Patients' perspective on the inclusion of intraoral scanners in routine oral examination: a clinical perception study. 患者对将口腔内扫描仪纳入常规检查的看法:临床认知研究。

Aim: The purpose of the present study was to investigate how integrating intraoral scanners (IOSs) into routine oral diagnosis affects patient-clinician communication as well as patient perceptions and preferences, offering valuable insights into the patient experience and utility of IOSs in clinical practice.

Materials and methods: A restorative dentistry specialist conducted intraoral examinations on 300 patients, initially using conventional techniques and radiographs, followed by a 3D intraoral scanner (iTero Element 2). The patients' existing oral issues and treatment plans were initially expressed verbally, and then repeated using the 3D intraoral scan. Subsequently, the patients were requested to respond to an 11-question survey, aimed at assessing both modes of examination and communication. Statistical analysis employed the Wilcoxon test to compare the patients' experiences on the same topic before and after the examination with the IOSs, considering a significance level of P 0.05.

Results: The majority (38%) of participants were 25 to 35 years of age (52% females; 48% males). Statistically, patients found it more comprehensible to visualize existing oral issues through 3D intraoral scans than through a verbal explanation (P = 0.000). Also, patients understood oral hygiene recommendations better when explained by dentists through the use of intraoral scans (P = 0.000). The vast majority of participants (94%) indicated that being examined with an IOS motivated them to obtain treatment, while 6% remained undecided. All of the participants thought that intraoral scanning should be included in routine oral examination.

Conclusion: Intraoral 3D scans enhance patient understanding and communication during routine oral examination and participants showed a high preference for their usage. However, more research is needed to replace conventional diagnostic methods.

Accuracy of intraoral scanners in the cleft anatomy of neonates. 口腔内扫描仪在新生儿唇裂解剖中的准确性。

Aim: The present study investigated the accuracy of intraoral scanners (IOSs) based on different image acquisition technologies in the field of presurgical orthopedic treatment (PSOT) in neonates with cleft palate.

Materials and methods: Dental casts of clinical situations representing unilateral cleft lip and palate (UCLP), bilateral cleft lip and palate (BCLP), and cleft palate (CP) with reference polyetheretherketone (PEEK) scanbodies (Cares RC Mono Scanbody) were scanned utilizing four IOS systems: Carestream CS3600 (CS), Medit i500 (MD), Cerec Omnicam (CO), and Trios 3 (TS). One calibrated operator made five scans from each model using each IOS (N = 60). Reference digital impressions were obtained by an industrial-grade laboratory scanner (Sirona inEos X5) and superimposed using a best-fit algorithm. The divergence measure was extracted and the scanners were compared in terms of their accuracy using generalized least squares statistical models that account for variance heterogeneity. Additionally, comparative 3D analysis of the scans was performed using reverse engineering software (Geomagic Control X) in order to measure the discrepancy between the intraoral scans and the reference scan in five different anatomical regions of interest (ROIs): alveolar crest (AC), cleft (CL), palate (PL), vestibulum (VS), premaxilla (PM).

Results: The four IOSs showed relevant and significant differences in estimated trueness (P 0.001) and precision (P = 0.009). Among all anatomical models and analyzed ROIs, TS had the best accuracy (trueness: -1.57 µm; precision: 9.41 µm), followed by MD (trueness: -20.63 µm; precision: 29.18 µm), CS (trueness: -40.43 µm; precision: 16.52 µm) and CO (trueness: 81.27 µm; precision: 40.32 µm).

Conclusions: Impression taking of the maxilla in cleft lip and palate patients is challenging for the operator. Relevant and significant differences in trueness and precision were found between the four IOSs. TS showed the best accuracy and was the least influenced under different anatomical ROIs. Effect of antagonist arch on precision and trueness of digital maxillomandibular relation in partially edentulous cases using five different intraoral scanners. 拮抗弓对五种不同口内扫描仪对部分无牙病例上颌下颌关系准确性的影响。

Aim: The study evaluated the trueness and precision of maxillomandibular relation records in intraoral scans of partially edentulous cases using dentate and edentulous arch antagonists.

Materials and methods: Three 3D-printed models were prepared: one maxillary and mandibular partially edentulous model and a mandibular dentate model. The partially edentulous models included two dental implants each (Bone Level RC) in the edentulous molar region. Edentulous antagonist (AE: models 1 and 2) and dentate antagonist (AD: models 1 and 3) groups were analyzed separately. Precision spheres were placed on each model to evaluate interocclusal distances. Reference scans were acquired by the industrial scanner (Nikon Altera). Separate model scans and interocclusal records were acquired using five intraoral scanners: Primescan (PS), Trios 3 (T3), Trios 4 (T4), Carestream 3600 (CS), and Medit i500 (Me). Statistical analysis included Shapiro-Wilk normality tests, Mann-Whitney Wilcoxon, or Student's t-tests (p < 0.05).

