Epidemiological survey of post-void residual urine volume in older adult patients in an acute-care hospital: A cross-sectional observational study 急诊医院老年患者排尿后残余尿量的流行病学调查:一项横断面观察研究。

Aim

To epidemiologically determine post-void residual urine volume (PVR) and identify the characteristics of “potential” older adult patients with voiding dysfunction in an acute-care hospital.

Methods

A cross-sectional study was conducted on 614 older adult patients in an acute-care hospital between November 2018 and May 2019. Using portable ultrasound devices, nurses measured the patients' PVR three times after admission. Lower urinary tract symptoms were evaluated using the Core Lower Urinary Tract Symptom Score prior to admission.

Results

A total of 107 older adult patients (17.4%) had episodes of abnormal PVR ≥100 mL. An age of over 75 years (adjusted odds ratio [aOR]: 1.899) and feeling of incomplete emptying (aOR: 4.337) were associated with having at least one episode of abnormal PVR, whereas neurological or circulatory diseases (aOR: 3.699) were associated with having two or three episodes.

Conclusions

The results showed a high prevalence of abnormal PVR in older adult patients. Screening for PVR in older adult patients aged over 75 years who experience incomplete emptying or neurological or circulatory diseases may help identify those at risk for voiding dysfunction.

Relationship between illness perception and self-management behaviors among Chinese diabetic foot patients 中国糖尿病足患者疾病认知与自我管理行为的关系。

Aim

The aims of the present study are to describe the status of self-management behaviors and illness perception, and explore the relationship between illness perception and self-management behaviors among Chinese diabetic foot patients.

Methods

A cross-sectional study was conducted at the endocrinology department of a comprehensive tertiary hospital in Guangzhou, China. Data were collected on illness perception, self-management behaviors, and demographic and clinical characteristics over 9 months among 156 subjects. Data were analyzed using Pearson correlation analysis, univariate analysis and multiple linear regression analysis.

Results

Only 3.2% of participants maintained excellent self-management behaviors. Additionally, the participants perceived diabetic foot as chronic and could be well controlled through treatment. Multiple linear regression analysis revealed that illness perception was associated with self-management behaviors.

Conclusions

Patient illness perception is an important factor influencing self-management behaviors. It may be helpful to improve self-management behaviors by tailoring the content of the intervention to fit the patients' illness perceptions.

The moderating role of work-to-family spillover in the association between nurses' work values and work engagement: A cross-sectional study 工作对家庭溢出在护士工作价值观和工作投入之间的关系中的调节作用:一项横断面研究。

Aim

Although researchers have emphasized the importance of enhancing work engagement in nurses to ensure work-related well-being, the underlying mechanisms of the influencing factors of work engagement remain unclear. This study aimed to elucidate whether work-to-family spillover moderates the relationship between nurses' work values and work engagement.

Methods

In total, 2600 nurses from 52 hospitals in the Tohoku region of Japan were recruited, and 1587 nurses participated. The questionnaire included items on demographic characteristics, the Nurses' Work Values Scale, the Japanese version of the Work-life Balance Scale, and the Work Engagement Scale. Data were analyzed using hierarchical multiple regression.

Results

The interaction terms of negative work-to-family spillover with extrinsic work values and prestige work values were significantly associated with work engagement. The interaction terms of nurses' work values and positive work-to-family spillovers did not show a significant association.

Conclusions

Nurses who emphasized extrinsic work values and those who did not emphasize prestige work values had the lowest work engagement in settings with high negative work-to-family spillover. To ensure nurses' work-related well-being and engagement, nursing managers could provide support for nurses in tackling work-related negative events, so that these do not carry over to family settings.

Pediatric outpatient experiences of senior nursing students: A qualitative study 高年级护生的儿科门诊经历:一项定性研究。

Objectives

This study aimed to explore the experiences of senior nursing students working in a pediatric outpatient clinic.

Methods

This study adopted a descriptive phenomenological approach, through semi-structured interviews. Purposive sampling was used to select 12 senior nursing students in the pediatric outpatient clinic of West China Second University Hospital, Sichuan University, China, from August 2020 to March 2021. The collected data were analyzed using the Colaizzi method.

Results

Four themes were identified in this study: knowledge and skills (triage, emergency care), communication (courage, communication skills), value re-evaluation (understand of outpatient nurses, judgment of outpatient nurses), and outpatient feelings (satisfaction, empathy, pressure).

Conclusion

Pediatric outpatient internships have a positive impact on senior nursing students. Research has found that constructing a nursing clinical practice pathway in pediatric outpatient clinics helps teachers and students clarify the objectives and content of internships, and the “shadow teaching” model is conducive to nursing students' empathy and thinking. In addition, outpatient internship experience can help students gain communication courage.

