Q fever is a worldwide infection caused by Coxiella burnetti. The acute form of this condition usually produces pneumonia and hepatitis through the direct effect of Coxiella burnetti on the involved organs. Here we describe the presence of autoimmune hepatitis and antiphospholipid antibodies as a transitory complication of this infection. These manifestations are probably more frequent than thought and they maybe underdiagnosed; so this report could be important for a deeper knowledge about Q fever and its complications.

Myeloid-derived suppressor cells (MDSC): Another player in the orchestra 髓源性抑制细胞(MDSC):乐队中的另一个演奏者

The simplest and most extended definition of myeloid-derived suppressor cells (MDSC) refers to them as immature myeloid cells with the ability to downregulate adaptative immune responses, a definition that reflects both their origin and function. Although initially described in experimental models and patients with cancer, accumulations of MDSC have also been found in other pathological conditions such as chronic/acute infections, autoimmune diseases and different types of stress. In all these situations MDSC may play their physiological role by modulating normal immune responses, both adaptive and innate. The mechanisms of action of MDSC are diverse requiring either cell to cell contact or the release of soluble factors. A better understanding of MDSC biology will open new windows of therapeutic opportunities, either by inhibiting their function (i.e. in cancer patients), or by enhancing their suppressive effects and promoting their expansion (i.e. in inflammation or autoimmunity).

The integrity of the interleukin 12/interferon gamma (IL-12/INF-γ) axis has been shown to be essential in the control of the Mycobacterium tuberculosis infection. The aim of this study was to assess whether the high incidence of tuberculosis (TB) in Galicia, Spain, could be related to an altered response in the IL-12/INF-γ axis in patients with TB. We studied 20 TB patients and 21 healthy controls: 7 with positive Tuberculin Skin test (TST+) and another 14 with TST–. The study of the IL-12/INF-γ axis was conducted by the analysis of INF-γ levels (in serum and supernatants of non-activated and activated cells) and by the cellular expression of IL-12Rβ1 and INF-γR1 in lymphocytes and monocytes, respectively. The assays were performed at baseline levels and after in vitro stimulation of peripheral blood mononuclear cells with phytohaemagglutinin (PHA) and purified protein derivative (PPD). Responses in patients and in healthy controls were analysed and compared. PHA and PPD in vitro stimulation significantly increased INF-gamma levels and the cellular expression of IL-12Rβ1 and INF-γR1 receptors compared to baseline levels in both TB patients and healthy controls (either with positive or negative TST). Our results suggest that there is an adequate immune response in all groups. Although the IL-12/INF-γ axis may have other abnormalities not analysed in this work, the functionality of the IL-12/INF-γ axis is preserved in the patients analysed with TB in Galicia. The high rates of disease historically observed in this community does not seem to be caused by a malfunction of the immune response at this level.

Influence of Toll-like receptor 2 and interleukin 10 on the intestinal epithelial barrier and their roles in inflammatory bowel disease toll样受体2和白细胞介素10对肠上皮屏障的影响及其在炎症性肠病中的作用

Different polymorphisms in key genes of innate and adaptive immunity disrupt the intestinal host-microbiotic balance in inflammatory bowel disease (IBD). In most cases these alterations affect the maintenance of the intestinal epithelium. Because of this, actions taken in order to reinforce or help intestinal epithelium recovery can provide benefits to idiopathic illnesses such as IBD. With this in mind, several studies support the idea that actions derived from the Toll-like receptor (TLR) 2 of the epithelial cells may help to maintain the epithelial barrier. However, in inflammatory conditions TLR-2 disappears from the intestinal epithelium and acquires a pivotal role in dendritic cells. In this event, the interleukin (IL)-10 production by sub-epithelial dendritic cells mediated by TLR-2 activation, thus leading to T-reg phenotypes in the lamina propria, may favour the recovery from epithelial damage. Therefore, the TLR-2/IL-10 axis can help to reinforce and recover the epithelial lining in IBD. In this way, several data suggest that early epithelial repair diminishes the antigen load reaching the lamina propria, which reduces inflammation and improves therapeutic performance. However, there is currently a lack of knowledge regarding molecular regulation of epithelial barrier by TLR-2 and IL-10.
