Background and aim: The present study evaluated the effect of triamcinolone on blood glucose levels in suprascapular nerve block in patients with periarthritis shoulder. Methods: The study was conducted in the Department of Anesthesia and Department of Orthopedics Dr. RPGMC Kangra at Tanda over a period of one year. Patients of both genders above 18 years with pain or stiffness in one or both shoulders suggestive of periarthritis shoulder with gross limitation of the active and passive range of movements for more than four weeks were included in the study. The patients were excluded from the study when 1) patients did not give consent to participate in the study, and uncontrolled diabetic patients with RBS >200mg/dl and HbA1c>6. Results: A total of 35 patients were included. Majority (37.14%) aged between 51-60 years, mean age of the patients was 56.97 years. 51% were males. Right PA was the most common among 10 patients. We found that there was no significant difference in mean blood glucose levels in and triamcinolone group at baseline, 2 hours, Day-3, and Day-7. Conclusion: Blood sugar levels did not rise significantly when triamcinolone was used for suprascapular nerve block in patients with periarthritis shoulder.

Objective: To evaluate the rate and type of contamination in open-system adapters used for filling the anesthetic agent sevoflurane across hospitals in India. Methods: Nineteen reusable adapters used for filling sevoflurane were collected from nine hospitals in India and evaluated for microbial contamination. Samples were collected by flushing the inner lumen and swabbing the outer surfaces of adapters and dispatched to a central laboratory in sealed sterile tubes/containers for culture and microbial identification. Results: Out of 19 swab samples collected from the outer surfaces, 11 (57.9%) were found to be contaminated/colonized. Out of these, 5 (26.3%) were found to be contaminated with Gram-positive cocci belonging to the Staphylococcus species, and 6 (31.6%), with Gram-negative organisms Pseudomonas aeruginosa (5 [26.3%]) , and Burkholderia cepacian (1 [5%]). Out of 19 rinse samples from the inner lumens, 8 (42.1%) were found to be contaminated, 2 (10.5%) with Gram-positive cocci belonging to Staphylococcus species and 6 (31.6%) with Gram-negative organisms Achromobacter denitrificans and Aeromonas enteropelogene (one (5.3%) sample each), and Pseudomonas aeruginosa and Ralstonia pickettii (2 (10.5%) samples each). Conclusion: Reusable open-system filling adapters are susceptible to contamination with nosocomial pathogens; therefore, single-use adapters to inhibit contamination and prevent hospital ‐ acquired infections should be encouraged.

Abstract: Intracranial aneurysm is one of the most common neurovascular complications. During the recent years the accepted treatment of enraptured cranial aneurysm is noninvasive endovascular coiling. This technique is modern but it is not without complications which can be serious and life-threatening. A clinical case of a patient admitted to the ICU of Military Medical Academy - Sofia with sub arachnoid hemorrhage is described. After a positive clinical course, the check-up magnetic resonance showed intracranial aneurism of the right carotid artery. The patient underwent angiographic endovascular treatment. Vasospasm of the middle and right brain artery and thrombosis were detected during the procedure. Attempt of thromboaspiration was made without success. This article reviews published data on broad-spectrum researches concerning complications of endovascular coiling of intracranial aneurysms and the ways to prevent and reduce them.

: Background : Over the past two decades, minimally invasive techniques have gained popularity in cardiac surgery that enable access to the heart via sternotomy free approaches for most adult procedures and isolated coronary artery bypass grafting. Here we present our experience and the strategies we used and the challenges we faced while starting a minimally invasive and robotic cardiac surgery program at a newly established tertiary care centre. Methods : We looked back at our methods and retrospectively analyzed our results of minimally invasive and robotic cardiac surgery in terms of morbidity and mortality and complications and other quality parameters like ICU stay and re-exploration rate etc as described later in detail. A step wise approach was adopted that introduced every team member to minimally invasive and robotic surgery in a gradual fashion slowly gaining confidence and increasing the complexity of the procedures. Results : 105 cases of minimally invasive and robotic cardiac surgery were performed over past one year. Out of these 92 were done by minimally invasive technique and 13 were done by robotic assistance. There was no mortality and none of the patient required any conversion to sternotomy or emergency bypass. One patient had to be re-explored for bleeding. The quality indicators were noted in terms of results and complications. Conclusion : Minimally invasive and robotic cardiac surgery is fast becoming procedure of choice across the world including in our country due to its many advantages in terms of early recovery, cosmesis and less blood product requirement but has got steep learning curve and is technically more difficult. So a gradual step wise technique with proper training and guidance has to be adopted to establish a successful program.

Heterotopic pregnancy is a presence of intrauterine and extra-uterine gestation concurrently. It might be rare but possess great challenge for an anaesthetist as both maternal and foetal safety are at risk. We report a case of 28 years primigravida diagnosed as heterotopic pregnancy and shifted for an emergency laparoscopic ectopic excision under General anaesthesia. Our primary concern for maternal and foetal safety for undergoing surgery in first trimester pregnancy was achieved by Meticulous attention to preoperative counselling, airway management, aspiration prophylaxis, haemodynamic stability, choice of anaesthesia and anaesthetic drugs which have minimal impact on utero-placental perfusion and uterine relaxation. On follow up the patient delivered a healthy full term live female child weighing 2.7 kg by elective cesarean section.
