Cuidados de enfermagem ao paciente transplantado renal: scoping review 肾移植患者的护理:scoping综述

Objectives: Mapping nursing care in kidney transplant patients.Materials and method: A scoping review was conducted according to the recommendations of the Joanna Briggs Institute Reviewers’ Manual. Data were collected through 13 national and international databases from December 2020 to January 2021, following scientific rigor in the selection of the material. The pre-selection was made by reading the title, abstract and introductory text in advance; the materials included in this stage were read in full to define the content for the study.Results: Fifteen studies were included. Of these, 60% are articles; dissertations, manuals, protocols, guidelines and bulletins totaled 40% of the material studied. 86.6% of the material has a quantitative approach. Regarding the methodological design, 73.3% were descriptive/transversal character studies. Regarding the mapping of nursing care, it was possible to divide them into two categories: nursing care after kidney transplantation (immediate, mediated and late) and nursing care after kidney transplantation in primary health/extra-hospital care.Conclusions: It is concluded that the study allowed mapping nursing care to kidney transplant patients in the immediate, late and primary health care periods.

Distal Symmetric Polyneuropathy Pain in Diabetes Mellitus 糖尿病远端对称多神经病变疼痛

Objective: To evaluate neuropathic pain (NP), its intensity, and complications in people with type 2 diabetes mellitus (T2DM) in a city of eastern São Paulo.Method: Cross-sectional study conducted with 96 individuals with T2DM served by primary health units in São João da Boa Vista-SP. The following instruments were used to screen NP: Michigan Neuropathy Screening Instrument, Leeds Assessment of Neuropathic Symptoms and Signs, Douleur Neuropathique 4, and Brief Pain Inventory. The data were analyzed using descriptive and inferential statistics, with a 5 % significance level.Results: Of the 96 people with T2DM for longer than five years, 22.9 % had pain. NP was related to high levels of fasting blood glucose (mean = 214 ± 65.58 mg/dl; p = 0.0002), glycated hemoglobin (mean = 8.8 ± 0.11 %; p < 0.001), absence of a balanced diet (p = 0.0066), obesity (p = 0.023), and high blood pressure (p < 0.001).Conclusion: Higher values ​​of glycated hemoglobin rates increased three times the chance of NP. The screening and management of painful diabetic neuropathy is a challenge but adopting a screening protocol supports the secondary prevention of this manifestation.

Factors Related to Depressive Symptoms in University Students 大学生抑郁症状的相关因素

Objective: To identify and discuss the frequency of depressive symptoms and their associated factors in university students.Materials and method: This is a cross-sectional and descriptive study with a quantitative approach, conducted in a Higher Education Institution. The study was developed with a convenience sample consisting of 571 undergraduate students from different areas. A questionnaire was used to collect sociodemographic/academic data, as well as depressive symptoms and their associations, life habits, emotional aspects, and possible eating disorders. For data analysis, descriptive statistics and Pearson’s chi-square test were used for associations, with a significance level of 5 % (p-value ≤ 0.05).Results: The university students presented depressive symptoms such as sleep disorders (61.1 %) and reduced self-confidence (50.9 %). There was an association of depressive symptoms with the course, satisfaction with academic performance, obesity, consumption of sugars and sweets, and practice and frequency of physical activity.Conclusions: It is expected that the results of this research contribute to the population’s critical and reflective thinking to expand visibility and scientific studies referring to the theme, as well as to increase resources for the management of mental health and reduce the stigmas generated.

Cuidado de la salud mental de las enfermeras: prioridad en la pandemia por la covid-19 护士的心理健康护理:covid-19大流行的优先事项

The COVID-19 pandemic has had several repercussions, particularly on the nurses’ mental health, which is adversely affected in their everyday life role as caregivers; therefore, it is a priority to take care of this issue as part of the human right to health. With this conviction and my experience as a clinical nurse, researcher, and scholar, this editorial presents part of a lesson learned in the support and fostering group for nurses in crisis, who face uncertainty, suffering due to contagion, and the death of their relatives and coworkers.

