Application of NANDA-I nursing diagnoses, nursing interventions classification, and nursing outcomes classification in research and practice of cardiac rehabilitation nursing: A scoping review NANDA-I 护理诊断、护理干预分类和护理结果分类在心脏康复护理研究和实践中的应用:范围综述。

Purpose

To describe the current application status of NANDA-I nursing diagnoses, Nursing Interventions Classification (NIC), and Nursing Outcomes Classification (NOC) in cardiac rehabilitation nursing and identify useful NANDA-I, NIC, and NOC (NNN) linkages for clinical nursing practitioners.

Methods

This scoping review was performed in accordance with the Joanna Briggs Institut guidelines. We systematically searched eight databases, and the literature search took place between June and July 2023. The characteristics and results of the studies were synthesized and analyzed in a narrative way.

Findings

The application of NANDA-I nursing diagnosis, NIC and NOC in cardiac rehabilitation nursing can be divided into three topics: the content, value and effect of NANDA-I nursing diagnosis, NIC and NOC.

Conclusion

The application of NANDA-I, NIC, and NOC in the field of cardiac rehabilitation nursing shows positive effects on the whole, which can provide more standardized theoretical guidance, improve nursing outcomes in clinical settings, and enhance nursing quality.

Implications for nursing practice

This experience report will guide nurses to use NANDA-I, NIC, and NOC for better cardiac rehabilitation care.

Proposal of nursing diagnosis “adult disuse syndrome”: A conceptual derivation and integrative review 提出护理诊断 "成人失用综合征":概念推导和综合回顾。

Purpose

To develop a nursing diagnosis proposal derived from the concept of “disuse syndrome” based on an integrative review.

Methods

The conceptual derivation technique, proposed by Walker and Avant, was used in the following phases: an integrative review of the literature was carried out, applying the parameters of Whitermore and Knalf, where 43 articles were selected that covered the existence of the concept in the nursing discipline and other health sciences, the concepts were subsequently analyzed, and the new concept was redefined following the middle-range theory of causality and validation of nursing diagnoses proposed by Oliveira-Lopes et al.

Findings

The proposal of the new nursing diagnosis was developed, including the following elements: diagnostic label adult disuse syndrome, the corresponding definition, 25 defining characteristics, eight related factors, eight risk populations, and 14 associated problems.

Conclusions

The concept of the proposed new diagnosis describes adult disuse syndrome as a phenomenon that transcends the physiological component and should be viewed from a multidimensional view, expressing a clinical judgment related to a series of nursing diagnoses that appear together.

Implications for nursing practice

The results of this study will help nursing professionals to identify the phenomenon of adult disuse syndrome in any setting and in turn guide optimal care interventions that decrease the consequences of adult disuse syndrome.

This brief report discusses the implementation and evolution of suicide prevention strategies at Taoyuan Psychiatric Center in Taiwan. These strategies are inspired by the nursing interventions presented in an “Umbrella review of nursing interventions NIC for the treatment and prevention of suicidal behavior.” The interventions are a part of an employee assistance program (EAP) designed to support the mental health of the nursing staff and other employees, with a particular focus on preventing and managing suicidal behaviors. The EAP involves multi-tiered primary, secondary, and tertiary prevention measures, offering different levels of psychological interventions.

Nursing interventions for adult patients undergoing cancer cardiotoxic therapy: Scoping review 对接受癌症心脏毒性治疗的成年患者进行护理干预:范围综述。

Purpose

To identify nursing interventions for the management of adult patients undergoing cardiotoxic oncologic therapy.

Methods

This scoping review was performed in accordance with the JBI guidelines. The literature search took place between July and August 2022. Studies examining interventions for the management of adult cancer patients undergoing cardiotoxic therapy were included. The characteristics and results of the studies were synthesized and analyzed in a narrative way.

Findings

In the nine included studies, it was verified that the interventions were implemented to guide the actions of the health team in general rather than specifically nursing staff. Nine nursing interventions related to the Classification of Nursing Interventions were included.

Conclusions

The nursing interventions identified focused on rigorous cardiovascular surveillance, risk assessment, and actions to identify and mitigate cardiotoxicity.