Aim

This study investigated the effects of differences in oral function and depressive tendencies on nutritional status in older adults requiring support or low-level care.

Methods

In 106 older adult participants residing in nursing/care homes or participating in community-level preventive care activities, the Mini Nutritional Assessment-Short Form (MNA®-SF) for nutritional status and oral diadochokinesis (ODK), tongue pressure, and repetitive saliva swallowing test (RSST) for oral function, 15-item Geriatric Depression Scale (GDS), Diet-Related Quality of Life Scale-Short Form (DRQOL-SF), and Functional Independence Measure (FIM) were administered. Basic information, including cognitive function, was evaluated. Hierarchical MNA (dependent variable)-based multiple regression analysis was performed, followed by path analysis using factors with significant associations with MNA scores.

Results

RSST, ODK, tongue pressure, FIM, and DRQOL were positively correlated with MNA scores and GDS was negatively correlated. Hierarchical multiple regression demonstrated associations with tongue pressure, GDS, FIM, DRQOL scores, and gender. Path analysis confirmed significant paths from tongue pressure to MNA, tongue pressure to FIM, and FIM to MNA (P < .001). Significant paths were confirmed from GDS to MNA (P < .01), from DRQOL to MNA (P < .05), and from gender to MNA (P < .01).

Conclusions

Tongue pressure, GDS, FIM, DRQOL scores, and gender were identified as factors that directly affect MNA. Tongue pressure showed the greatest effect and indirectly affected MNA via FIM. These findings emphasize the importance of early detection of low nutritional risk for preventing depression and oral function deterioration, and of evaluation of dietary satisfaction and improvement of quality of life in diets.

Significant paths were confirmed from GDS to MNA (<i>P</i> &lt; .01), from DRQOL to MNA (<i>P</i> &lt; .05), and from gender to MNA (<i>P</i> &lt; .01).</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Conclusions</h3>\u0000 \u0000 <p>Tongue pressure, GDS, FIM, DRQOL scores, and gender were identified as factors that directly affect MNA. Tongue pressure showed the greatest effect and indirectly affected MNA via FIM. Correlates of delirium care competency among shift leader nurses in acute medical wards in Japan: A cross-sectional study 日本急诊病房值班长护士谵妄护理能力的相关性:一项横断面研究。

Aim

This study identified the correlates of delirium care competency among shift leader nurses in acute medical wards in Japan.

Methods

A cross-sectional study was conducted from November 2019 to February 2020. We sent request letters to a random sample of 381 general acute care hospitals in Japan. Of these, 68 agreed to participate and distributed 735 self-administered questionnaires to shift leader nurses working in their acute medical wards. The questionnaire included the Self-rated Delirium Care Competency Scale for Shift Leader Nurses in Acute Medical Wards (DCSL-M), developed by the authors. It gathered data on the respondents' demographics and delirium care competency, and investigated a total of 25 variables. We calculated descriptive statistics and examined associations between delirium care competency and the demographics using multiple logistic regression analysis.

Results

Of the total, 301 (40.9%) questionnaires were returned. Delirium care competency was high among shift leader nurses who: (1) had experience as a clinical practice preceptor for nursing students; (2) had experience attending training related to dementia or delirium care; (3) worked at a hospital/ward that charged additional medical fees for dementia care; and (4) had access to consulting psychiatrists to refer patients with delirium.

Conclusions

The results suggest the need for efforts to improve delirium care competency among shift leader nurses who work in hospitals that do not charge additional medical fees for dementia care or that do not have consulting psychiatrists to refer patients with delirium.

The effect of an attachment-based intervention program on attachment, expectation, and stress in pregnant women: A randomized controlled study 基于依恋的干预计划对孕妇依恋、期望和压力的影响:一项随机对照研究。

Aim

This study aimed to determine the effect of an attachment-based intervention program (ABIP) on attachment, prenatal expectation and stress levels in pregnant women.

Methods

This randomized controlled study was conducted in the pregnant outpatient clinics of a public hospital in Turkey. The study sample consisted of a total of 154 pregnant women (77 experimental, 77 control) at 28–38 weeks of gestation. The ABIP was applied to the pregnant women in the experimental group for 5–7 days. The ABIP included five interventions: (1) perceiving/counting fetal movements; (2) music therapy; (3) preparation for the baby; (4) writing notes/letters to the baby; and (5) watching images of the fetus/pregnancy.

Results

After the ABIP, pregnant women in the experimental group had higher prenatal maternal attachment and prenatal positive expectation mean scores than those in the control group, and the difference between them was statistically significant in favor of those in the experimental group (P < .001). In addition, pregnant women in the experimental group had lower prenatal negative expectation and prenatal distress mean scores than those in the control group, and the difference between them was statistically significant in favor of those in the experimental group (P < .001).