Nursing Leadership, Associated Sociodemographic and Professional Factors: The Perception of Leaders and Evaluators 护理领导力、相关社会形态和专业因素:领导者和评估者的认知

Objective: To assess the leadership styles of the assistance Nursing professionals and their associated sociodemographic and professional factors.Materials and methods: An analytical and cross-sectional study. The sample comprised 75 Nursing professionals and 170 Nursing assistants selected for convenience. The Multifactorial Leadership Questionnaire was applied to assess leadership and the organizational result variables.Results: The predominant leadership styles were as follows: transformational (mean of 3.43) and transactional (3.40), which presented high correlation rates with satisfaction, efficacy and additional effort. The leadership style least perceived by the staff was corrective/avoidance (2.10). In transformational leadership, behavioral influence and inspirational motivation presented better scores in the leaders’ self-perception. Age, marital status and having children evidenced statistically significant differences with the transformational and transactional leadership styles; work experience was significant with transactional leadership.Conclusions: In their work, Nursing professionals denote encouraging practices that are inherent to transformational leadership, which is related to changes in the organizational culture, leads to motivating and inspiring the subordinates to transcend the routines and increases their satisfaction and commitment to their duties, thus being an important factor in health institutions.

Sociodemographic and Clinical Factors Associated with Postpartum Hemorrhage in a Maternity Ward 产科病房产后出血的社会病理和临床因素

Objective: Checking the sociodemographic and clinical factors associated with the prevalence of postpartum hemorrhage (PPH) in a maternity school.Materials and methods: A quantitative cross-sectional study in a maternity hospital in Salvador, Bahia, Brazil. In data collection, we used a standardized form that contained sociodemographic and clinical data from the medical records of 83 women for the period of 2018. Stata version 14 software was used in the analyses. Bivariate analysis was conducted using Pearson’s or Fisher’s exact tests. Poisson regression was performed with robust variation in multivariate analysis. Prevalence ratios (PR) and respective 95 % confidence intervals were estimated. The significance level of the tests was 5 %.Results: The prevalence of PPH was 38.6 % and 25.6 % for atony as the cause. In the bivariate analysis, there was an association between PPH and non-breastfeeding in the first hour of life (p = 0.039). In the multivariate analysis, it was identified that multiparous women had an increase in the prevalence of PPH by almost twice (PR = 1.97). Not breastfeeding in the first hour of life increased this prevalence more than four times (PR = 4.16).Conclusions: Monitoring multiparous women during birth care and encouraging breastfeeding in the first hour of life may decrease the prevalence of PPH.

Objective: To identify the implications, for Nursing, of pulmonary infections by coronavirus in people with chronic non-communicable diseases and to propose actions for care.Materials and method: A literature review, with a search for primary studies in the Biblioteca Regional Virtual de Saúde, Cumulative Index to Nursing and Allied Health Literature, National Library of Medicine and Scopus databases, from March 15th to March 30th, 2020, in Portuguese, English, and Spanish, with a quantitative and qualitative approach, in adults with chronic non-communicable diseases with respiratory infection by viruses of the coronavirus family, from 2010 to 2020.Results: A total of 11 articles were analyzed, which made it possible to identify guidelines for Nursing actions at the community and hospital levels and in critical care; among the care actions proposed for people with chronic diseases are education in health, encouragement to control the disease, immunization and lifestyle change, monitoring of suspected and confirmed cases, and use of masks in public environments.Conclusions: The study highlights the role of Nursing at all health care levels and the possibilities for learning and improving care actions through the use of evidence obtained from previous experiences.