Result: Trueness and precision varied across scanners. For AD, significant differences in trueness were noted for most scanner comparisons, except for Me vs. PS, Me vs. T3, Me vs. T4, and T3 vs. T4 (p > 0.05). AD precision was statistically different among scanners, except for PS (p > 0.05). In AE, only PS vs. T3 did not show a significant difference (p > 0.05).

Conclusion: Dentate and edentulous antagonist arches affect precision in intraoral scans of partially edentulous jaws but do not influence trueness.

Aim: The application of paraclinical diagnosis in temporomandibular joint (TMJ) has always been a controversial topic due to the lack of universally accepted standards. Compared to medical imaging that provides TMJ structural analysis, axiography involve functional recording of condylar movement. The purpose of this study is to explore the diagnostic value of computerized axiography for patients with temporomandibular joint disc displacement.

Materials and methods: 17 patients (34 TMJs) with bilateral TMD were treated with a stabilization splint for 7.26 ± 1.82 months. TMJs were divided into 2 groups as displacement with reduction (DDwR) (n=20) and disc displacement without reduction (DDwoR) (n=14) according to pretreatment MRI. A computer-aided diagnosis axiography (CADIAX) was used to record and analyze the results of the condylography examinations, including range, morphological characteristics and stability of condylar movement.

Results: Computerized axiography can effectively assist in the three-dimensional visualization of TMJ. Condylography confirms that both range and stability of condylar movement have been improved. In addition, it also improved coordination of bilateral joint movements.

Conclusion: Computerized axiography could perform comparative analyzes of the two joints and provide real-time evaluation of condylar movements in three dimensions. The result of this study suggests that computerized axiography can be used for the periodic evaluation of treatment progression of TMD.

Preliminary insights into dental student satisfaction in flipped-classroom seminars enhanced by artificial intelligence-generated podcasts: a comparative study. 人工智能生成的播客提高了牙科学生在翻转课堂研讨会上的满意度:一项比较研究。

Aim: To evaluate the perceived usefulness of a flipped-classroom model that integrated podcasts generated using an artificial intelligence (AI) tool in undergraduate dental education.

Materials and methods: This within-subject, quasi-experimental study was conducted at a Swiss dental school. A cohort of 49 clinical-year dental students participated in eight conventional seminars and two flippedclassroom seminars, each covering a distinct clinical topic. In both formats, a subset of 5-6 students acted as instructors under the supervision of residents or faculty members. The flipped-classroom model required students to engage with AI-generated podcast summaries of research articles before in-class discussions. These podcasts were created using NotebookLM, an online platform powered by a large language model. Surveys using Likert-type scales assessed student and student instructor satisfaction, engagement, and learning preferences.

Results: The surveys obtained response rates of ≥93.9%. The flippedclassroom model achieved high satisfaction levels among students. Compared with conventional seminars, students reported increased engagement and comprehension in the flipped-classroom format. Student instructors valued the ease of preparation and the enhanced interactivity of in-class sessions. AI-generated podcasts were rated as effective pre-class resources.

Conclusion: The integration of a flipped-classroom model with AI-generated podcast resources seemed to improve student self-perception of engagement, comprehension, and satisfaction in dental education. This instructional approach may also foster critical appraisal skills for AI-generated content-an increasingly essential competence in academic and professional practice as AI tools gain prominence.

Color change of laminate veneer restorations applied to bleached teeth. 用于漂白牙齿的层压板贴面修复体的颜色变化。

Aim: The aim of the present study was to examine the color stability of laminate veneer restorations fabricated with various CAD/CAM materials applied to bleached teeth.

Materials and methods: Eighty maxillary central incisors extracted due to periodontal, orthodontic, or trauma problems were used. The teeth were embedded in acrylic blocks and divided into eight groups (n = 10). Groups A, B, C, and D were bleached with a vital bleaching agent before preparation, and the teeth were prepared for laminate veneer restorations. Groups E, F, G, and H were prepared without bleaching. Groups A and E were restored with GC Initial LiSi HT Blocks A1, Groups B and F with GC Initial LiSi LT Blocks A1, Groups C and G with IPS e.max CAD HT Blocks A1, and Groups D and H with IPS e.max CAD LT Blocks A1. All restorations were adhesively cemented and aged for 2 and 5 years with thermal cycling. Color measurements of the restorations were taken with a spectrophotometer at baseline and after 2 and 5 years of aging.

