Vas Deferens Abscess Rupture: A Case Report. 输精管脓肿破裂1例。

A vas deferens abscess is a very rare complication of acute vasitis and lower urinary tract infection. A case of vas deferens rupture due to an abscess with severe pelvic inflammation requiring surgical drainage is reported. Teaching Point: Vas deferens abscess rupture is an example of a very rare complication of severe inflammation of the vas deferens.

We studied the US imaging findings of lipomatosis of nerve and macrodystrophia lipoma in three patients. Three patients were seen at three affiliated institutions with an enlarged digit. They all underwent US, subsequently followed by an MRI study. The nerves showed marked enlargement and extension over a length of 9-16 cm. Digital branches were always involved. The appearance on cross section was an enlarged hyperechoic endoneurium with inlying thickened and hypoechoic fascicles. On transverse images this resulted in a 'chocolate cookie' aspect and in the long axis a 'spaghetti-like' image. The US appearance of lipomatosis of nerve, not unlike MRI, is rather typical. With US, care should be taken in areas that are more difficult to assess as the sole of the foot, or where the 'chocolate cookie' appearance is not so obvious, such as the digits.

Long-Term Outcome of Single-Session, Ultrasound-Guided, Radiofrequency Ablation for Symptomatic Small, Lower Limb, Venous Malformations. 单次超声引导射频消融治疗症状性下肢静脉畸形的远期疗效。

Objectives: To analyze the long-term clinical outcome of percutaneous, ultrasound-guided radiofrequency ablation (RFA) of peripheral low-flow vascular malformations (VM).

Materials & methods: Adolescent patients presenting with symptomatic VMs and referred for percutaneous management using RFA between January 2010 and January 2015 were identified by a search in the institutional interventional radiology database. Clinical and radiological follow-up up to April 2021 was based on retrospective analysis of patients' electronic medical records and imaging files. This retrospective study was approved by the institutional ethics committee.

Results: Four female patients (median age 16 years) presented with lower extremity pain, swelling and functional disability related to VM as confirmed by magnetic resonance imaging. Two patients underwent percutaneous sclerotherapy previously. Clinical follow-up (mean of 8,5 years) showed complete and sustained resolution of the symptoms in all patients; in one patient a persistent decrease in muscular strength of the treated limb was noted.

Conclusion: Percutaneous, ultrasound-guided RFA is relatively safe and efficient with durable symptom relief in the management of small, lower limb VMs in adolescent patients.

Comparison of Initial CT Findings and CO-RADS Stage in COVID-19 Patients with PCR, Inflammation and Coagulation Parameters in Diagnostic and Prognostic Perspectives. PCR、炎症及凝血指标对COVID-19患者CT首发表现及CO-RADS分期诊断及预后的影响

Objectives: This study aims to determine whether COVID-19 patients with different initial reverse transcriptase-polymerase chain reaction (RT-PCR), computed tomography (CT) and laboratory findings have different clinical outcomes.

Materials and methods: In this multi-center retrospective cohort study, 895 hospitalized patients with the diagnosis of COVID-19 were included. According to the RT-PCR positivity and presence of CT findings, the patients were divided into four groups. These groups were compared in terms of mortality and need for intensive care unit (ICU). According to the COVID-19 Reporting and Data System (CO-RADS), all patients' CT images were staged. Multivariate binary logistic regression analysis was used to examine the relationship between CO-RADS and predictive inflammation and coagulation parameters.

Results: RT-PCR test positivity was 51.5%, the CT finding was 70.7%, and 49.7% of the patients were in the CO-RADS 5 stage. The need for ICU and mortality rates was higher in the group with only CT findings compared to the group with only RT-PCR positivity, (14.9% vs. 4.0%, p < 0.001; 9.3% vs. 3.3%, p > 0.05; respectively). Mortality was 3.27 times higher in patients with CO-RADS 4 compared to those with CO-RADS 1-2. Being in the CO-RADS 4 stage and LDH were discovered to be the most efficient parameters in determining mortality risk.

Conclusion: Performing only the RT-PCR test in the initial evaluation of patients in SARS-CoV-2 infection may lead to overlooking groups that are more at risk for severe disease. The use of a chest CT to perform CO-RADS staging would be beneficial in terms of providing both diagnostic and prognostic information.

Our case report describes a 54-year-old man who was admitted to our hospital complaining of visual impairment with gastrointestinal and neurological symptoms. Initial computed tomography examination showed bilateral symmetric putaminal and cerebral white matter hypodensities. Evaluation of the following magnetic resonance imaging, restricted diffusion in these corresponding areas were found to be compatible with cytotoxic edema. Teaching Point: Diffusion-weighted imaging (DWI) plays a crucial role in the diagnosis of acute methanol intoxication.

Teaching Point: Iron deposition in the pituitary gland in patients with primary and secondary hemochromatosis is a lesser-known cause of pituitary dysfunction and results in T2- and T2*-signal loss on MRI, other brain structures, in which iron deposition can be seen, are the choroid plexus and sporadically the circumventricular organs (e.g., the pineal gland).

Elective Transjugular Intrahepatic Portosystemic Shunt Using Viatorr Stent-Grafts: A Single-Center Experience from China. 选择性经颈静脉肝内门静脉系统分流使用Viatorr支架移植物:来自中国的单中心经验。

Background and aims: Transjugular intrahepatic portosystemic shunt (TIPS) is a well-established approach for the management of variceal bleeding, refractory ascites, hepatic hydrothorax, and preoperative treatment of portal hypertension prior to major abdominal surgery in patients with compensated cirrhosis, and so on. This study aimed to investigate the safety and long-term efficacy of TIPS implantation using Viatorr TIPS stent-grafts.

Material and methods: A cohort of 59 patients undergoing TIPS placement using Viatorr TIPS stent-grafts were included, and the periprocedural events, and long-term mortality, shunt dysfunction, variceal rebleeding and incidence of hepatic encephalopathy (HE) were analyzed.

Results: The technical success rate was 100%. The median portosystemic pressure gradient was reduced from 21 mmHg (interquatile range: 19-25) to 13 mmHg (interquatile range: 10-16) before and after TIPS, leading to a hemodynamic success rate of 72.9%. The cumulative rate of overall mortality was 34.2% at five years, and direct bilirubin (hazard ratio [HR] = 1.336, 95% confidence interval [CI]: 1.050-1.700, P = 0.018) and post-TIPS right atrial pressure (HR = 1.238, 95% CI: 1.015-1.510, P = 0.035) were independent predictors for mortality. The cumulative rates of shunt dysfunction and variceal rebleeding were 11.0% and 28.3% at five years, respectively, and portal venous pressure gradient (HR = 2.572, 95% CI: 1.094-6.047, P = 0.030) was the only independent predictor for shunt dysfunction. The cumulative four-year HE-free rate was 48.6%. No severe adverse event was noted during TIPS procedures.

Conclusion: Elective TIPS implantation using Viatorr TIPS stent-grafts is generally safe, and the long-term efficacy is favorable for the treatment of cirrhotic patients with recurrent variceal bleeding or refractory ascites.
