Socioenvironmental and mental health determinants of alcohol use among Black sexually minoritized men and transgender women: Findings from the N2 cohort study. 性少数黑人男性和变性女性酒精使用的社会环境和心理健康决定因素:来自N2队列研究的发现

Introduction: We aimed to identify socioenvironmental determinants of alcohol use among Black sexually minoritized men (SMM) and transgender women (TW).

Method: Data were from the Neighborhoods and Networks (N2) cohort study among Black SMM and TW (01/2018-08/2020). We used stepwise negative binomial regression to identify associations with alcohol use.

Result: Among our sample of 142 Black SMM and TW (agemean=24±4 years), 68% reported past month alcohol use. Adverse childhood experiences (IRR=1.10;95%CI=1.03-1.18), GAD-7 (IRR=1.03;95%CI=1.00-1.07), and community violence (IRR=1.06;95%CI=1.02-1.10), were positively associated with alcohol use.

Conclusion: Multilevel alcohol reduction interventions can consider reducing community violence and increasing access to mental health services.

Introduction: High body mass index (BMI) can be a barrier to accessing gender-affirming surgery (GAS).

Method: This case report discusses the mental health implications of being denied GAS based on BMI in a transgender woman presenting to psychiatric care integrated with primary care. Subsequently, we summarize existing literature regarding GAS and mental health.

Result: Existing literature on GAS outcomes robustly demonstrates the mental health benefits of undergoing desired GAS and does not support using BMI to determine surgical candidacy.

Conclusion: This case highlights why psychiatrists and other mental health clinicians should be concerned about BMI serving as a barrier to our patients accessing GAS.

Background: Young Black and Latinx Gay and Bisexual Men and Transgender Women (YBLGBMTW) face high levels of mental health related concerns. While online interventions show promise for addressing mental health conditions, few models focus on this population.

Methods: We used a pre-post design to assess the feasibility and acceptability of a peer delivered mental health awareness intervention for YBLGBMTW. This study was nested as the control arm from a larger study investigating if Peer Influencers (PIs) can improve HIV prevention. PIs helped develop an online outreach campaign to raise awareness of pertinent mental health topics as chosen by the PIs. PIs recruited YBLGBMTW participants from online social networks, posted contents over a six-week period, and participants completed baseline and post-intervention surveys.

Results: PIs successfully recruited 71 participants of whom 92% completed the six-week post-intervention survey. Participants had on average 6.7 engagements with posted contents and 82% of participants indicated being very satisfied with the intervention.

Conclusion: Using an intervention informed and delivered by PIs, participants were presented with information to address relevant mental health concerns. We found that such novel means of engaging this sample for mental health is feasible, acceptable, and merits further development.

Purpose: Sexual self-schema (SSS) are cognitive frameworks for one's thoughts and feelings about oneself as a sexual being. These schema interpret and organize sexual information and attitudes, and can guide sexual behavior. This study examined how plurisexual attractions may interact with gender to predict SSS.

Materials and methods: I surveyed 602 young adults about their gender, patterns of attractions to men, women, and gender non-binary people, and SSS using the validated SSS Scale.

Results: In this sample, SSS were similar across participants with monosexual vs. plurisexual patterns of attraction. However, a few significant effects emerged. Cisgender women with low attraction to any gender reported significantly lower romantic/passionate self-schema relative to women with at least some attraction to one or more genders. In contrast, cisgender men with exclusively same-gender attractions reported higher open/liberal self-schema relative to men with either exclusively other-gender or plurisexual attractions. Novel to this study, 22% percent of participants reported attractions to gender non-binary and gender non-conforming people, and these attractions significantly predicted SSS.

Conclusion: These findings point to subtle gender differences in how attractions interact with young people's conceptualization of their sexual selves.
