Aim: To explore the experiences of adults who completed a constraint-induced movement therapy (CIMT) programme, and the barriers and enablers to their participation.
Methods: Qualitative design using semi-structured interviews. Stroke and brain injury survivors (n = 45) who had completed CIMT as part of their usual rehabilitation were interviewed 1 month post-CIMT. Interviews were audio-recorded, transcribed and imported into Nvivo for analysis. Inductive coding was used to identify initial themes. Themes were then deductively mapped to the Capability, Opportunity, Motivation - Behaviour system, a behaviour change model, to identify barriers and enablers to CIMT programme adherence and engagement.
Results: Enablers influencing participation included being provided with education about the programme (Capability - psychological), seeing improvements in arm function (Motivation - reflective), being committed to the programme (Motivation - reflective) and having strong social support from staff, family and allied health students (Opportunity - social). The structured programme was a motivator and offered a way to fill the time, particularly during inpatient rehabilitation (Opportunity - physical). Barriers to participation included experiencing physical and mental fatigue (Capability - physical) and frustration early in the CIMT programme (Motivation - automatic), and finding exercises boring and repetitive (Motivation - automatic).
Conclusion: Therapist provision of educational supports for CIMT participants and their families is important to maximise CIMT programme uptake. During CIMT delivery, we recommend the provision of positive feedback and coaching in alignment with CIMT principles, and the inclusion of social supports such as group-based programmes to enhance participant adherence.

Endoscopy is a diagnostic and therapeutic procedure used in paediatrics. This invasive procedure is anxiety-inducing, a natural response that can affect physiological indicators. Today, the use of nursing theories in the form of a comprehensive nursing care model plays a significant role in improving patient health. The aim of this study was to determine the effect of the comfort-based care model on the physiological indicators of children undergoing endoscopy. This clinical trial study was conducted on 56 paediatric patients referred to the endoscopy unit of Hajar Hospital (affiliated with the Shahrekord University of Medical Sciences, Iran). Children were enrolled by convenience sampling and randomly assigned to either the intervention (28 participants) or control group (28 participants). The intervention, which was based on comfort theory, was carried out 2 hours before endoscopy, encompassing four dimensions: physical, psycho-spiritual, socio-cultural and environmental. The control group received routine care. Physiological indicators were recorded in both groups before and immediately after endoscopy. Data were analysed with the Chi-square, independent t-test and paired t-test. Before the intervention, there was no statistically significant difference between the intervention and control groups in terms of their mean pulse or systolic and diastolic blood pressures. After the intervention, the mean pulse and systolic blood pressure in the intervention group was significantly decreased compared to the control group (P<0.05). The mean diastolic blood pressure after the intervention was 73.39±8.17 in the intervention group and 73.35±9.09 in the control group, and the difference was not statistically significant (P=0.801). The research findings show that the comfort-based care model had a significant effect on reducing pulse and systolic blood pressure.

The NHS long term plan sets a target that by 2028, 75% of cancers will be diagnosed at an early stage. One of the ways this ambition will be reached is through targeted screening and personalised surveillance of those most at risk of developing cancer, such as those with Lynch syndrome. This article outlines the essential steps to consider when setting up and managing an in-house nurse-led colorectal Lynch syndrome clinic. It highlights the educational requirements, patient criteria, the importance of thorough planning and the need to liaise-on with key stakeholders.

The effects of microbiota on neonatal bilirubin are poorly understood, which creates a critical knowledge gap about how they affect the neonate's blood bilirubin levels and how the gut delicately balances close contact with the host. Prebiotics and probiotics have a significant impact on the management of hyperbilirubinemia, but their effectiveness is still being debated. To offer a compelling novel therapeutic approach for newborn hyperbilirubinemia, a thorough understanding of the process is required.

Liver cirrhosis is a chronic, life-threatening disease. Studies show that the 1-year mortality rate is 57%. According to the World Health Organization, palliative care can improve the quality of life for patients with liver cirrhosis, reduce unnecessary hospital admissions and lower healthcare costs. However, there is limited research on the use of palliative care in patients with liver cirrhosis. Four studies were identified that explored interventions conducted for patients with alcoholic liver cirrhosis related to palliation. Three studies also examined the experiences of patients, relatives, and professionals with palliation. Three themes emerged from the examination of these articles: organisational structure, lack of knowledge, and anxiety about discussing the subject.
