After the spontaneous epithelialisation of the defects of soft parts so called unstable scars remain which, because of ulceration, are sources of infection, endangering both soft parts and bones. Substitution with soft tissues of adequate blood supply enables the complete sanation of the septic foci and by this the rapid healing of the patient with full function. Beside this a better cosmetic result can be achieved. In septic environment, if possible, local muscle flaps and free flaps are used, depending on the extent and localization of the defect. Good fixation, ability for full weight-bearing in most of the cases, closed surgical technique-these are the advantages of lately developed intramedular osteosynthesis for instabil pertrochanteric and subtrochanteric fractures, called gamma nailing. These advantages do make the general and local complications less and do help the fast rehabilitation. Authors give report on 28 patients, were operated on with gamma nailing within the last 1.5 year. Local complication, infection, loosening, breakage, pseudoarthrosis was not observed. The healing was complete in all evaluated cases. 2/3 of the patients had full weight-bearing following surgery. According to the authors the results are very good in the cases of instabil subtrochanteric and pertrochanteric fractures, therefore the method is highly recommended in similar cases.

Authors review the possibilities of the treatment of degenerative damage of the knee. Their new method of operation is described, used in cases of severe cartilage damage of the superficial articular surface of the patella and of the weight bearing surface of the knee. In the available literature one similar method was found, described by Japanese authors in 1993. Authors gain since 1992 from the non weight bearing articular surface of the femur cartilagineous-osreous cylinders for the supply of the defects of the weight bearing surface and the cartilagineous defects of the patella. By this a gliding surface, better than gained with the former method can be created. The method described after an early follow up of their first 14 operations.
