The methods applicable to the diagnosis of polycystic kidney disease are reviewed. By assessing the clinical data of 314 patients suffering from adult polycystic kidney disease who had been treated in the 30 years between 1950 and 1979, the importance of conservative therapy and nephrological care is emphasized. The significance of hemodialysis in therapy and the special circumstances of transplantation are discussed. The indication of surgical treatment can be extended in complication-free cases.

In the open chest the heart loses more heat and its epicardiac temperature will fundamentally depend upon the temperature of the body and its environment, upon the flow in the coronaries and upon the actual state of myocardial metabolism. The experiments were carried out with the AGA Thermovision Type 750 equipment. The epicardial temperature changes induced by reactive hyperaemia due to the temporary coronary ligation in 24 dogs, and by permanent coronary ligation in 70 dogs, as well as by the gradually reduced coronary flow of another 8 dogs were recorded by means of thermography. The results indicated a relationship between the changes in epicardial flow and epicardial temperature, consequently thermography is considered a method suitable for the study of myocardial flow.

In 37 of the 258 patients with pelvic fractures rather severe urological complications were diagnosed. It seems that the majority of polytraumatized patients can be considered as potential urologic cases. The characteristic urological complications associated with the various types of fractures are described. The importance of urgent urological examination is stressed in order to detect urological injuries requiring acute care and to determine the need for operation or for conservative treatment. Co-operation between the traumatologist, the urologist and the radiologist will ensure the definitive care of the polytraumatized patient and the best chances for recovery.

The simultaneous occurrence of Graves' disease with ophthalmopathy lymphocytic and subacute de Quervains' thyroiditis has been observed in two male patients suffering from follicular-type thyroid carcinoma. In one of them Sjögren syndrome and a mild form of diabetes were also diagnosed. It is assumed, on the basis of indirect evidence, that the humoral and cellular lesions determined by genetic factors and influenced by prolonged thyrostatic and steroid treatment are jointly responsible for tumour genesis.

In the preparation of colorectal operations, beside the mechanical cleaning of the bowels, orally administered metronidazole, acting on anaerobic bacteria, was found to be highly effective. Compared to the controls, the drug considerably reduced the number and severity of postoperative infections, and, consequently, also the mortality. With metronidazole prophylaxis no wound suppuration indicating anaerobic infection was noted. The combined application of metronidazole with some antibiotic acting on the aerobic bacteria of the intestinal flora resulted in a significant reduction of postoperative infections.

Changes in the circulation parameters measured during modified neuroleptanalgesia were assessed for 75 cases. No significant deviation from the initial values were observed in the systolic blood pressure and pulse rate in the introductory phase of anesthesia introduced with Etomidate after Droperidol + diazepam premedication and maintained with Fentanyl, relaxation and the inhalation of nitrous oxide. Because of this marked stability of the circulation, the method is considered particularly advantageous for the analgesia of high-risk-patients.