Implications for Nursing Practice

It is believed that the implementation of the identified nursing interventions will lead to evidence-based nursing practice and will contribute to the development of care products and processes that assess the cardiological risks and cardiotoxicity.

Modifiable risk factors of cognitive frailty in community-dwelling older adults: A systematic review and meta-analysis 社区老年人认知虚弱的可调整风险因素:系统回顾和荟萃分析

Purpose

To perform a meta-analysis identifying the modifiable risk factors of cognitive frailty in the elderly.

Methods

We systematically searched databases including PubMed, EMBASE, China National Knowledge Infrastructure, China Science and Technology Journal Database, Wanfang Data Knowledge Service Platform from January 01, 2017 to March 26, 2022. Quantitative, original research reporting associated factors were included.

Findings

7,854 records in total were identified, of which 14 articles (1 prospective, 13 cross-sectional) with 36 factors were included. Studies on cognitive frailty included 20,390 community-dwellers (≥60 years) from three countries. Meta-analysis indicated that depression [OR = 3.60, 95%CI (2.25,5.78), p < 0.01] and sleep problems [OR = 2.36, 95%CI (1.62,3.43), p < 0.01] were associated with cognitive frailty.

Conclusions

Effective interventions targeting depression and sleep problems may lower the risk of cognitive frailty in the community seniors but need further study in high-quality, prospective studies.

Implications for Nursing Practice

Building on previous work, the objectives of this systematic review and meta-analysis were to explore potential modifiable risk factors for cognitive frailty in community-dwelling older adults, which is expected to throw light on the prevention of cognitive frailty.

Aim

This study aims to examine the nursing diagnosis skills of students who took the case-based online nursing processes course during the COVID-19 pandemic.

Methods

The study followed a descriptive and cross-sectional design. The participants were first-year students (n = 148) enrolled in the nursing principles course in the spring semester of the 2020–2021 academic year in the nursing department of a university. The course on nursing processes was delivered online due to the COVID-19 pandemic. At the end of the course, students who volunteered to participate in the study developed nursing diagnoses for the cases assigned to them. The data were collected from the students using two forms and were evaluated using a form developed by the researchers. The data were analyzed with numeric and percentage calculations.

Findings

Although 56.8% of the students stated that they had difficulty making nursing diagnoses, 56.8% of them thought online education was not useful. The most commonly made diagnoses by the students who participated in the study included hyperthermia (66.2%), ineffective breathing pattern (54.7%), risk of falling (39.9%), fatigue (34.5%), and anxiety (33.8%). It was also found that students were able to identify objective data more easily as diagnosis criteria, whereas they were not able to identify abstract concepts.

Conclusions

It was seen that the students who participated in the study had low levels of nursing diagnoses. Based on the findings, it is recommended that different teaching methods be employed in the online nursing processes course, and these methods be evaluated in terms of their effects on students’ learning outcomes.

Implications for nursing practices

The online nursing process course should be made more efficient. First-year nursing students are not yet ready to identify nursing diagnoses in terms of knowledge and skills.

Purpose

To evaluate the accuracy of defining characteristics and causal relationships of the etiological factors of the nursing diagnosis deficient knowledge in individuals with heart failure .

Data sources

An analytical, cross-sectional study on the diagnostic accuracy of the defining characteristics and causal relationships of the etiological factors of the nursing diagnosis. The sample consisted of 140 patients with chronic HF and in outpatient follow-up. The latent class analysis method was used to test the accuracy of measurements and estimate the prevalence of the diagnosis. The calculation of subsequent probabilities and the odds ratio ( were also parameters employed. The study was approved by the Research Ethics Committee of the Federal University of Pernambuco.

Data synthesis

The diagnosis had an estimated prevalence of 38.57% in the sample. The inaccurate statements about the disease and/or therapy, self-care deficient performance, and inadequate behavior were the clinical indicators that best predicted the presence of the diagnosis and demonstrated the same sensitivity value (1.0000), specificity (1.0000), and 95% confidence interval (0.9999–1.0000) for all. The populations at risk was elderly (OR = 2.12, confidence interval 95% = 1.05–4.27), and illiterate individuals (OR = 2.07, confidence interval 95% = 1.03–4.16) had an approximately twofold great chance of developing havening deficient knowledge.

