Substance use disorder (SUD) is a significant problem for millions of Americans. The incidence of SUDs among nurses and nursing students is thought to be like that found in the general population. Some studies suggest increases in substance use among college students given stressors related to the COVID-19 pandemic. Nurses are in unique positions to care for one another as we care for our patients. Identification, referral, and treatment of SUDs can prevent both nurses and nursing students from terminating their careers or nursing programs of study. The purposes of this article were to increase nurses' knowledge of SUDs among nurses and nursing students, provide an organizing framework for identification of substance problems among nurses/nursing students, and discuss ways to create a culture of support and recovery for nurses/nursing students with SUDs.

A Comparison of Medication-Assisted Treatment Options for Opioid Addiction: A Review of the Literature. 阿片类药物成瘾药物辅助治疗方案的比较:文献综述。

Abstract: In individuals in the United States with opioid addiction, what is the effect of a medication-assisted treatment (MAT) in reducing the relapse and harm reduction when comparing the use of buprenorphine, methadone, and naltrexone? In 2017, it was estimated that 1.7 million individuals suffer from overuse of prescription opiates, 652,000 individuals suffer from heroin use disorder, and greater than 130 individuals die from opiate overdose daily (National Institutes of Health, 2019). Using a systematic literature review, the following results were found. Buprenorphine is currently the second most effective MAT in harm reduction and relapse prevention, can be initiated and maintained through primary care, has a low risk for overdose, but needs to be started only when moderate withdrawals have begun. Methadone is currently the gold standard in MAT and can be started in any stage of withdrawal; however, titrating to effective dose is a lengthy process, and it must be administered at a specialty clinic. Naltrexone in oral form has not been shown to be effective because of lack of adherence; however, the extended-release intramuscular injection form has been shown to reduce relapse and increase the quality of life before initiation individuals must be opioid free for 7-14 days. Choosing the proper MAT is highly individualized. It is recommended that more research be conducted in comparing all MAT options, looking at the quality of life on each MAT, researching motivations to stay on MAT and remain opioid free, and looking at the impact of external reward on adherence to the MAT program.

Expectations of Patients and Their Families After the Treatment of Substance Use Disorder. 药物使用障碍治疗后患者及其家属的期望。

Background: Substance use disorders are economically and socially devastating to families and societies. Expectations of the patients and their families during the posttreatment phase of substance use disorder need to be emphasized to maintain a patient's sobriety and prevent relapse.

Aim: The aim of this study was to examine the prediction power of personal and sociodemographic factors of patients and their families to treatment outcomes.

Methods: A descriptive, correlational, cross-sectional design was used. Data were collected from 80 patients treated for substance use disorders from a major psychiatric hospital and their family members regarding social, health, and psychological expectations. A three-step multiple hierarchical regression analysis was used to predict the power of personal and sociodemographic characteristics of patients' and their families' expectations to treatment outcomes.

Results: Heroin use, codeine use, family education, and family support were significant predictors of rehabilitation expectation ( p < .05). Level of education and heroin use were predictors for lower levels of rehabilitation expectation, whereas family support and codeine use were indicators for higher scores of rehabilitation expectation.

Conclusion: Emphasis should be given to expectations of the patient and their family through appropriate psychoeducation and enhanced understanding and partnership.

Interplay of Depression, Smoking Intention, and Smoking Behavior in Chinese Dai Adolescents. 中国傣族青少年抑郁、吸烟意向和吸烟行为的交互作用。

