Black stain and dental caries in primary dentition of preschool children in Qingdao, China. 中国青岛学龄前儿童乳牙的黑斑和龋齿。

Black stain (BS) and caries are common oral issues in children worldwide. This study aimed to reveal the prevalence of BS and caries in primary dentition of children in Qingdao, China and evaluate the potential association between them. A total of 672 preschool children aged 3-5 years old from 12 kindergartens in Qingdao, northern China were enrolled in the study. The prevalences of BS and caries were counted, and their distributions across different ages and genders were analyzed. The potential protective role of BS (independent variable) on caries (dependent variable) was analyzed via binary logistic regression analysis. BS was observed in 103 children (15.33%), including 3 severe- (0.45%), 28 moderate- (4.17%), and 72 mild (10.71%) forms. Caries was observed in 374 children (55.65%) and it was positively associated with age. Notably, the prevalence of caries was lower in children with BS than in those without BS (42.72% vs. 58.00%, respectively). Children with BS were less likely to suffer from caries than those without BS. BS in mild or moderate/severe forms was associated with a low risk of caries. In summary, the prevalence of BS in primary dentition in Qingdao, China is at a relatively high level worldwide. BS is a protective factor for caries in primary dentition.

Knowledge, attitudes, and practice of general pediatricians and pediatric subspecialists towards oral health in children: a survey in Turkey. 土耳其普通儿科医生和儿科亚专科医生对儿童口腔健康的认识、态度和实践:一项调查。

This study aimed to evaluate the level of knowledge, attitudes, and practices of general pediatricians (GPs) and pediatric subspecialists (PSs) practicing in Turkey toward oral health in children. A national sample of 642 pediatricians who attended the Turkish National Pediatrics Congress completed a survey consisting of 36 questions. The results are segmented based on sex, years of experience, and whether the pediatrician was a general pediatrician or pediatric subspecialist. Relationships between dependent categorical variables were tested using the Chi-square test. Four hundred eighty-seven questionnaires were completed, resulting in a 75.8% response rate; 69.8% of general pediatricians and 74.1% of pediatric subspecialists recommended the first dental visit before the first year or eruption of the first teeth; 62.6% agreed that pediatricians have a role in inculcating oral hygiene habits in patients; 98.2% of the respondents indicated they had performed oral examinations on their patients since birth; 72.8% of PSs suggested bottle-feeding to their patients, whereas only 33.5% of GPs did; 65.4% of PSs, 78.2% of GPs (p = 0.003), and 76.8% of the physicians with <5 years experience recommended fluoridated toothpaste under 2 years of age. There were statistically significant differences between the knowledge and attitudes of GPs and PSs. Female respondents were more knowledgeable than male respondents about oral health in children. Additionally, years of experience did not correlate with increased knowledge and proper attitudes toward oral health in children.

Management of oblique root fracture in the middle third of two maxillary immature central incisors with severe caries in mixed dentitions: a case report. 混合牙列中两颗上颌未成熟中切牙中1/3斜根折伴严重龋齿的处理:病例报告。

Mid-root fractures are rare injuries in young permanent teeth and tend to have poor prognoses. This study presents a case of oblique root fracture of both maxillary immature central incisors in the middle third accompanied by delayed dental visit and severe caries of all primary teeth. After restoring all the primary and permanent teeth that needed stabilization, the coronal fragments were repositioned and stabilized with a flexible splint consisting of orthodontic wire and composite resin. A comprehensive and sequential dental treatment for other oral diseases and oral hygiene instructions were provided. A 16-month follow-up revealed that the two injured young permanent incisors were healed, surrounded by hard tissues and continued to grow both in length of the root and thickness of the root canal wall, with significant improvement in oral hygiene. Based on the outcome of this case, initial stabilization without endodontic therapy could be considered a successful treatment modality for young permanent teeth with oblique root fracture due to the growth of fractured teeth with vital pulp and the maintenance of natural dentition.

Image segmentation of impacted mesiodens using deep learning. 利用深度学习对撞击性间质进行图像分割。

This study aimed to evaluate the performance of deep learning algorithms for the classification and segmentation of impacted mesiodens in pediatric panoramic radiographs. A total of 850 panoramic radiographs of pediatric patients (aged 3-9 years) was included in this study. The U-Net semantic segmentation algorithm was applied for the detection and segmentation of mesiodens in the upper anterior region. For enhancement of the algorithm, pre-trained ResNet models were applied to the encoding path. The segmentation performance of the algorithm was tested using the Jaccard index and Dice coefficient. The diagnostic accuracy, precision, recall, F1-score and time to diagnosis of the algorithms were compared with those of human expert groups using the test dataset. Cohen's kappa statistics were compared between the model and human groups. The segmentation model exhibited a high Jaccard index and Dice coefficient (>90%). In mesiodens diagnosis, the trained model achieved 91-92% accuracy and a 94-95% F1-score, which were comparable with human expert group results (96%). The diagnostic duration of the deep learning model was 7.5 seconds, which was significantly faster in mesiodens detection compared to human groups. The agreement between the deep learning model and human experts is moderate (Cohen's kappa = 0.767). The proposed deep learning algorithm showed good segmentation performance and approached the performance of human experts in the diagnosis of mesiodens, with a significantly faster diagnosis time.

