Urgent Pneumonectomy as a Result of an Endoleak Type III-A Late Complication after Aortic Stent Implantation. 主动脉支架植入术后出现III-A型内漏并发症的紧急全肺切除术

We report on an octogenarian, who was hospitalized with acute hemoptysis. Computed tomography angiography revealed a monstrously large thrombosed aortic aneurysm in the left thoracic cavity, completely displacing the lung. Eighteen years ago, the patient had suffered traumatic rupture of the descending aorta loco typico. Surgical exploration revealed a large calcified aneurysmal sac, which had perforated into the left lung. Pneumonectomy was performed, and hemoptysis did not reoccur.

Interventional Closure of a Ruptured Sinus of Valsalva with a Konar-Multifunctional Device. 用konar -多功能装置介入治疗瓦尔萨尔瓦窦破裂一例。

A 14-year-old girl with trisomy 13 presented with signs of respiratory failure. This was caused by the rupture of a sinus of Valsalva aneurysm into the right atrium. A Konar Multifunctional Occluder was used for closure. This is the first report of this device for this indication.

Acute Type A dissection is a life-threatening condition requiring urgent surgical treatment. The operative technique involves repairs of a variety of distal extents of the transverse aortic arch and the downstream aorta. We review the evidence surrounding the extent of repair for acute Type A aortic dissection and describe our approach to this disease.

There is currently no standard, evidence-based approach for sizing the Thoraflex Hybrid prosthesis in frozen elephant trunk surgery. We present regional data on 906 Thoraflex grafts implanted in the United Kingdom between December 2012 and August 2021 to emphasize the heterogeneity in sizing practices and evaluate the impact this may have on clinical outcomes. Highlighting this heterogeneity will help develop an evidence-based approach to prosthesis sizing, thereby aiding decision-making for arch repair.

Iatrogenic aortic dissection (IAD) is a rare but devastating complication in cardiac surgery and related procedures. Due to its rarity, published data on emergency surgery following IAD are limited. Herein, we discuss IAD occurring intra- and postoperatively, including those occurring during transcatheter aortic valve replacement and cardiac catheterization, and present benchmark data from our consecutive, single-center experience. We demonstrate changes in patient characteristics, surgical approaches, and outcomes over a 23-year period.

Despite the improvements, spinal cord ischemia is still one of the major and most dramatic potential complications after thoracic and thoracoabdominal aortic treatments, for both open and endovascular procedures. A multimodal approach, which includes several intraoperative and postoperative maneuvers, may contribute to optimizing the spinal cord tolerance to ischemia. The aim of this article is to report the different techniques employed to improve spinal cord perfusion, directly and indirectly through collateral circulation.

The frozen elephant trunk (FET) procedure is known as an effective treatment option for patients with any aortic pathology involving the aortic arch. However, there is growing evidence that many patients often require secondary intended, expected, or unexpected aortic reinterventions during follow-up. In those with underlying aortic dissection pathology, a substantial risk for developing distal stent-graft-induced new entries (dSINEs) has been identified as one cause for secondary aortic reinterventions. dSINE can develop at any time after the FET procedure. Endovascular treatment is generally feasible and safe to close the newly formed entry with low procedural risk. Nevertheless, all patients need continuous follow-up after FET treatment, ideally in a specialized aortic outpatient clinic.

Neurological complications following aortic surgery are most often cerebrovascular accidents due to embolism, or spinal infarcts resulting in hemiparesis or hemiplegia. Guillain-Barre syndrome is a rare cause of quadriparesis. Here, we report a 49-year old male who presented with acute aortic dissection and underwent the Bentall procedure following which he developed quadriparesis, subsequently diagnosed to be a case of Guillain-Barre syndrome. He was successfully treated with intravenous immunoglobulin.
