A modified technique aimed at the conservation of maxillary anterior esthetics is proposed for the surgical treatment of periodontal disease when surgery is indicated in addition to nonsurgical debridement. This technique allows access to periodontally diseased root surfaces and suggests a flap design and a method for suturing which minimize postsurgical soft tissue recessions.

The purpose of this article is to reexamine the indications of surgical techniques and materials used in the regenerative treatment of periodontal disease. Two cases are presented to illustrate the potential for regenerative therapy in instances where limited success might have been anticipated.

Procedures to augment the severely atrophic maxilla using sinus lift techniques have been increasingly used for endosseous implant placement. A case report is presented in which a patient had a severely resorbed maxilla bilaterally. A situation developed during the course of treatment which created a unique opportunity to compare an autogenous iliac graft on one side with a hydroxylapatite/freeze-dried bone allograft on the other.

The aim of this study was to determine (1) the influence of root conditioners, i.e., citric acid (CA) and tetracycline (TC), on the vascular response of closed periodontal wounds, and (2) the effect of implantation of nonresorbable membranes on the vascularization process. Thirty-two alveolar fenestrations were created in the maxillae of four healthy dogs. The depth of the fenestrations reached the periodontal ligament, cementum, and dentin. Topical application of CA, TC, or sterile water (SW) was done on the exposed dentin. Nonresorbable membranes were then implanted in half of the created defects between the mucoperiosteal flap and the alveolar bone. The vascular response at the experimental sites was determined by histological evaluation and cleared specimens following observation periods of 3, 7, 14, and 21 days post-operatively. Results from this pilot study indicated that: (1) the coronal aspect of the periodontal ligament contributed the most to the initial vascularization of the defect; (2) there was no definitive advantage of the administration of CA or TC on the vascular response at 14 or 21 days; (3) no differences in vascular response were observed between treatment with or without the implantation of nonresorbable membranes; and (4) at 21 days, the vasculature in the defect area had not returned to its presurgical state.

Dental twinning derives from abnormal events during the embryological development of tooth germs. The majority of fused teeth are found in the deciduous dentition, particularly in the anterior region of dental arches. Fusion of posterior teeth is considered a rare event. We report here a true case of posterior fusion between a second and third mandibular molar occurring in a black African woman. The morphology and the position of this abnormal tooth were responsible for a related periodontal problem.

A wide spectrum of viruses can give rise to oral and periodontal manifestations. This article reviews viral infections of clinical importance to the periodontologist and confirms the important role of the periodontologist in the recognition and treatment of common viral infections of the mouth.

The purpose of this study was to evaluate the effectiveness of a hydrophilic dentin primer to eliminate dentinal hypersensitivity to thermal stimuli. Eight patients enrolled in the study, with a total of 42 teeth having cervical sensitivity caused by erosion, abrasion, or periodontal therapy. All teeth were sensitive to cold air and tactile exploration. All test sites were rinsed with water, air-dried, and rubbed with a moist cotton pledget. Several coats of a dentin primer, consisting of N-tolylglycine-glycidyl methacrylate and bisphenyl dimethacrylate, were then applied with a brush to the exposed dentin and light-cured. All patients reported a complete absence of pain when the sites were retested with thermal stimuli immediately after treatment. After a 9-month period, seven patients remained symptomless, and one patient indicated slight sensitivity to ice. It was concluded that a single application of a hydrophilic dentin primer is highly effective in reducing or eliminating dentinal hypersensitivity to thermal stimuli for at least 9 months.

The end-stage renal patient undergoing dialysis is susceptible to a number of infections. We report a case of localized suppurative osteomyelitis of the jaws, secondary to periodontitis, in a patient with chronic renal failure, with review of the physiologic and biochemical changes associated with renal failure. The clinical, radiographic, histologic, and microbiologic findings are presented with a discussion of the differential diagnosis and treatment provided.

Tuberous sclerosis is a rare congenital disorder characterized by cutaneous angiofibromas, mental retardation, seizure disorders, and a variety of other, less common systemic anomalies. The present report details the features and periodontal management of a patient with gingival fibromata secondary to tuberous sclerosis.