Effect of electronic book "Traumatic dental injury to children's permanent teeth" on primary school teacher's knowledge. 电子书《儿童恒牙外伤》对小学教师知识的影响。

Most dental trauma in children aged 8-12 years occurs in schools. Teachers should properly and timely manage the emergency pertaining to traumatic dental injuries before paying visit to dental health center. Studies had revealed that the elementary school teachers carried poor knowledge of managing the dental trauma. Electronic books were the easily accessible digital and visual educational media for the users through internet-connected devices. This study compared the knowledge of primary school teachers before and after reading the innovative electronic book "Traumatic Dental Injury to Children's Permanent Teeth". The questionnaire was shared via a link, before and after reading this electronic book. The contents of electronic book included the kinds of trauma, management, and preventive measures as explained via appealing illustrations. The median score of knowledge before the intervention was 6 (poor), and afterwards it was 13 (good). Wilcoxon test on scores before and after reading this book resulted in statistically significant difference p-value ≤ 0.05. The electronic book "Traumatic Dental Injury to Children's Permanent Teeth" was innovative, and effective educational media having high impact of increasing the dental trauma knowledge among primary school teachers.

In the current odontological era, carious lesions are removed while tooth tissue is preserved. Most of these ideals are met by chemomechanical caries removal (CMCR) methods, which are easy and comfortable to use, differentiate and eliminate infected tissues, minimize pressure, vibration and heat, and are cost-effective. This study examines the efficacy of commercially available CMCR agents, namely Papacarie®, Carie-Care™ and BRIX3000™, and a conventional hand instrumentation method for caries removal in deciduous molars in terms of time consumption, ease of application, and pain perception. For this randomized clinical trial, 120 children aged 4 to 9 years were selected and randomly allocated to four groups of 30 patients each. Time consumption, ease of application, and pain perception were evaluated at three intervals: pre-, during- and post-caries removal, using Wong-Baker FACES (WBF) Pain Rating Scale and the Face, Legs, Activity, Cry, Consolability (FLACC) scale. The results showed that among the compared materials and conventional hand instrumentation technique, Carie-Care™ was statistically found to be the least time-consuming with a p-value of 0.019, have the least pain perception with a p-value of 0.02, and was clinically the best with respect to manipulation and handling. While all three CMCR agents aid in the removal of carious tissue, Carie-Care™ was the most effective based on time consumption, pain perception and simplicity of administration.

PHACES syndrome is an acronym for the syndromic presentation of Posterior fossa malformation, Hemangioma, Arterial anomalies, Coarctation of aorta/cardiac defects, Eye abnormalities and Sternal malformations. Infantile hemangiomas are the most common tumors of infancy. Regional odontodysplasia, commonly referred to as "ghost teeth", is a rare localized developmental malformation of enamel and dentin with varying levels of severity that results in unusual clinical and radiographic appearances of affected teeth. This report describes a rare case of a two-year-old Caucasian male diagnosed with PHACES syndrome also presenting with multi-regional odontodysplasia. Ten of twenty teeth were dysplastic. The patient was treated under general anesthesia in a hospital setting. All affected primary teeth were extracted due to sensitivity, abscess and extremely poor long-term prognosis. Moving forward, a long-term interdisciplinary approach will be necessary to address this child's dentition as it develops.

Patients being reported for vitamin D deficiency (VDD) are increasing, particularly among the children and adolescents. This study aims to manifest the clinical and dental evaluations of a child with VDD, referred to the dental office. A 10-year-old British Asian boy was referred to the paediatric specialist dentistry clinic by the general dentist for dental management. The medical history depicted that the patient was diagnosed with VDD, secondary hyperparathyroidism and delayed growth. Moreover, his mother had the VDD during pregnancy. The patient was breast fed and had rickets in infancy. He was prescribed vitamin D supplements at the age of 16 months. He had received multiple dental treatments under local anaesthesia but with limited cooperation. Clinical examination revealed that the patient had chronological enamel hypoplasia shown as bands at the occlusal third on specific teeth. Suboptimal hygiene with general plaque induced gingivitis, dental caries in permanent and primary teeth, and delayed the teeth eruption. Preventions included appropriate oral hygiene and dietary advice, fluoride varnish application and fissure sealant placement. The treatments included anterior direct composite restoration, posterior composite restoration, stainless steel crowns and extractions. Thorough medical history is essential to understand the underlying causes of dental defects. Early dental intervention can restore the patient appearance and function and prevent further dental damage.

The term "artificial intelligence" (AI) has become increasingly prevalent, but it is essential to provide clarity before proceeding. AI pertains to the theory and creation of systems capable of executing tasks that typically necessitate human intelligence. Integrating artificial intelligence (AI) in pediatric dentistry has emerged as a promising avenue to enhance patient care, improve diagnostic accuracy, streamline treatment planning, and augment patient engagement. AI-driven tools such as image analysis, natural language processing, and machine learning algorithms assist in early caries detection, orthodontic treatment planning, behavior management, and personalized oral hygiene education for pediatric patients. This paper presents an overview of AI's applications in pediatric dentistry, particularly behavior management, highlighting its potential to revolutionize traditional pediatric dental practices.
