on muscle tone, spasms, coordination, symmetry, and balance in older adults diagnosed with advanced stages of PD.

Complications During the Inpatient Rehabilitation Period in Neurological Subjects 神经内科病人住院康复期并发症

Objective To explore common complications in people with neurological conditions during rehabilitation admission. Methods: All neurological subjects, including those with stroke, spinal cord injury (SCI), and traumatic brain injury, who were admitted for intensive and less-intensive rehabilitation programs during January to December 2012 were recruited. Both avoidable and unavoidable complications and related factors were assessed. Results: Among 1146 subjects, 65% was male with a mean age of 54.5 years. Almost 90% were admitted for intensive rehabilitation. The common avoidable complications that occurred were urinary tract infection (UTI) (12.8%), musculoskeletal pain (7.0%), neuropathic pain (2.3%), pneumonia (1.5%), and pressure injury (1.2%). Spasticity, which is an unavoidable complication, occurred in 2.1% of subjects. Older age, male gender, diagnosis of SCI, and longer stay were important factors related to complications. Conclusions: Most of the common complications during rehabilitation were avoidable complications, especially UTI and pain. A heightened level of awareness and vigilance should be exercised in older male subjects, those with prolonged hospital stay, and those with a diagnosis of SCI.
