Bortezomib-Induced Bilateral Eye Swelling and Cutaneous Adverse Reaction in a Patient with Plasma Cell Leukemia—A Case Report 硼替佐米致浆细胞白血病患者双侧眼肿胀及皮肤不良反应1例报告

Bortezomib, a proteasome inhibitor, is an established therapy against plasma cell leukemia—a variant of plasma cell dyscrasias. Its most frequent side effects have been listed as peripheral neuropathy, neuropathic pain, thrombocytopenia, and gastrointestinal problems. Allergic skin reaction is a rarely documented side effect in patients receiving bortezomib-based chemotherapy. A combination therapy consisting of intravenous bortezomib, oral Revlimid tablets, and oral dexamethasone tablets has been prescribed for the patient after his recent diagnosis of plasma cell leukemia. While receiving his third treatment cycle, he developed an allergic reaction (skin rash) involving the neck, and wrists, and mild bilateral eye swelling. The infusion was stopped immediately and then ciprofloxacin ophthalmic solution and oral diphenhydramine 25 mg were prescribed to the patient with significant improvement in his clinical condition. He was temporarily taken off bortezomib. At a follow-up visit a week later, a significant improvement was noticed in his condition. Rash had reduced on neck and wrists, and eye swelling had reduced as well. As of the time of writing this case report, he has been temporarily taken off bortezomib, but other medications in the treatment regimen were continued as prescribed.

A Review of the Impact of Covid-19 on the Male Genito-Urinary Tract 新冠肺炎对男性生殖尿道影响的研究进展

Introduction and Objectives: Since its advent in 2019, Corona Virus Disease 19 (Covid-19) has changed everyday life in many unprecedented ways. With the availability of Covid-19 vaccines, clinical practice has stepped gradually towards its everyday routine. The clinician, however, needs to address such concerns as “Can Covid-19 present with urinary tract symptoms predominantly?”, “Can the urinary tract develop long-term Covid-19 symptoms?”, “Can Covid-19 vaccines/drug therapy cause male infertility?” Methods: This mini-review of the current literature has been carried out by searching the Pubmed database. Results and Conclusions: Our literature review suggests that there is insufficient evidence to conclude a significant impact on the urinary tract from Covid-19 infection. There is supporting evidence that mRNA anti-SARS-CoV-2 vaccines are safe for male gonads. Research to further support these conclusions is required.

Background: A subset of cancer is particularly vulnerable to hydration prior to chemotherapy protocols; however, real-world outcomes-based data on pre-hydration elements of patients with tumor is sparse. Case Presentations: This retrospective case series describes a cohort of adult patients with terminal stage cancer who were seen between July 1, 2023, and September 25, 2023 and underwent various chemotherapy protocols, which, to our knowledge, represents comprehensive analysis of this patient population. We performed a retrospective search of patients identifying 50 cases of the chemotherapies of patients with cancer and extracted the clinical data via individual chart reviews. At the time of diagnosis, 24 patients were prehydrated with NaCl with KCl prior to the chemotherapy against the tumor, and 26 patients were taking normal saline only before undergoing cancer-specific treatment. Hydration and, prior chemotherapeutic agents use were the most common predepositse. Carcinomatosis peritoneii were the most common signs of the terminal stage of cancer within patients. Twenty-four patients were admitted to the hospital and two received NaCl with KCl hydaration prior to the platinum based chemotherapeutic agent treatment. Twenty-six patients received only normal saline hydration. Conclusion: Our data suggest that NaCl with KCl prehydaration results in better prognosis, including reduced tumor burden or decreased lymph node invasion, when compared to other vulnerable patients populations that receive only normal saline hydration. The graphene exfoliation effect of NaCl with KCl may have contributed to these results.