Reflections: The Responsibility of Doctoral Nurses to Lead a Change in Practice 反思:博士护士领导实践变革的责任

This paper aims to consider the responsibilities of doctoral nurses to lead changes in practice through a very personal reflection of over 52 years in nursing. The reflective learning moves from an early training experience where I learned to ‘do’ to becoming a nursing professor with a doctoral qualification and an ‘evidence-based doer.’ The change witnessed has been considerable. As the highest educated professional, I have learned that doctoral nurses are responsible for leading and directly influencing clinical practice, either as a practitioner, an educator, or a researcher. They are capable of encouraging the development of critical thinking skills and helping practitioners to be curious, take risks with ideas, identify gaps in the evidence base, and be creative in their problem-solving. If the strategic vision for nurses globally is to provide the best quality of patient care, then evidence-based practice is key to leading from the head, hand, and heart. Doctoral nurses understand the patient benefits of a high staff-to-patient ratio and having a critical mass of university qualified nurses and must strive to influence policy to this effect. As each country, particularly in Latin America, develops a critical mass of doctorally qualified nurses, then they can harness their innovation, create new ways of working, attract them back into practice, and strengthen their political voice to lead strategic change. Doctoral nurses must develop their leadership skills and their confidence to lead. They have a responsibility to realise their potential and identify the opportunities to really make a difference.

Effect of the organizational culture for patient safety in the hospital setting: A systematic review 组织文化对医院患者安全的影响:一项系统回顾

Objective: To identify the effect of the organizational culture on patient safety in the hospital context.Materials and methods: A systematic review, without meta-analysis, registered in PROSPERO with number CRD42020162981. Cross-sectional and observational studies were selected that assessed the safety environment and safety culture published between 2014 and 2020 in journals indexed in the EMBASE, Latin American and Caribbean Literature in Health Sciences (Literatura Latinoamericana e do Caribe em Ciências da Saúde, LILACS) via the Virtual Health Library (Biblioteca Virtual em Saúde, BVS), Medline (International Literature in Health Sciences) via PubMed, and Cumulative Index to Nursing and Allied Health Literature (CINAHL).Results: The findings show that a positive safety environment exerts a beneficial effect on the safety culture, favors the notification of events, and enables improvements in the quality of health care.Conclusions: The effective interaction between safety culture and organizational culture is still scarce in the literature. Most of the studies carried out investigate the situational diagnosis and little progress is made in terms of deepening the implications for the professional practice and the repercussions for the safety of hospitalized patients.

Effects of Pulmonary Rehabilitation in Men Compared to Women with Chronic Obstructive Pulmonary Disease in Colombia 哥伦比亚男性与女性慢性阻塞性肺病患者肺部康复效果的比较

Objective: To determine the effects of pulmonary rehabilitation in men and women with a COPD diagnosis in a Colombian clinic.Method: A longitudinal and descriptive study was conducted with 75 COPD patients who entered a pulmonary rehabilitation program during 2018 and 2019. The population was divided into groups according to gender and some sociodemographic characteristics were assessed, such as spirometry expressed in percentage of the predicted value, body mass index, mMRC dyspnea, and capacity for physical exercise, as well as some physiological variables with the six-minute walk test, anxiety-depression (HADS) and quality of life (SGRQ). A p-value<0.05 was considered statistically significant.Results: The cigarette packs smoked per year index was higher in men, 34±29.11 (p<0.001), whereas the exposure to firewood smoke was five times higher in women (p=0.037). All the variables related to the capacity for physical exercise improved significantly (p<0.021), with distance covered standing out: 63.26±60.03 vs. 51.53±61.02 in favor of women. mMRC dyspnea was initially higher in women when compared to men, 0.709±0.287, and presented a greater change in women with 1.294±0.415 when compared to men, 0.736±0.880, at the end of pulmonary rehabilitation.Conclusion: Pulmonary rehabilitation in women shows an increase of 63.26 meters in the distance covered and a 1.294-point reduction in dyspnea; while, for men, these figures were 51.53 meters and 0.736 points, respectively, as well as an improvement in quality of life in men by 11.47 SGRQ points when compared to women: 0.600.
