Awareness during general anaesthesia is a potentially traumatising risk of any general anaesthetic, which can have lasting effects on the patients who experience it. This article assesses the issues and causes of anaesthetic awareness, together with the current status of research being conducted into its prevention, and the effect of market forces and litigation.

The blue line has appeared in the relevant window and a potential future perioperative practitioner is on the way. There are a number of issues and requirements which parents to be who are working in the perioperative environment should consider.

Fasting patients prior to their surgical procedures is accepted practice. Although the latest research shows that it is safe for most patients to drink clear fluids 2-3 hours before a general anaesthetic many key staff are not aware of these findings. This article discusses preoperative fasting and describes an audit which culminated in a change in practice in a large district general hospital.

Malignant hyperthermia (MH) is a rare genetic condition which may manifest for the first time during anaesthesia associated with a routine surgical procedure. Characterised initially by muscle rigidity, increased body temperature and metabolic acidosis, the syndrome may prove fatal unless prompt, effective treatment is administered. The sudden development of MH constitutes a medical emergency; hence it is essential that theatre practitioners are knowledgeable about the presenting symptoms and management of the condition.

Airway obstruction is the most common cause of death in the prehospital environment. It is estimated that 85% of these patients had otherwise survivable injuries. The aim of the project was to increase firefighters' skills in airway management techniques, to include the laryngeal mask airway (LMA), by providing theatre-based hospital placements under the supervision of a consultant anaesthetist. The training in the use of the LMA by firefighters is currently a pilot project to assess the potential for its use operationally, based on ease of training to a competent level and retention of skills after one year.

This thought-provoking article describes how a perioperative nurse of twenty years' experience reflected on a critical incident that happened in the hospital where she works, and what she gained from the experience. She also provides us with a survey of the literature pertaining to the processes underlying critical analysis.

In the previous edition of the British Journal of Perioperative Nursing, Susan Pirie, AfPP Professional Officer outlined the purpose of the disciplinary process and best practice for employers managing such a process. As part of the continued human resources series John Beesley, Lead Professional Officer outlines why the grievance procedure for employers and perioperative staff is every bit as important with regard to best practice and legislative changes.
