Death by Opioids: Are there non-addictive scientific solutions? 阿片类药物致死:是否有不上瘾的科学解决方案?

In the face of the current Opioid crisis in America killing close to 800,000 people since 2004, we are proposing a novel approach to assist in at least attenuating these unwanted premature deaths. While we applaud the wonderful efforts of our governmental institutes and professional societies (NIDA, NIAAA, ASAM, ABAM ) in their extraordinary efforts in combating this continued dilemma, the current approach is failing, and other alternative approaches should at least be tested. These truths present a serious ethical dilemma to scientists, clinicians and counselors in the Reward Deficiency Syndrome (RDS) treatment community. It is important to realize that the current DSM-5 does not actually accurately display the natural brain reward process. The human brain has not been designed to carve out specific drugs like opioids, alcohol, nicotine, cocaine, benzodiazepines or cannabis and process addictions such as gambling as distinct endophenotypes. This is true in spite of natural ligands for cannabinoids, endorphins, or even benzodiazepines. The most accurate endophenotype is indeed reward dysfunction (e.g hypodopaminergic or hyperdopaminergic). With this mind, we are hereby proposing that the current Medication Assisted Treatment (i.e. 'MAT') expands to needed individuals as an initial "Band-Aid" to reduce harm avoidance, with the long-term goal of prophylaxis. So, to be clear, there may be other promising modalities other than MAT such as repetitive transcranial magnetic stimulation (rTMS), exercise and even new medications with positive allosteric modulators of GABA-A receptors, as well as the highly researched Genetic Addiction Risk Score (GARS) coupled with precision KB220Z. This will induce "dopamine homeostasis" to effectively rebalance and restore healthier brain function by promoting the cross talk between various brain regions (e.g. Nucleus accumbens, cingulate gyrus, hippocampus etc.) resulting in dopamine homeostasis. Our laudable goal is to not only save lives, but to redeem joy and improve the quality of life in the recovery community through scientifically sound natural non-addicting alternatives.

Our results showed that the prevalence of ID was 71% in boys versus 29% in girls, and several symptoms of ID were observed in our patients. The 24 bp duplication of ARX was not identified in any of the male patients tested. The prevention of ID is obviously based on the genetic, the social, the toxic and the infectious causes. Purpose of the study.- The objectives of this study were to: 1) to assess self-esteem, anxiety and depression in dyslexic Arabic-speaking children and adolescents, 2) to describe psychiatric comorbidities in these subjects by comparing them to their non-dyslexic peers. Patients and method.- 204 students (56 dyslexics and 149 good readers), pursuing their education in ordinary schools in the Beni Mellal-Khenifra region of Morocco, responded to Taylor's self-assessment scale of anxiety, the Beck's depression questionnaire, and the coopersmith self-esteem inventory (SEI). Results.- Overall, dyslexics are more anxious, more depressed and have a disturbed self-esteem compared to their "normal" peers. The percentage of psychiatric comorbidity is higher in the dyslexic group. Conclusion.- The results of this work highlight the need for a multidisciplinary approach that integrates emotional needs assessment into reeducational care of dyslexic children and adolescents In this study, the association between the reading acquisition level, the anxiety and depressive symptoms and self-esteem status is examined in children and adolescents school-age. Then, the links between these problems and academic achievement were analyzed. A sample of 245 students (130 boys and 115 girls) aged 9 to 17 years (M= 11.5 years), pursuing their studies in ordinary institutions school in the Beni Mellal-Khenifra region of Morocco, voluntarily participated in this research. The subjects completed the measurements of three self-evaluative scales: the Beck Depression Inventory (BDI), the Coopeersmith Scale of Self Esteem (SEI) and the Taylor questionnaire of manifest Anxiety. The results confirm that emotional manifestations are more frequent in "weak readers" and those with intermediate reading skills by comparing them to good readers. Academic performance and reading level are negatively correlated with anxious-depressive symptoms and positively with self-esteem (the correlations are significant). In each reading level, the analysis of the results does not show any relevant difference, in terms of the psycho-pathological disorders intensity, between girls and boys. These data urge education and public health officials to adopt multidisciplinary strategies to confront learning difficulties and emotional problems in the school environment. The aim of the present investigation was to evaluate the role of carnosine on aging-induced Aβ deposition in different brain regions (prefrontal cortex, hippocampal CA1 and CA3 zones, hypothalamus, pons-medulla and cerebellum) in connection with the locomotor activity (open field locomotion and rearing count) and corticosterone status in plasma and adrenal gland. During aging, both in 18 and 24 months aged rats (a) a graded deposition of Aβ in their brain regions was observed with a maximum deposition in prefrontal cortex of 24 months aged rats without any Aβ deposition in cerebellum, (b) the plasma and adrenal corticosterone level was increased, and (c) locomotor activity were reduced. Carnosine (2.0 μg/Kg/day, i.t.; for 21 consecutive days) reduced the aging-induced brain regional graded Aβ deposition and attenuated the aging-induced increase of both plasma and adrenal corticosterone level as well as the locomotor activity. Thus, it may be concluded from these observations that (a) aging reduces both rearing and locomotor activities which may be correlated with the increase of corticosterone in both plasma and adrenal gland with a graded Aβ accumulation in the brain regions, except cerebellum and (b) carnosine attenuates these aging-induced phenomena, observed in those brain regions, plasma and adrenal gland, and also in locomotor activities. *Correspondence to: Mrinal K. Young pregnant woman with mild dysphagia: Diagnostic and treatment workup predicting laryngeal schwannoma 轻度吞咽困难的年轻孕妇:诊断和治疗检查预测喉神经鞘瘤

Schwannomas of the larynx are uncommon, they represent the 0.1-1.5% of all the benign laryngeal tumours. Patients usually report multiple symptoms related to mass-effects and their treatment of choice is surgical excision. We present the case of a young woman presenting a schwannoma within her left false vocal fold, not suitable for endoscopic surgical approach. The suspected diagnosis of schwannoma was confirmed by the final histopathological report, showing a protein S110 positivity at the immunohistochemistry staining. Complete surgical excision is the treatment of choice and can be performed by either transoral or external approach depending on lesion dimension, patient’s laryngeal exposure and surgeon’s skills. In both cases we do suggest a conservative approach.
