Stem cell markers in the heart of the human newborn 人类新生儿心脏中的干细胞标记物

The identification of cardiac progenitor cells in mammals raises the possibility that the human heart contains a population of stem cells capable of generating cardiomyocytes and coronary vessels. Several recent studies now show that the different cell types that characterize the adult human heart arise from a common ancestor. Human cardiac stem cells differentiate into cardiomyocytes, and, in lesser extent, into smooth muscle and endothelial cells. The characterization of human cardiac stem cells (CSCs) has important clinical implications. In recent years, CD117 (c-kit) has been reported to mark a subtype of stem/progenitor cells in the human heart, with stem cell-like properties, including the ability to self-renewal and clonogenicity multipotentiality. Snoring in a cohort of obese children: association with palate position and nocturnal desaturations 肥胖儿童的打鼾:与上颚位置和夜间去饱和有关

Purpose of the study: Frequency of habitual snoring is significantly higher in obese than in normal-weight subjects. Obesity and adeno-tonsillar size are risk factors of snoring. Other factors, such as fat distribution and upper airway collapsibility, could explain the relationship between obesity, snoring and obtructive sleep apneas. The aim of the study was to investigate clinical and instrumental significance of snoring in exogenous obese children referred to our department. Methodology: This preliminary study takes part of a larger prospective respiratory sleep study. In 36 consecutive obese children (18 males), body mass index (BMI), BMI Z score and neck circumference were calculated according to age and sex. Nasal patency, tonsil size, palate position scoring were also recorded. An overnight polygraphy was performed using a portable ambulatory device. Statistical analysis was done using SPSS® Statistics 19.0 software for Windows®. Main findings: Snoring, objectively measured by polygraphy, was associated with palate position and with oxygen desaturation index (ODI). The correlation between snoring and ODI completely disappeared when adjusting for palate position scoring. Key conclusions: Low palate position can be identified as an adjunctive, although not unique, factor that can contribute to making snoring and increased desaturation events possibly related to increased risk of upper airway collapsibility during sleep in obese children.

Multipotent stem cells of mother's milk 母乳中的多能干细胞

In recent years the presence of stem cells (hBSCs: human breastmilk-derived stem cells) and epithelial progenitors has been demonstrated in mother’s milk (MM). Stem cells present in samples of fresh MM exhibit a high degree of vitality and this makes possible the performance of cell cultures and to evaluate the differentiation capacity of the hBSCs. The most important datum that expresses the enormous potential of the use of MM stem cells is the presence of a cell population capable of differentiating into the three mesoderm, endoderm and ectoderm lines. The small number of studies and MM samples analyzed and the different sampling methods applied suggest standardization in the collection, analysis and culture of MM in future studies, in consideration of the well-known extreme variability of MM composition, also from the standpoint of cells. The analysis of literature data confirms the uniqueness of MM and its enormous potential. The existence of stem cells within the adult pancreas is supported by the ability of this organ to regenerate its endocrine component in various conditions such as pregnancy and following partial pancreatectomy. Several studies have shown that progenitor or adult stem cells may reside within the pancreas and particularly in the pancreatic ducts, including acinar cells and islets of Langerhans. Stem cells (SC) look like to be the possible solution to a number of human pathologies, including those involving the heart. In fact, some studies based on animal models suggest that SC can be used to repair the damaged cardiac tissue, such as in case of myocardial infarction. In fact it has been demonstrated that it would be possible to produce a quantity of SC sufficient to repair an animal heart having physiology and dimensions as the human heart. The aim of this short review is to examine the different subtypes of SC potentially involved in the heart repair (autologous and heterologous) processes as well as the serious concerns that have still to be overcome before considering SC a sure therapy for the heart diseases: rejections, oncogenesis due to SC high proliferative activity, difficult in ruling their differentiation, massive SC death when introducing them in an ischemic environment, ethical problems when SC are derived from embryos. Very low birth weight infants are particularly vulnerable to bacterial and fungal infections. This leads to a common use of antiinfectives, often on a prophylactic basis. Due to the limited available information and the lack of guidelines, the use of antibacterials and antifungals in preterm newborns admitted to Neonatal Intensive Care Units is characterized by a large variability and these drugs are frequently given with different modalities, particularly as regards dosage and frequency, and in an off-label manner. This article provides an updated overview of the current situation on the use of antiinfectives in prematures, by reporting information derived by an analysis of the literature. Renal endogenous stem cells: a new source for regenerative medicine in preterms? 肾内源性干细胞:早产儿再生医学的新来源?

The creation of new medical approaches based on stem cells to treat chronic kidney disease (CKD) and in particular end stage renal disease (ESRD) has become imperative in recent years, due to the significant burdens of patients affected by renal failure and to the limitations of dialysis and kidney transplantation to solve the problem. The initial prospective of utilizing stem cells for regenerating the affected kidney has been at the basis of excitement and hope for all patients affected by ESRD. Unfortunately, too many challenges have halted the possibility to make such regenerative approach a reality, and the vast majority of patients with CKD and renal insufficiency experience a reduced quality of life associated with high mortality. The problem appears particularly severe when ESDR develops in childhood. Children submitted to kidney transplantation have a 95% of survival rate at 5 years, but only 66% of them survive at 20 years after renal transplant. As a result, patients transplanted in childhood will need repeated renal transplants during their life. Renal regenerative medicine might experience a major renaissance in the next years, developing new methodologies stemmed from the previous attempts. Here, we present some major points to be addressed, in order to open a debate on the potential offered by the different regenerative methodologies: the “exogenous” approach; the “endogenous” approach; the “therapeutic” approach; the “prevention” approach. Aggression in children: unravelling the interplay of genes and environment through (epi)genetics and metabolomics 儿童的攻击性:通过(epi)遗传学和代谢组学揭示基因和环境的相互作用

Aggression inflicts a huge burden on affected children, their families, and society. Estimates for the prevalence of clinical aggression in children range between 2 and 16%, and childhood aggression tends to continue into adulthood. Current psychological treatments and pharmacological interventions are not effective for all children with aggressive behaviors and there is a huge need for more personalized approaches, which requires insight into the heterogeneity and the mechanisms underlying aggression and its associated comorbidities. Here we discuss what is currently known with regard to individual differences in childhood aggression. Studies employing new opportunities in large scale genotyping, epigenetics and metabolomics technology will in future help to explain heterogeneity and highlight pathways from molecule to phenotype. The FP7-ACTION project (Aggression in Children: Unravelling gene-environment interplay to inform Treatment and InterventiON strategies) aims to contribute to knowledge that will help children, their families, teachers and society at large.
