Survey Finds That Medication and Supply Shortages Are Harming Patients

ECRI and the Institute for Safe Medication Practices have released findings from a survey of 200 pharmacists and pharmacy technicians, procurement specialists, physicians, and nurses on shortages of medications and medical supplies and equipment and their effect on patient care; 86% of the respondents worked in hospitals. Overall, 60% of the respondents reported shortages of more than 20 medications, disposable supplies, and medical devices in the previous six months. They also reported that these shortages have affected the quality of care they provide related to surgery and anesthesia (74%), emergency care (64%), pain management (52%), cardiology (45%), hematology and oncology (44%), infectious diseases (39%), and obstetrics and gynecology (37%). Fifty percent of respondents indicated that the shortages have canceled or postponed treatment (eg, surgery) for patients, 33% indicated that they could not provide care with optimal medications or treatments (eg, administering more opioid anelgesia because of lidocaine shortages), and 25% indicated that at least one error has occurred as a result of a shortage.

Guidelines in Practice: Prevention of Venous Thromboembolism. 实践指南:预防静脉血栓栓塞。

Venous thromboembolism (VTE), a condition involving deep vein thrombosis and embolism, can cause death when left untreated. Hospitalized patients and those who have recently undergone surgery or have a cancer diagnosis are at increased risk for VTE development. The updated AORN "Guideline for prevention of venous thromboembolism" provides perioperative nurses with a variety of evidence-based recommendations associated with the topic. This article provides an overview of the guideline and discusses recommendations for a VTE protocol, VTE and bleeding risk assessments, pharmacologic and mechanical VTE prophylaxis, postoperative ambulation, and patient and family education. It also includes a scenario that illustrates the importance of the VTE assessment and the use of mechanical prophylaxis for high-risk patients undergoing operative or other invasive procedures. Perioperative nurses should review the guideline in its entirety and implement recommendations in operative and procedural settings.

Proper use and disinfection of medical devices and instruments is essential to preventing infection and providing safe patient care. Because of an increase in the complexity of medical devices and patient interventions, it has become more difficult for perioperative staff members to ensure that equipment is properly used and disinfected according to the manufacturer's instructions for use and the Spaulding classification system. One type of medical device that may have multiple clinical uses is the ultrasound transducer. In the perioperative setting, ultrasound transducers can be used for surface, percutaneous, and endocavitary procedures. They also may be used at multiple times before, during, and after a procedure, which may make it challenging for perioperative staff members to know the correct disinfection practice. They should use the device's instructions for use and the Spaulding classification system to guide their clinical practice.

Accrediting organizations, third-party payers, and patients review the surgical site infection (SSI) rates of health care organizations. Infection preventionists collaborate with perioperative personnel to decrease SSI rates; they also monitor and report SSI information to national organizations. The standard infection ratio is a comparison of the observed number of SSIs to the predicted number of SSIs based on national benchmark data. Leaders of a midwestern teaching hospital convened an interdisciplinary team (eg, surgeons, perioperative leaders, infection preventionists) to address a standard infection ratio after hysterectomies that was greater than 1.0. The team reviewed national guidelines and published articles on decreasing SSIs (including recommendations for vaginal preparation) before developing and implementing a hysterectomy-specific bundle for SSI prevention. The rate of SSIs decreased 68% after the implementation of the bundle. Perioperative personnel at this facility continue to use the bundle and infection preventionists monitor and report compliance with the bundle's elements.

During minimally invasive surgery (MIS), surgeons create small and percutaneous incisions to access internal structures without open surgical incisions. Some MIS equipment is complex and challenging for perioperative nurses to manage. Patients also can experience life-threatening complications during MIS procedures. The updated AORN "Guideline for minimally invasive surgery" provides recommendations that perioperative nurses can use when caring for patients undergoing MIS procedures. This article provides an overview of the guideline and discusses several recommendations, including creating a safe environment in which to perform MIS procedures; using gas distension media, irrigation and fluid distension media, and computer-assisted navigation and robotics; and performing intraoperative magnetic resonance imaging in a hybrid OR. It also includes a scenario describing care of a patient undergoing a hysteroscopy. Perioperative nurses who care for patients undergoing MIS procedures should review the guideline in its entirety and apply the recommendations as applicable in their practice.
