Optimizing Geriatric Venipuncture: A DRG-Compatible Team-Based Reengineering Strategy 优化老年静脉穿刺:与 DRG 兼容的基于团队的再造策略

Objective: This study evaluates a reengineered intervention aimed at improving the clinical management of intravenous indwelling needles in geriatric patients, focusing on cost-efficiency within the Diagnosis-Related Group (DRG) payment framework. Methods: The intervention was assessed through a comparative study involving 387 elderly patients in the Geriatric Department of Xuanwu Hospital, between June 2021 and March 2022. The study contrasted outcomes between patients treated before and after implementing a new team-based management protocol in November 2021. Results: Findings indicate enhanced first-attempt venipuncture success, reduced consumable costs, and decreased complication rates in the post-intervention group (P < 0.001), compared to controls. Conclusion: The intervention demonstrates significant benefits in venipuncture efficiency, cost reduction, and patient safety, suggesting its potential for broader adoption in geriatric care.

Factors That Influence the High Number of Elderly People Working in the Informal Sector 影响大量老年人在非正规部门工作的因素

Aim: As individuals age, they are expected to experience a natural progression that usually involves a slight and permanent decrease in bodily functions and physical abilities. Despite this, many older people remain active in the workforce. Purpose: This study seeks to explore the correlation between the health conditions, disabilities, social factors, and demographic circumstances of elderly individuals engaged in both formal and informal employment sectors. Methods: This study utilizes an observational analytical method with a cross-sectional structure. The participants in the research involved all individuals aged 60 years and above who met the outlined criteria, totaling 15,034,946 respondents. The data analysis was conducted using multivariate logistic regression in Model II. Results: Overall, most older adults are working in informal jobs. Moreover, most respondents are 60–69 years old, married, have a primary school certificate, own health insurance and a house, and live with family. In addition, the multivariate analysis shows the relationship between each variable and the informal sector working elderly. It is known that older women have a 1.45 times higher limited educational background, not having savings is 1.21 times, rural areas living 1.93 times, not being the head of the family is linked 1.34 times, poor health conditions are 1.01 times, and having a disability has a 1.20 times higher likelihood of informal-sector employment. Conclusion: Several variables are assigned as the determinants that increase the number of elderly working in informal sectors, including gender, education level, savings ownership, living areas, head of the family role, health status, and disability.

The Impact of the COVID-19 Pandemic on Orthodontic Adult Patients’ Characteristics and Decision on Orthodontic Appliance COVID-19 大流行对成人正畸患者特征和正畸器械决定的影响

Purpose: There is an overall paucity of data examining the specific details of orthodontic patients’ patterns or orthodontic service disruptions possibly influenced by COVID-19 pandemic. Therefore, this study aimed to explore the impact of the COVID-19 pandemic on orthodontic clinic disruption regarding the change in adult patients’ characteristics and decisions of orthodontic treatment devices. Patients and Methods: A retrospective sample of 311 patients receiving orthodontic treatment from 2018 to 2022 were collected and divided into two groups: before (n = 167) and during (n = 144) the COVID-19 pandemic. Demographics, dental indices, the index of complexity outcome and need (ICON), and the degree of treatment difficulty were analyzed. Results: There were fewer students among patients during the COVID-19 pandemic than before (24.5% versus 35.9%, P = 0.036). Compared with patients before the pandemic, more patients selected ceramic brackets or Invisalign during the pandemic (P = 0.022). There were higher percentage of class I dental malocclusions among patients during than before the COVID-19 pandemic (P = 0.044). Moreover, the ICON score and the score of the degree of treatment difficulty were both significantly lower for patients during than before the COVID-19 pandemic (63.9±14.0 versus 58.3±15.3, P=0.001 and 7.4±2.6 versus 6.8±2.6, P=0.049, respectively). Conclusion: The COVID-19 pandemic influenced the characteristics and decisions of orthodontic patients. Those who still came to the orthodontic clinic despite the COVID-19 outbreak may have been those with less malocclusion severity and treatment difficulty. Besides, during the time of covid-19 pandemic, more patients chose ceramic bracket and Invisalign as their orthodontic treatment device rather than conventional or self-ligating metal brackets.

Compared with patients before the pandemic, more patients selected ceramic brackets or Invisalign during the pandemic (P = 0.022). There were higher percentage of class I dental malocclusions among patients during than before the COVID-19 pandemic (P = 0.044). Moreover, the ICON score and the score of the degree of treatment difficulty were both significantly lower for patients during than before the COVID-19 pandemic (63.9±14.0 versus 58.3±15.3, P=0.001 and 7.4±2.6 versus 6.8±2.6, P=0.049, respectively). Conclusion: The COVID-19 pandemic influenced the characteristics and decisions of orthodontic patients. Those who still came to the orthodontic clinic despite the COVID-19 outbreak may have been those with less malocclusion severity and treatment difficulty.
