The differences in DSM-III-R psychiatric diagnoses between HIV-positive and HIV-negative patients were assessed. Subjects were patients admitted for psychiatric treatment to a metropolitan, state-supported facility. Experimental group subjects consisted of all adult patients identified as HIV-positive. Control group subjects were non-HIV-positive adult patients of the same gender and within 10 years in age of the experimental group subjects. Results suggest that HIV-positive subjects were significantly more likely to have received a personality disorder diagnosis and to have a history of intravenous psychoactive substance use. The likelihood of HIV-positive subjects to have a lower incidence of thought disorder diagnosis than non-HIV-positive subjects approached significance. No significant differences were obtained for diagnoses of psychoactive substance use disorders, psychoactive substance-induced organic mental disorders, mood disorders, or schizoaffective disorder. The diagnosis of a personality diso...

The HIV p24 antigen has demonstrated predictive value for HIV disease progression independent of CD4 count. An acidification procedure has resulted in 40–70 percent p24 antigen positivity across HIV clinical stages, although the predictive value of post-acidification p24 antigen for disease progression has not been fully validated. We hypothesized that there would be significant differences in p24 antigen positivity between matched cohorts of rapid progressors (RP) and slow progressors (SP). Seven hundred sixteen people enrolled in the USAF HIV Natural History Study were stratified according to the slope of CD4 change over time. RP were defined as people in the fastest 15 percent of CD4 decline who also progressed at least 2 Walter Reed (WR) stages. SP were defined as people in the slowest 15 percent of CD4 decline who did not progress more than 1 WR stage. Twenty-five RP were matched to 25 SP based on initial CD4 count (median CD4 RP=750, SP=720), WR stage, age, and duration of follow-up (RP=1086 das, SP...

With the rapid growth of HIV infection among women, the issue of how medical problems influence psychological functioning in the female population has acquired new meaning and urgency for health care policy on patient care. To develop effective strategies to meet the mental health needs of infected women we need a better understanding of the pattern of association between HIV and psychological distress. The objective of this study is to test several hypotheses that predict depression and anxiety in 95 adult females with HIV. This analysis addresses the following question: Is the level of perceived coherence, i.e., the extent to which individuals derive meaning and purpose in their lives despite their stage of HIV illness and degree of symptomatology, related to the level of psychological functioning?

The Florida dental AIDS tragedy involving Dr. David J. Acer and at least six patients represents one of the greatest unsolved mysteries in the annals of medicine. This article compares the social and psychological profile of the dentist with those obtained by studying two groups of serial killers and a set of people who knowingly exposed others to HIV. With each comparison, a very close match was found providing evidence that the doctor-to-patient transmissions were most likely intended. A discussion of the correlates, predictors, and theoretical motives for the infections is provided.
