Anterior open bite is a major orthodontic problem that is very difficult to treat (Fig. 1). It may not be the most frequent problem but it is a major functional problem. Quite often it is very difficult to correct an anterior open bite problem and even more difficult to treat in an adult after growth. Most of the time anterior open bite is a myofunctional problem related to a bad habit like thumb or finger sucking and/or is related to a breathing and swallowing problem. How can we understand the functional open bite problem? What are the available treatment options? This article will try to answer some of these questions.

Clinical use of this new appliance design is meant only to be an adjunct to other treatment mechanics used in the treatment of Class II malocclusions. For example, the primary goals of the appliance are usually accomplished within six months, so I often follow the appliance with a transpalatal bar or a simple Nancebutton to maintain my expansion and molar rotation as I continue to treat the malocclusion. As I use the appliance in the upper arch, I am often using another fixed expansion appliance (e.g., the Williams appliance) in the lower arch and because the bite has been opened by the bonded occlusal rests of the upper, I can use utility arches to level, align, intrude, etc. the lower incisors. As we move through the 1990's and into the next century, orthodontic research will concentrate on treatment of malocclusions earlier than ever before and orthodontic technology will focus on fixed-functional appliances, which improve efficiency, reduce treatment time, and give the clinician primary control. This appliance achieves these goals in excellent fashion. This is a two-dimensional appliance that corrects in three dimensions.

A number of options have been presented to treat patients with congenitally absent (or malformed) MLI's and second bicuspids. Overall the treatment of choice was shown to be essentially dependent on diagnostic parameters, length of treatment time, facial esthetics and age of patient. Orthodontic space closure is the treatment of choice since it should result in a lasting natural healthy dentition. In prosthetic cases, implant-supported or otherwise, the skill of the practitioner and the quality of the prosthesis, plus the general health and the oral hygiene of the patient, would be important considerations to ensure longevity of the prostheses. TMJ dysfunction occurring during orthodontic treatment is a possibility. Procedures that may precipitate a dormant predisposition to these kind of difficulties should be carefully evaluated and/or compensated.
