Evaluation of Inflammatory Markers in COVID-19 Disease in Children 儿童COVID-19疾病炎症标志物的评价

The lymphocyte count, platelet count, mean platelet volume (MPV), plateletcrit (PCT), C-reactive protein (CRP), neutrophillymphocyte ratio (NLR), platelet-lymphocyte ratio (PLR), and systemic immune-inflammation index (SII) obtained from the patients' complete blood count were evaluated. Children are less affected by COVID-19 than adults (1). Since the first case was reported by Chan et al. on January 20, 2020, the number of cases has been gradually increasing (2). According to the inclusion criteria, patients under 18 years of age with positive PCR test were included in the study. Electronic impedance + optical scatter and blood gas tests were performed on an ABL80 FLEX BASIC analyzer using the electrochemical biosensor method in the Diyarbakır Pediatric Disease Hospital laboratory.

Diphtheria Epidemiology in Türkiye Throughout History 历史上日本白喉流行病学

Difteri hastalığı çok eski çağlardan bu yana salgınlarla seyreden ve çocukluk döneminin önde gelen ölüm nedenlerinden biri olmuştur. On dokuzuncu yüzyılın sonlarında difteri antitoksinin bulunması ve sonrasında yaygın kullanımıyla fatalite hızında önemli düşüşler sağlanmıştır. 1920’lerin erken dönemlerinde difteri toksoid aşısının geliştirilmesiyle salgınlarda azalma görülmüş, 1971 yılında Dünya Sağlık Örgütü tarafından Genişletilmiş Bağışıklama Programının hayata geçirilmesiyle difteri vakalarında %90’ın üzerinde azalma sağlanmıştır. Bununla birlikte dünyada halen aşılama kapsayıcılığında düşüşle sonuçlanan savaşlar, göçler, aşı tereddütü gibi nedenlerle difteri salgınları görülmeye devam etmektedir. Difteri hastalığı 19. yüzyıl sonları ve 20. yüzyıl başlarında Osmanlı İmparatorluğu coğrafyasında yaygın olarak salgınlarla seyretmiştir. 1895 yılından itibaren difteri antitoksini imparatorlukta üretilmeye başlanmış, çıkarılan nizamname ve layihalar ile bildirim zorunluluğu getirilmiş, tanı, tedavi ve hastalığın kontrolüne yönelik alınması gereken önlemlere yönelik ayrıntılı talimatlar verilmiştir. Erken Cumhuriyet döneminde, koruyucu hizmetlere ağırlık verilmiş, aşı üretimi ve tanı olanakları için Refik Saydam Merkez Hıfzıssıhha Enstitüsü ve personel eğitimi için Hıfzıssıhha Okulu açılmış, 1930’da hastalık ve salgınların önlenmesine yönelik önlemlere yer veren Umumi Hıfzıssıhha Kanunu çıkartılmıştır. Sağlık hizmet sunumunun yaygınlaştırılması, Genişletilmiş Bağışıklama Programının hayata geçirilmesi ve tüm bileşenleriyle etkin bir aşılama hizmetinin sürdürülmesiyle 1958 yılında 4999 vaka ve 661 ölümün bildirildiği difteri hastalığı 2011 yılından bu yana bildirimi yapılmamış bir hastalık durumuna gelmiştir.

Altı Ay ile Beş Yaş Arasındaki Çocuklarda Alt Solunum Yolu Enfeksiyonları ile D Vitamini ve Çinko Düzeyleri Arasındaki İlişki

Objective: We performed this study to investigate serum vitamin D and zinc levels in lower respiratory tract infections in children aged six months to five years old. Material and Methods: In this retrospective study, 223 children admitted to the Department of Pediatrics at Kastamonu Training and Research Hospital between November 1, 2021, and March 1, 2022, were included. The study group consisted of 111 children with lower respiratory tract infections. The control group consisted of 112 children who were healthy and had not been diagnosed with lower respiratory tract infections in the last one month. Vitamin D and zinc levels of the patient and control groups were compared in the study. Additionally, the data of inpatients and outpatients, and those with or without the need for intensive care were compared in the patient group. Results: Serum vitamin D (p= 0.03) and zinc levels (p= 0.01) were significantly lower in the study group than in the control group. Vitamin D (p= 0.04) and zinc levels (p= 0.01) were significantly lower in the inpatients than the outpatients, and in those with the need for intensive care compared to those without the need for intensive care. As the vitamin D levels of inpatient children decreased, the length of hospitalization was significantly prolonged (p= 0.01). Conclusion: A decrease in serum vitamin D and zinc levels leads to an increase in the risk of lower respiratory tract infections and the severity of the disease in children aged six months to five years.

COVID-19 Enfeksiyonuna Bağlı Gelişen Serum Transaminaz Yüksekliği

Although COVID-19 was first described as a respiratory disease, current data has shown that it is a disease with multisystemic involvement including respiratory, cardiovascular, gastrointestinal, neurological, hematological and immune systems. COVID-19 associated liver injury may be due to various potential mechanisms. Direct viral cytotoxic effect, immun mediated injury, drugs, ischemic injury due to hypoxia-hypoperfusion are among these mechanisms. Here we present a five year-old male patient who had no known history of liver disease admitted to our clinic due to elevated transaminase during the course of COVID-19 infection.

Different Tuberculosis Clinics in Two Unimmunized Siblings: Caviter Pulmonary Tuberculosis and Tuberculous Pleurisy 两个未免疫的兄弟姐妹的不同结核临床:空洞性肺结核和结核性胸膜炎

Tuberculosis is a health problem that preserves its importance in developing countries and populations with low socioeconomic means. The treatment process is long-term, laborious, costly and has high morbidity and mortality with many complications. The most important step to get rid against this disease is to prevent contamination. For this purpose, routine vaccination and chemoprophylaxis of the contacts are of great importance. In this case report, while examining different tuberculosis presentations in the pediatric population, we emphasized the importance of vaccination and screening the households that the index patient is in contact with, the treatment of patients, and the importance of chemoprophylaxis in contacts.

According to this guideline, in the presence of epidemiological and clinical findings, SARS-CoV-2 RNA RT-PCR positive cases were accepted as confirmed cases, and SARS-CoV-2 RT-PCR negative cases were accepted probable cases (10).