Results: Teeth in all the bleached groups showed more color changes than those in the unbleached groups. After 2 years of aging, the least color change was observed with GC Initial LiSi LT (ΔE00 = 0.808) and IPS e.max CAD LT (ΔE00 = 0.813) materials, which were used on unbleached teeth, and the most color change was observed with GC Initial LiSi HT (ΔE00 = 0.934) and IPS e.max CAD HT (ΔE00 = 0.923) materials, which were used on bleached teeth. After 5 years of aging, the least color change was observed with IPS e.max CAD LT (ΔE00 = 0.831) and GC Initial LiSi LT (ΔE00 = 0.839) materials, which were used on unbleached teeth, and the most color change was observed with GC Initial LiSi HT (ΔE00 = 0.957) and IPS e.max CAD HT (ΔE00 = 0.938) materials, which were used on bleached teeth.

Conclusions: Bleaching and translucency affect color stability. No difference was detected between the color changes of GC Initial LiSi and IPS e.max CAD materials. The increase in aging time increased the color changes of all materials.

Clinical significance: Bleaching and laminate veneer restorations may be preferred in many patients. For this reason, the long-term color changes of laminate veneer restorations applied to bleached teeth is clinically very relevant. The present study aimed to evaluate the effect of tooth bleaching on the long-term color stability of laminate veneer restorations fabricated with various CAD/CAM materials of different translucencies.

Use of optical see-through head-mounted display (OST-HMD) in dentistry: a scoping review. 在牙科中使用光学透视头戴式设备--范围综述。

Aim: The aim of the present scoping review was to identify the scientific evidence related to the utilization of optical see-through head-mounted display (OST-HMD) in dentistry, and to determine future research needs.

Methods: The research question was formulated using the 'Population' (P), 'Concept' (Cpt), and 'Context' (Cxt) framework for scoping reviews. Existing literature was designated as P, OST-HMD as Cpt, and Dentistry as Cxt. An electronic search was conducted in PubMed, Embase, Web of Science, and Cochrane Central Register of Controlled Trials (CENTRAL). Two authors independently screened titles and abstracts and performed the full-text analysis.

Results: The search identified 286 titles after removing duplicates. Nine studies, involving 138 participants and 1760 performed tests, were included in the present scoping review. Seven of the articles were preclinical studies, one was a survey, and one was a clinical trial. The included articles covered various dental fields: three studies in orthodontics, two in oral surgery, two in conservative dentistry, one in general dentistry, and the remaining one in prosthodontics. Five articles focused on educational purposes. Two brands of OST-HMD devices were used: in eight studies Microsoft HoloLens was used, while Google Glass was utilized in one study.

Conclusions: The overall number of included studies was low; therefore, the available data from the present review cannot yet support an evidence-based recommendation for the clinical use of OST-HMD. However, the existing preclinical data indicate a significant capacity for clinical and educational implementation. Further adoption of OST-HMD devices will facilitate more reliable and objective quality and performance assessments as well as more direct comparisons with conventional workflows. More clinical studies must be conducted to substantiate the potential benefits and reliability for patients and clinicians.

Background: Implantology is becoming increasingly digital, opening up new therapy options to achieve preferably predictable and complication-free results. One key parameter for success seems to be stable and fixed soft tissue around the implant as well as the prosthodontic supply. The concept presented in this article aims for an anatoform and stable emergence profile around implants in a digital workflow applying individual polyetheretherketone (PEEK) healing abutments.

Case presentation: A 63-year-old female patient presented to the Department of Prosthodontics at the Center for Dentistry and Oral Health of the Goethe University Frankfurt with implants inserted at regions 45 and 47 and conventional titanium healing abutments. An intraoral scan was performed and a screw-retained 3-unit fixed dental prosthesis (FDP) was designed. On the basis of the CAD, individual healing abutments were designed and fabricated out of PEEK, representing a similar emergence profile to that of the final FDP. The components were fabricated in a milling center and finalized. After soft tissue relocation, the individual PEEK healing abutments were placed. Then, after a 3-week rest period, a screw-retained FDP fabricated out of monolithic zirconium oxide was inserted without further soft tissue compression.

Conclusion: Individual healing abutments fabricated out of PEEK might be an interesting therapy option for shaping the soft tissue around implants and can be supplemented as a straightforward tool especially in fully digital workflows.

One key parameter for success seems to be stable and fixed soft tissue around the implant as well as the prosthodontic supply. The concept presented in this article aims for an anatoform and stable emergence profile around implants in a digital workflow applying individual polyetheretherketone (PEEK) healing abutments.</p><p><strong>Case presentation: </strong>A 63-year-old female patient presented to the Department of Prosthodontics at the Center for Dentistry and Oral Health of the Goethe University Frankfurt with implants inserted at regions 45 and 47 and conventional titanium healing abutments. An intraoral scan was performed and a screw-retained 3-unit fixed dental prosthesis (FDP) was designed. On the basis of the CAD, individual healing abutments were designed and fabricated out of PEEK, representing a similar emergence profile to that of the final FDP. The components were fabricated in a milling center and finalized. After soft tissue relocation, the individual PEEK healing abutments were placed.
