The premature development of coronary atherosclerosis is the most well-known heart disease in diabetes, however, a special subset of heart failure in diabetes has been proposed, the diabetic cardiomyopathy, which is myocardial dysfunction not caused by ischemia, valvular disease or hypertension [3]. The exact cause remains unclear. It is generally accepted that the most important mechanisms are hyperglycemia, increased free fatty acids, activation of the rennin angiotensin system, microangiopathy, increased oxidative stress, and cardiac autonomic neuropathy [4].

ECMO Plus Intra-arrest PCI - Excellent Option in a ‘no other Option’ Situation ECMO加停搏内PCI -在“没有其他选择”的情况下的最佳选择

A major cause of sudden unexpected death is cardiac arrest. Despite a properly performed cardiopulmonary resuscitation, only a minority of patients who suffer cardiac arrest are able to be meaningfully resuscitated. ECPR (an alternative CPR method in which Extracorporeal Membrane Oxygenation (ECMO) is used) along with intra-arrest PCI can be effective lifesaving dual procedures for patients in whom conventional Cardio Pulmonary Resuscitation (CPR) cannot achieve the return of spontaneous circulation. Also, important would be the fact that near optimal temporal perfusion of the other vital organs is ensured during the course of this high-risk situation needing a quick salvage.

subjects, Abstract Background: The mechanisms of cardiovascular adverse events in Obstructive Sleep Apnea Syndrome (OSAS) are not fully clarified, but vascular dysfunction plays a key role. However, the protocols for assessing vascular function are heterogenous and there are new vascular function parameters that could emerge into practice. Methods: The vascular function was assessed through Intima Media-Thickness (IMT), “e-tracking” and “wave intensity” technologies in 60 subjects (55±12 years, 9 women): 20 patients with moderate to severe OSAS (mean apnea-hypopnea index 43±23) without diabetes; 20 patients with type 2 Diabetes Mellitus (DM), and 20 control, normal subjects (C), age, gender and cardiovascular risk factors matched. Results: IMT in OSAS and DM groups were similar, but statistically higher compared to C group. Arterial stiffness parameters, like Peterson’s pressure-strain elastic modulus (Ep), beta stiffness index (β), Arterial Compliance (AC), First Systolic Peak (FP), and second Systolic Peak (SP) were similar between OSAS and DM group, but significantly affected compared to C group. Endothelial dysfunction quantified through Flow Mediated Dilation (FMD) was similar in the OSAS and DM groups and statistically significant lower compared to control group. Conclusion: Patients with moderate to severe OSAS have increased vascular remodeling, vascular stiffness and endothelial dysfunction, with similar vascular profile with patients with DM, suggesting that OSAS should be considered a disease associated with a high cardiovascular risk.

Interrupted IVC is a rare anomaly with an incidence of 1 in 5000 cases [5]. It has a strong association with polysplenia (86%). As a rule, it is generally associated with situs abnormalities and complex congenital heart defects. Although PDA device closure in an interrupted IVC is technically difficult, procedure done through internal jugular, femoral vein and retrograde trans arterial approach had been reported [1-3]. We describe a novel case in which we did PDA device closure in a five-year-old girl with situs inversus Dextrocardia interrupted IVC with Azygous continuation through trans jugular approach.
