Hand-assisted laparoscopic nephrectomy for a kidney transplant: surgical technique—tips and tricks—how we do it? 手助腹腔镜肾切除术用于肾移植:手术技术——技巧和窍门——我们是如何做到的?

: Kidney transplant is considered the best therapy for patients with chronic kidney disease in renal replacement therapy or dialysis stage, which has better survival and quality of life with lower morbidity and lower long-term expenses, resulting in overall cost saving for the health care system compared to other therapies. In a living donor kidney transplant patient, the safety must remain as maximal priority because it is performed on a healthy patient who accepts the risks of major surgery for the benefit of the recipient. The process of donation should be as least traumatic, painful, and uncomfortable as possible to the potential donor. Over the years several approaches have been described, currently there is no gold standard to perform a nephrectomy on potential donors. Hand-assisted laparoscopic nephrectomy encompasses the benefits of the minimally invasive approach reducing the operative time and learning curve in comparison with the totally laparoscopic procedure. This technique has shown excellent results, both for the donor and the recipient. This work presents the technical description of the hand-assisted laparoscopic nephrectomy focused on the anatomical aspects, tips and tricks. Furthermore, the equipment preference card, patient’s workup, postoperative management, and narrated technique video are added so that the reader can follow them visually.

Natural orifice specimen extraction surgery in left-sided colon and upper rectal cancer: a narrative review 左侧结肠癌和上直肠癌自然口标本提取术:叙述回顾

Background and Objective: Natural orifice specimen extraction surgery (NOSES) is an innovative technique, used in minimally invasive surgery for colorectal cancer. NOSES has been demonstrated as a safe and effective method in patients matching the inclusion criteria. Objective was to analyze and review articles regarding short- and long-term outcomes in NOSES in left-sided colon and upper rectal cancer. Methods: We conducted a search in PubMed and Cochrane Reviews databases and reviewed the studies in English regarding NOSES in left-sided colon and upper rectal cancer published until May 2022. Key Content and Findings: This literature review contains short summary of the recent articles’ results regarding NOSES in left-sided and upper rectal colon cancer. Studies showed better short-term outcomes and comparable with conventional laparoscopy long-term outcomes. Pitfalls and challenges of NOSES-studies include implementing standardized research protocols and thoughtful recruitment of patients due to the necessity of getting accurate and reproducible results. Conclusions: The advantages of NOSES in colorectal cancer treatment including reduction in postoperative pain and wound complications, less use of postoperative analgesics, faster recovery of bowel function have been described in left-sided colon and upper rectal cancer treatment. Oncological outcomes do not differ with conventional laparoscopic surgery with specimen extraction via laparotomy. is LCCS patients the best treatment results. Natural orifice specimen extraction 95% 0.59 to 1.25; I = 0%; P=0.43]. Two studies provided data on the 5-year OS rate.
