A woman stands at her son's grave in a newly opened section of a large cemetery in the Bronx. She and her therapist, psychologist Kathleen Romano, pluck dead leaves from the geranium planted there, and talk about the son as a child, as a man, and now as a spirit hovering nearby, at peace and released from the pain of drug addiction and AIDS. In an HIV-positive psychotherapy group at a methadone program, the men segue from English to Spanish and back again. Fred Millan, a Puerto Rican clinical psychologist who grew up in New York, conducts the group with the view that membership in an oppressed group is a significant dynamic. A hospitalized woman, with a long history of substance use, believed she would always be utterly alone. Now, members of her psychotherapy group for HIV-positive women congregate at her bedside, and among them is social worker Noel Elia, one of the group's facilitators.

There has been considerable progress in the management of HIV and the associated infections and malignancies. Many of the treatments used lead to oral complications, and some may cause systemic complications that have implications in oral care. Among the common toxicities of many medications are hematologic toxicity, neurologic toxicity including effects on memory, cognition, motor dysfunction and peripheral neuropathy, and enhanced risk of oral infection. It is essential that oral health care workers treating patients with HIV infection have a basic understanding of the medical therapies and their implications in oral care.

A sample of 60 clients with HIV disease were surveyed. The survey examined three areas of interest: locus of control (LOC), perceptions about professional health care (PHC), and general knowledge (KNOW) about HIV disease. Internally controlled clients were compared with clients who were externally controlled on variables of their perception of professional health care and their personal knowledge of this disease. Clients who demonstrated more internal control had a more positive perception of their health care. No difference was found on knowledge; both groups had a high degree of knowledge of AIDS.