Conclusion

The evaluation of the accuracy of clinical indicators, corresponding to the defining characteristics in the study, contributed to screening and diagnostic establishment capacity in clinical practice, and to the translation of theoretical and practical knowledge.

Implications for nursing practice

Accurate clinical indicators of the nursing diagnosis deficient knowledge facilitate the clinical reasoning of nurses and favor the professional's role in the development of health education strategies focused on the acquisition of knowledge about the disease by patients, family members, and caregivers.

Purpose: When submitted to mutilating surgery, patients with head and neck cancer may present profound physical and psychosocial changes, temporary or definitive, with an individual impact and family. The most frequent human responses to this health condition are subjective, particularly concerning body image and self-esteem. Differential validation emerges to provide nurses with clinical indicators for clinical reasoning and planning of effective interventions, considering the specific defining characteristics of each diagnosis. This study aimed at conducting a differential clinical validation of the nursing diagnoses of disturbed body image and low situational self-esteem in patients with head and neck cancer using Q methodology.

Method: We performed an observational cross-sectional study using Q methodology. Thirty-eight participants were included. Data were analyzed using Q methodology procedures. The research and ethics committee approved the study.

Findings: Factor analysis was performed, and eight defining characteristics were identified for the diagnosis of disturbed body image and one for situational low self-esteem: avoiding looking at one's body, avoiding touching one's body, the behavior of monitoring one's body, depersonalization of body part by use of impersonal pronouns, focus on past appearance, focus on past function, negative feeling about body, and refusal at acknowledging change and helplessness.

Conclusions: The use of the Q methodology in this process of differential validation allowed the study of two diagnoses of a subjective nature, disclosing the most specific defining characteristics of each and contributing to increasing the knowledge of these human responses.

Implications for nursing practice: This study contributes to a better understanding of the predominant defining characteristics of the diagnoses under study. In terms of the NANDA-I taxonomy, it helps to raise the levels of evidence for diagnoses. Using a qualitative and quantitative methodology allows for maintaining a more holistic research approach and greater rigor and acceptance of the results.

Evaluation of clinical oral care outcomes according to nursing outcomes classification 根据护理成果分类评估临床口腔护理成果

Purpose

This research was planned to follow the healing process of the oral mucosa in patients in intensive care with an “Impaired Oral Mucous Membrane Integrity” nursing diagnosis based on the “NOC (1100) Oral Health Assessment” outcome criteria.

Method

This study, which was planned in a methodological and descriptive type of research, was carried out with 50 patients who were hospitalized in the intensive care clinic of a state hospital between June and December 2022, with a nursing diagnosis of “Impaired Oral Mucous Membrane Integrity.” Data were collected using a Patient Information Form and the “NOC (1100) Oral Health Evaluation Scale” for the nursing outcomes classification. In the analysis of the data descriptive statistical methods, Pearson correlation test, Friedman test as well as Cohen's kappa test were used to evaluate the agreement between two independent observers.

Findings

In the study, content validity index value of the NOC scale was calculated to be 0.90. The examination of the participants’ mean scores on the NOC (1100) Oral Health Assessment Scale showed that there were statistically significant differences in terms of the repeated evaluations, except for the NOC indicators of “Oral mucosal integrity,” “Gum integrity,” and “Tooth integrity” (p < 0.01). No statistically significant correlation was found between the mean NOC scale scores of the patients according to the variables of age, body mass index, mechanical ventilation time, and length of stay in the intensive care unit (p > 0.05).

Conclusions

The findings showed that the Turkish version of NOC (1100) Oral Health Assessment Scale was a valid tool for monitoring the healing process of the oral mucosa in patients in intensive care.

Implications of nursing practice

With the use of NOC (1100) Oral Health Assessment Scale, a common language will be formed in the evaluation for monitoring the healing process of the oral mucosa in nursing care.

The examination of the participants’ mean scores on the NOC (1100) Oral Health Assessment Scale showed that there were statistically significant differences in terms of the repeated evaluations, except for the NOC indicators of “Oral mucosal integrity,” “Gum integrity,” and “Tooth integrity” (<i>p</i> &lt; 0.01).
