Evidence for microvesicle particles in UVB-mediated IL-8 generation in keratinocytes. 微泡颗粒在uvb介导的角质形成细胞中产生IL-8的证据。

Recent studies have implicated bioactive microvesicle particles (MVP) in the keratinocyte response to many environmental stressors, in partricular ultraviolet B radiation (UVB). The generation of MVP in response to UVB involves the Platelet-activating factor receptor (PAFR) and the enzyme acid sphingomyelinase (aSMase). As UVB generates some cytokines such as interleukin-8 (IL-8) in a PAFR-dependent manner, one question is if the production and release of IL-8 and MVP could be linked. Using the human keratinocyte-derived cell line HaCaT, the present in vitro studies indicate that pretreatment of HaCaT keratinocytes with PAFR agonist ester can synergize with low fluences of UVB to generate high levels of MVP as well as IL-8 protein. Treatment of cells with an aSMase pharmacologic inhibitor blocked both processes. These studies indicate the possibility that MVP could be involved in pathologic processes involving UVB-generated production of pro-inflammatory cytokines such as IL-8.

Serum Amyloid A as an Inflammation Marker in Lichen Planus 血清淀粉样蛋白A作为扁平地衣炎症标志物

Background: Lichen planus (LP) is a chronic T cell-mediated inflammatory disorder that can affect skin, mucosa, hair, and nails. Serum amyloid A (SAA) is a conserved acute phase protein in response to trauma, infection, malignancy, and severe stress. SAA may have a homeostatic role rather than a pro-inflammatory or anti-inflammatory one. Serum levels of SAA were demonstrated to be raised in several inflammatory systemic and skin diseases as rheumatoid arthritis, systemic lupus erythematosus, and psoriasis. Aim: This study aimed to evaluate serum levels of SAA, and IL 6 in a sample of Egyptian LP patients and to estimate its correlation with disease severity. Patients and methods: We included 21 adult patients with LP and 21 healthy adults as control. The total score of LP severity was measured for all patients through measurement of the affected body surface area in cutaneous LP patients while using a semi-quantitative clinical scoring system for oral LP together with the visual analog scale for pain assessment in OLP. Serum levels of SAA were estimated in all participants using enzyme-linked immunosorbent assay. Results: The expression levels of SAA and IL 6 in peripheral blood of patients in the two groups were detected. Pearson analysis was used in the correlation between SAA and IL 6 and Receiver operating characteristic (ROC) curve was employd to analyze the predictive value of SAA and IL 6 for LP severity. Logistic regression analysis was used to analyze the risk factors of LP patients. The expression levels of SAA and IL 6 of patients in sever form were significantly higher than those in mild form (P<0.05). Pearson analysis showed that SAA was positively correlated with IL 6 expression (P<0.05). ROC curve analysis showed that AUC predicted by SAA and IL 6 for LP severity was 0.789 and 0.762 (P<0.05). Logistic regression analysis showed that SAA and IL 6 were prediction indexes of LP severity. Conclusion: The levels of SAA and IL 6 were significantly increased during LP and effectively predicted the severity of LP and is a risk factor affecting LP patients. Further studies are needed to establish this association then it might be used for the evaluation of therapeutic outcomes. NOTCH1 in Cutaneous Squamous Cell Carcinoma Arising in Immunosuppressed Patients: A Systematic Review and Quantitative Analysis 免疫抑制患者发生的皮肤鳞状细胞癌中的NOTCH1:系统回顾和定量分析

Immunosuppression is a strong risk factor for cutaneous squamous cell carcinoma (cSCC). Immunosuppression is also associated with unique mutagenic stressors that likely contribute to cSCC pathogenesis. However, it is unknown whether these stressors contribute to a distinct mutation profile that may drive disease progression. This review was conducted to assess the mutational landscape of cSCC arising in immunosuppressed hosts. Specifically, we sought to determine gene mutation frequencies in immunosuppressed cSCC. An electronic search was performed in PubMed, Embase, Scopus, and Cochrane databases. Studies performing DNA sequencing or genotyping of cSCC were identified. Studies were excluded if the immune status of each tumor was not available. Eighteen studies met inclusion criteria. Due to study heterogeneity a meta-analysis was unable to be performed. However, statistical analysis was performed on the most frequently reported genes. NOTCH1 was the most frequently mutated gene in immunosuppressed cSCC, and was significantly higher than immunocompetent cSCC after multiple comparison adjustment (77.7 versus 58.1%, OR 2.50, 95% CI 1.40-4.46, p=0.002). No other statistically significant differences were observed. Our results suggest that NOTCH1 mutations are more common in cSCC arising in immunosuppressed hosts. Several prior observations reviewed here further support a role for NOTCH1 in immunosuppressed cSCC, however larger studies are needed to confirm our findings. mattress sutures were associated with reduced infection rates than other suture techniques. Our goal in performing the study was to expand on this prior study, describe the SSIs that we have encountered and treated over the previous 9 years to identify relationships between cultured bacteria and host and clinical factors, as well as understand the bacterial resistance patterns in our geographic location to better help us treat SSIs going forward.

Pyodermatitis-Pyostomatitis Vegetans: A Cutaneous Clue for Crohn’s Disease 素食者的皮炎-化脓性口炎:克罗恩病的皮肤线索

Pyodermatitis-pyostomatitis vegetans is a neutrophilic dermatosis that is classically associated with inflammatory bowel disease. It is rare with less than 40 cases reported in the literature within the last 10 years. We discuss the case of a 30-year-old male who presented with pyodermatitis-pyostomatitis vegetans and previously undiagnosed Crohn’s disease. We aim to emphasize the importance of prompt recognition of pyodermatitis-pyostomatitis vegetans order to commence a thorough investigation of underlying inflammatory bowel disease, and initiate appropriate therapy. Because no standardized treatment regimens have been established, we also describe the variability in treatment responses seen with pyodermatitis-pyostomatitis vegetans with a review of the literature.