Conclusions

The results of this study suggest that ABIP is a unique and pioneering program to increase maternal-antenatal attachment and prenatal positive expectations and reducing prenatal negative expectations and distress through diverse interventions. However, further research is required to assess the effectiveness of ABIP on maternal-fetal attachment, prenatal maternal expectations, and prenatal distress.

Resilience of first-line nurses during adaptation to the COVID-19 pandemic: A grounded theory study 一线护士在适应新冠肺炎大流行期间的复原力:一项有根据的理论研究。

Aim

Nurses are a vital resource in providing care to COVID-19 patients. During adaptation to the pandemic, the mental health of nurses was disturbed. The present study aimed to explain the resilience development process and adaptive strategies of first-line nurses during the COVID-19 pandemic.

Methods

This study was conducted based on the grounded theory qualitative approach. Twenty-two Iranian first-line nurses, who were employed in one teaching hospital in Qazvin city, were included via purposive and theoretical sampling. The data were collected through semi-structured interviews and were analyzed based on the Corbin and Strauss approach 2015.

Results

The resilience development process of nurses had three stages including initial confrontation with changes, managing conditions and reorganizing, developing resilience. Professional commitment was identified as a core category that affected all stages of resilience development. Negative emotions, nurses' characteristics, and caring challenges were identified as contextual factors which affected the adaptation of nurses to the COVID-19 pandemic and the development of resilience.

Conclusion

Regarding the importance of professional commitment in the nurses' resilience development and not leaving the profession in the COVID-19 pandemic, it is important to emphasize the ethical values and principles of the nursing profession in practice and especially in the education of nursing students. It is necessary to monitor mental health and provide professional psychological counseling by the healthcare systems; also, nursing managers should follow a supportive leadership style and consider first-line nurses' worries.

Does workplace violence affect healthcare workers' turnover intention? 工作场所暴力是否会影响医护人员的离职意愿?

Aims

This study aimed to explore the association between workplace violence and turnover intention among Chinese healthcare workers and explore the role of gender as a moderator in this relation.

Methods

A cross-sectional survey recruited 692 healthcare workers from a single center in a Chinese province. The content included a questionnaire regarding workplace violence, authoritarian leadership, and turnover intention. The moderated mediation effects were analyzed using the PROCESS tool in SPSS and bootstrap method was used to extract 5000 samples to estimate the 95% confidence interval of each effect.

Results

The results indicated that the effect of workplace violence on turnover intention was mediated by authoritarian leadership. In addition, gender moderated the association between authoritarian leadership and turnover intention.

Conclusions

Healthcare worker managers should establish a workplace violence intervention system and change the leadership style of direct leaders to reduce healthcare workers' turnover intention.

Interventions to reduce nurses' burnout: A systematic review and meta-analysis 减少护士倦怠的干预措施:系统综述和荟萃分析。

Aim

To evaluate the effectiveness of interventions to reduce nurses' burnout.

Design

A systematic review and meta-analysis.

Methods

The research was carried out using the following databases: MEDLINE, CINAHL, Cochrane Library, ULAKBİM Turkish National Database, Science Direct, and Web of Science. The study selection, quality assessments, and data extractions of the included studies were carried out by the researchers independently. The PRISMA checklist was used to assure the quality and transparency of the report. The risk of bias of the included studies was evaluated using the Cochrane Collaboration tool. The meta-analysis was performed using Comprehensive Meta-Analysis (CMA) 3.0 software.

Results

A total of 19 studies including 1139 nurses were included in the study. Of these, only 13 were included in the meta-analysis, as six contained incomplete data. Interventions aimed at reducing burnout in nurses were mostly person-directed interventions. The meta-analysis revealed that attempts to reduce burnout had a small effect on nurses' emotional exhaustion and depersonalization, and a moderate effect on their personal accomplishment.

Conclusions

Interventions are more effective at preventing the sense of personal accomplishment of nurses from decreasing. Evidence in the literature on organization-directed interventions and combined interventions to reduce burnout in nurses is limited. Person-directed interventions are effective at low and medium levels. In future studies, it will be more effective to implement combined interventions including both person-directed and organization-directed interventions to reduce the burnout of nurses.

Of these, only 13 were included in the meta-analysis, as six contained incomplete data. Interventions aimed at reducing burnout in nurses were mostly person-directed interventions. The meta-analysis revealed that attempts to reduce burnout had a small effect on nurses' emotional exhaustion and depersonalization, and a moderate effect on their personal accomplishment.</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Conclusions</h3>\u0000 \u0000 <p>Interventions are more effective at preventing the sense of personal accomplishment of nurses from decreasing. Evidence in the literature on organization-directed interventions and combined interventions to reduce burnout in nurses is limited. Person-directed interventions are effective at low and medium levels.
