OBJECTIVE: To review the concept of quality, the processes of quality assurance, and the methods of quality assessment as they relate to the delivery of pharmaceutical services. DATA SOURCES: Recent management and pharmacy literature. STUDY SELECTION: Not applicable. DATA EXTRACTION: Not applicable. DATA SYNTHESIS: Quality implies value, worth, and degree of excellence. The structure-process-outcome paradigm of Donabedian is an often applied measure of quality. Quality assurance (QA) is the process by which we ensure that services are appropriate, effective, and efficient. Examples of QA programs in pharmacy are those of the performance indicators of the Joint Commission on Accreditation of Healthcare Organizations, criteria used in drug-use evaluations, disease-specific profiles used in disease management, and profiling of provider care systems using instruments such as the Health Plan Employer Data and Information Set (HEDIS, now in version 2.0). OBJECTIVE: To assess perceived importance of drug use review (OUR), pharmacoeconomic analyses, formulary design, and patient-oriented pharmacy services for future academic initiatives in managed care pharmacy. DESIGN: Mailed survey. SETTING: Not applicable. PARTICIPANTS: 405 managed care pharmacy directors. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Percentage of respondents identifying the importance of the topics. RESULTS: A response rate of 59.5% was achieved; 81% of the respondents thought CE programs related to specific topics would be valuable to persons in their positions. The longer the respondents held their present positions, the less likely such programs considered to be valuable. Pharmacists placed more emphasis than did non-pharmacists on topics related to traditional pharmacy practice in managed care settings. OBJECTIVE: To describe selected alternative medicine practices and discuss their value in managed care settings. DATA SOURCES: Recent clinical and alternative medicine literature. DATA SYNTHESIS: A central difference between conventional and alternative medicine lies in their philosophies about their roles in affecting or controlling illness. While conventional practitioners attempt to cure the patient by acting as authoritative experts, alternative practitioners try to teach the patient to tap his or her own inner healing resources. Alternative practitioners hold that the individual has the capacity to maintain health and that people get sick when they are out of balance with their environment or society. Alternative medical practices typically address a spiritual component of health and illness largely ignored in conventional medicine. It encompasses a variety of care environments that offer high quality yet less-expensive alternatives to traditional nursing homes and represents a wide array of services formats that differ in name and description among states and regions. Health improvements, increased attention to quality of life, and the need to reduce the cost of care will stimulate the growth of long-term care facilities that are alternatives to nursing homes.