Abstract: Tobacco smoking and depression are important contributors to the burden of disease in China, and their onset typically occurs in adolescence. However, there is no consensus on the nature and underlying mechanisms of their interplay, and related studies on Chinese adolescents and ethnic minorities are limited. This study tested the mediation role of depression in the link between smoking intention and behavior in relation to sex. A secondary analysis was conducted on data from a survey of 1,322 Chinese Dai middle school students aged 15-19 years (M = 17.02 years; 773 female students and 542 male students) in Xishuangbanna, China. We found that the mediation role of depression between smoking intention and smoking behavior was nonsignificant, although smoking intention and depression both had significant associations with smoking behavior. Therefore, depression might be better theorized as an underlying predictor of smoking intention or that other volitional factors may link smoking intention and smoking behavior more closely. Nevertheless, depression was a significant independent variable for smoking behavior even when smoking intention was adjusted. Women perceived more depression than men with similar smoking intention levels, yet the relationship between smoking intention and smoking behavior was stronger in men. Although it seems that men were abler to translate their smoking intention into actual smoking, the high level of depression among young women who reported higher levels of smoking intention is noteworthy. Tobacco control for Chinese adolescents could incorporate sex-specific psychological therapies for negative emotions and for the internalization of problems by children.

Background: Healthcare professionals' knowledge of safe use of opioids for chronic pain management is critical in preventing opioid abuse and overdose. Undergraduate curricula of health professional schools, including undergraduate nursing programs, need to improve and adopt a comprehensive educational plan regarding this issue.

Method: This study implemented and assessed an educational module on undergraduate nursing students' knowledge and attitudes regarding prescription opioids.

Results: The educational module improved nursing students' knowledge and attitudes. Students reflected positively on the module.

Conclusion: Undergraduate nursing students benefited from the educational module on prescription opioids and preferred to include a similar module in their undergraduate curricula.

Undergraduate curricula of health professional schools, including undergraduate nursing programs, need to improve and adopt a comprehensive educational plan regarding this issue.</p><p><strong>Method: </strong>This study implemented and assessed an educational module on undergraduate nursing students' knowledge and attitudes regarding prescription opioids.</p><p><strong>Results: </strong>The educational module improved nursing students' knowledge and attitudes. Risk and Protective Factors for the Use of Illicit Drugs: Perspective of the Users. 使用非法药物的风险和保护因素:使用者的观点。

Abstract: This article aimed to describe the perspective of people who make harmful use of illicit substances on the contributions of risk and protective factors to the development of problems related to drug use. One hundred eleven participants were recruited from a health service for drug users in a city in the interior of the state of São Paulo through posters with information, distribution of brochures, and snowball sampling technique. The sample consists of 51.9% men and 48.1% women, with 81.3% single and 10.3% married. Among them, 54.2% of participants completed high school, 64.1% were Catholic, and 34% rarely practiced their religion. The results on risk factors were classified into three domains, namely, "personal characteristics and behaviors," "family circumstances," and "other social pressures," and those on protective factors were also classified into three domains, namely, "personal characteristics and behaviors," "family circumstances," and "circumstances in the community." The data make it possible to guide the incorporation of multiple strategies to protect the consumption of illicit drugs in human biopsychosocial development, especially among children and adolescents.

Transcranial Direct Current Stimulation in Nicotine Use: Nursing Implications for Patient Outcomes. 经颅直流电刺激尼古丁的使用:对患者预后的护理意义。

Abstract: Tobacco use is a leading cause of cancer, cardiovascular and respiratory disease, and preventable death in the United States. The brains of individuals with nicotine dependence are characterized by damaged mesolimbic pathways in the medial portion of the limbic and frontal lobes, creating positive reinforcing mechanisms. Transcranial direct current stimulation (tDCS) targets this neuroadaptation to improve smokers' nicotine-related outcomes, such as craving and smoking behavior, by depolarizing or hyperpolarizing the neurons of the brain. Recent literature reported promising outcomes in smokers after tDCS treatment interventions. tDCS has great potential for clinical nursing research for tobacco control given its multiple methodological advantages and few disadvantages. Nurse researchers can consider individualized and home-based tDCS interventions for community-based tobacco control research and may need to consider objective outcome measures (e.g., cotinine in urine) and addiction-related cognitive variables (e.g., self-regulation). Users of electronic nicotine delivery systems also need to be considered as participants in tDCS interventions. Additional considerations for nursing research are discussed.
