Perioperative Communication Gaps: A Qualitative Exploration. 围手术期沟通缺口:定性探讨。

Communication breakdowns are a leading cause of sentinel events in health care environments, including perioperative areas. Effective interdisciplinary communication exchanges can contribute to staff member collaboration and patient safety. This qualitative research study explored perceptions associated with communication among surgical services personnel across the perioperative phases of care. At a variety of sites in a midwestern health system, focus group participants (ie, OR RNs, preoperative and postoperative RNs, surgical technologists, surgical assistants, patient care assistants, and sterile processing technicians) discussed communication workflows and areas of communication gaps. Focus group data were then analyzed using rapid qualitative analysis techniques, which revealed three themes regarding communication gaps: technology impacts on communication, communication inconsistencies, and communication omission and audience. Although additional research is needed, perioperative nurses should be aware of communication gaps as they strive to enhance patient safety, improve workflow efficiency, and foster a positive perioperative care environment.

Guidelines in Practice: Sharps Safety. 实务指引:利器安全。

Sharps injuries are a recognized and preventable occupational risk among health care workers, particularly those who work in the perioperative setting. The recently updated AORN "Guideline for sharps safety" provides perioperative team members with information on identifying potential sharps hazards and interventions to prevent sharps injuries. This article provides an overview of the guideline and discusses recommendations for an organizational sharps safety program, evaluating and selecting devices with engineered sharps injury prevention features, using devices with engineered sharps injury prevention features, neutral zone and safer sharps handling, sharps disposal, and personal protective equipment and double gloving. It also includes a scenario related to the implementation of safety-engineered sharps devices. Perioperative nurses should review the guideline in its entirety and apply the recommendations when handling or disposing of sharps.

Improving OR first-case on-time starts (FCOTS) is a multifactorial challenge for perioperative nurses and leaders seeking to improve efficiency. This article describes the implementation of a quality improvement project at a hospital-based ambulatory surgery center to decrease FCOTS delays, which did not allow the organization to meet its FCOTS adherence goal of 80% or higher, averaging 59% in 2023. Through the creation and adoption of standard work documents to reduce variability, nurses were given a preoperative time frame within which to complete the nursing components required to prepare a patient for the OR. When comparing intervention and baseline data after the three-month intervention period, a significant reduction in total minutes delayed was noted-from 1,422 to 799 minutes (P < .001)-as well as an improvement in FCOTS compliance to 82%. Estimated cost savings were $35,111 for the three-month intervention period, demonstrating that modifications to nursing workflows alone can improve perioperative efficiencies.
