Patients tell the story: interrelationships among patient satisfaction, communications with providers, and emergency department care.
Carol Lynn Esposito, Julia M Macchiaroli, Jacqueline Witter, Elsa DeGoias, Elsa-Sophia Morote

This study examines the relationship between four variables and patient satisfaction in an emergency department setting in a small community hospital in Suffolk County New York. Patients were assessed utilizing four variables: communications with doctors, communications with nurses, communications with ancillary staff, and environment of ED care. The study adds to the literature on which factors have the greatest influence on patient satisfaction in an emergency department setting. The purpose of the present study was to explore how ED patient satisfaction was influenced by their communications with physicians, nurses, and ancillary staff. Pearson correlation coefficients resulted in a statistically significant correlation between all variables and patient satisfaction. Path analysis showed the interrelationship between the four variables and patient satisfaction. Regression analysis predicted the extent to which each variable influenced patient satisfaction. The strongest predictor of patient satisfaction was communications with ancillary staff.

Perspectives on academic misconduct: implications for education and practice.
Marilyn B Klainberg, Andrea McCrink, Patricia Eckardt, Rose Schecter, Anne Bongiorno, Laila Sedhom

From Harvard to high school, concern related to academic misconduct, specifically cheating and its impact on societal issues, has become a great concern for educational communities. While a significant number of studies on ethical behaviors in practice in other professions such as business have been published, little research exists on registered nurses in practice. Even fewer studies have, for registered nurses, addressed if there is an association between perceived academic misconduct as students and perceived unethical behaviors in the workplace. The purpose of this study was to determine the relationship between perceptions of registered professional nurses' (RNs) current workplace behaviors and the RNs' retrospective perceptions of their academic misconduct as students. A convenience sample of 1 66 RNs enrolled in master's degree programs at four university schools of nursing completed questionnaires regarding their beliefs and behaviors. The outcome of this study was significant. Results revealed a strong relationship between unethical behaviors of the RN in practice and their prior academic misconduct when they were students.

Psychosocial needs of cancer patients living in the Adirondacks: a needs assessment.
Alison Riley-Clark, Barbara B Pieper

As the national conversation on solutions to healthcare factors such as affordability and access is likely to intensify, health inequities continue to persist in rural areas. National studies suggest a growing, aging, rural population impacting the revenues of local healthcare facilities, as well as contributing to complexity of care. This paper examines factors influencing the health of rural citizens in upstate New York and offers the results of an initial needs assessment looking at the psychosocial healthcare needs of cancer patients living in the Adirondack Park region. Patients were surveyed regarding their perceptions of psychosocial needs and experiences as a cancer patient. Psychosocial factors while acknowledged as important influences on recovery and healing remain an underdeveloped intervention toward improving the quality of cancer care. Recommendations are made based on the results to enhance the quality of life for this vulnerable population.

The answer is questions: accelerated-nursing students report practice questions are fundamental to first-time NCLEX-RN success.
Barbara B Blozen

There are a number of anecdotal reports on demographic characteristics and academic success of accelerated-nursing students; yet few empirical studies have examined accelerated-nursing students NCLEX-RN success. Applying Knowles' adult learning theory as a guiding framework, the purpose of this qualitative study was to explore, from the accelerated-nursing students' perspective, the factors reported as contributing to their success on the NCLEX-RN. The research questions aimed to elicit participants' descriptions of their experiences and factors contributing to their success via individual interviews. The most significant finding the participants identified as the factor that contributed to their success was the practicing of NCLEX-RN questions. The findings of this study have several implications for educational policy and practice for universities and schools of nursing as the information gained from this study applies to recruitment and retention as well as curriculum and educational strategies in an accelerated-nursing program.

Education to increase women's knowledge of female myocardial infarction symptoms.
Melanie Kalman, Pamela S Stewart, Margaret Wells, Aaron Blumkin, Margaret Pribulick, Roberta Rolland

Objective: To test the use of acronyms to increase women's knowledge of female prodromal and myocardial infarction (MI) symptoms using acronyms, and the appropriate response to these symptoms.

Design: A quasi-experimental design.

Method: An educational program, emphasizing two acronyms, was presented and knowledge of female heart attack, prodromal symptoms, and appropriate response was tested before and after the presentation.

Participants: The sample consisted of 51 women.

Results: Knowledge scores increased from 81% pre-test to 91% post-test. This difference was statistically significant on a paired sample t-test, with each subject serving as her own control. Scores measuring knowledge of content specific to the acronyms also improved and were statistically significant.

Conclusion: Lack of knowledge of female heart attack symptoms may contribute to the delay of women seeking care and appropriate treatment, thus increasing morbidity and mortality. The findings from this study demonstrated that an educational program that focused on the use of acronyms was effective in increasing women's knowledge of female prodromal and MI symptoms.

Provision of culturally competent health care: an interim status review and report.
Carol Lynn Esposito

This review reports on the current nursing literature as it relates to the progress the nursing community has made in the field of transcultural nursing. This review also reports on the current status of the nursing professions' initiative to integrate multi-cultural, diversity training into its educational programs and on the degree to which nurses' report on their preparedness to provide exemplary, culturally competent care following their educational training. The literature reveals we are still only beginning to understand how nursing's educators and employers can provide critical learning environments and workplaces for students, faculty and practitioners to grasp the concepts of transcultural nursing and to evaluate the effectiveness of our actions following training. This status report provides the nursing profession with opportunities to be creative in designing cultural immersion experiences, faculty workshops, educational programs, certification programs, and qualitative and quantitative evaluation tools to measure the effectiveness of our cultural competence programs and experiences. Further opportunities include the dissemination of important concepts and findings through research, conferences, scholarly journals, nursing publications, staff development programs, and continuing education programs.

Peer tutoring program for academic success of returning nursing students.
Jennifer Bryer

High attrition rates among students in associate degree nursing programs are a concern for faculty, administrators, and students. Programs offering academic and emotional support for students at risk for failing a clinical course may decrease attrition rates and improve academic performance. A peer tutoring program was developed for returning nursing students who were unsuccessful in a previous clinical course. Peer tutors met with returning students weekly to review course work, complete case studies and practice NCLEX questions. Trusting, supportive relationships developed among students and a significant increase in grades was noted at the end of the course for 79% of students. Implementation of peer tutoring was beneficial for returning students, tutors, and the nursing program and may be valuable in other courses where academic achievement is a concern.
