Patterns and trends in health care governance. 医疗保健治理的模式和趋势。

Recent studies show that many hospital chief executive officers spend more than 50% of their time preparing materials for governing board meetings. In view of the impact of governing boards on the function of the hospital and its executives, this issue of DRG Monitor summarizes recent trends in health care governance, including the role and responsibilities of hospital trustees, the size, composition, and terms of office of governing boards, structures and functions of committees, and other emerging issues.

What differences exist between provision of health care benefits, cost containment measures, and health care concerns between large and small companies? This issue of DRG Monitor examines these differences through the analysis of responses to a survey and suggests strategies that hospitals and other health care providers may employ to target both markets.

Effective ambulatory service capacity management. 有效的门诊服务能力管理。

An effective capacity management program can increase operational productivities while reducing patient congestion. A framework for ambulatory care capacity management is presented that includes both demand-smoothing and supply-matching strategies. A case study focuses on analyzing congestion in the patient care delivery process at a medical oncology outpatient clinic. Through the use of a balanced and coordinated set of capacity management strategies, a low-cost approach to improved performance may be designed.

This issue of DRG Monitor describes a productivity enhancement system that is decentralized to the work unit level in the hospital. The system depends on a budgeted excess bonus pool that triggers team-based bonuses when unit personnel positively influence indicators of work unit productivity and work unit indicators of the quality of health care services.

When challenged, hospital administrators may find themselves faced with the responsibility of defending both the existence and use of the profits of the hospital. A Disease Staging system is described that defines patient-level severity of illness and is an efficient DRG enhancement tool. Not only is it highly predictive of resource use, it is also valid, reliable, and easy to interpret. Examples of its use in analyzing hospital costs are provided, and a current bibliography of research involving Disease Staging is presented.
