Background: Inflammatory bowel disease( IBD) is a group of chronic and non-specific intestinal inflammatory diseases of undetermined origin. Functional impairment of macrophages has been associated with the dysregulation of mucosal immunity in intestinal tract of patients with IBD. Aims: To investigate the correlation of serum levels of macrophage polarization-related cytokines with the development and disease activity of IBD. Methods: A total of 105 IBD patients admitted from May 2013 to May 2014 at Shanghai Ruijin Hospital were recruited,of them 65 were Crohn's disease( CD) and 40 were ulcerative colitis( UC). Twenty-four patients with colonic polyps were served as controls. Serum samples were obtained and the levels of interleukin-1beta( IL-1β),IL-6,IL-10,IL-13,interferon-gamma( IFN-γ) and inducible nitric oxide synthase( i NOS) were determined by ELISA method. Results: Serum levels of IL-1β,IL-6,IL-10,IL-13 and IFN-γ were significantly higher in CD group than in control group( P 0. 05),and serum levels of IL-10,IL-13 and IFN-γ were significantly higher in UC group than in control group( P 0. 05). Logistic regression analysis revealed that serum IL-13( OR = 1. 009,P = 0. 005) and i NOS( OR = 0. 982,P = 0. 013) were correlated with CD,while Spearman correlation coefficient demonstrated a link between serum IL-10 and disease activity of CD( rs=- 0. 432,P =0. 014). No correlations were observed between serum levels of these cytokines and development and disease activity of UC( P 0. 05). Conclusions: Serum levels of macrophage polarization-related cytokines increase to varying degrees in IBD patients,but these cytokines have no obvious correlations with IBD and its disease activity. Presumably,theses cytokines are only involved in but not the triggers in the development of IBD.

Background:Estrogen and estrogen receptor(ER) are considered to play important roles in the tumorigenesis and progression of colon cancer,however,the mechanism of ER in colon caner is still unclear.Adaptor protein p66Shc is upregulated by steroid hormones in hormone-sensitive cancer cells,which is correlated with the steroid hormone-stimulated cell proliferation.Aims:To investigate the expressions and significance of ER and p66Shc in colon cancer.Methods: Protein expressions of ER and p66Shc in 65 paraffin-embedded colon cancer tissue and the corresponding adjacent noncancerous tissue were detected by immunohistochemistry.Correlation of p66Shc protein expression with the clinicopathological characteristics of colon cancer was analyzed.Results;Expressions of ER and p66Shc protein were located in nucleus and cytoplasm,respectively.Positivity rates of both ER and p66Shc protein were significantly higher in colon cancer tissue than in adjacent non-cancerous tissue(ER:47.7%vs.0%,P 0.01;p66She:49.2%vs.0%,P 0.01).Moreover,positive correlation was found between ER and p66Shc protein expressions(r = 0.43,P 0.05 ). Expression of p66Shc protein had no correlation with gender,age,primary tumor location,tumor size,gross type,depth of tumor invasion,lymph node metastasis and AJCC stage,however,it was significantly correlated with histological grading of colon cancer(well differentiated;22.2%,moderately differentiated:47.7%,poorly differentiated;75.0%,P = 0.046).Conclusions;Colon cancer might be an estrogen-sensitive malignancy.Estrogen and ER might cooperate with p66Shc to play a role in the tumorigenesis and progression of colon cancer.

Inflammatory bowel disease(IBD)is a chronic intestinal inflammatory disease,and its etiology has not yet been clarified.Recent studies have shown that expressions of microRNA(miRNA)were frequently altered in serum and intestinal tissue of patients with IBD,and the aberrant miRNA expressions are correlated with the disease activity and site of lesion,etc.miRNA plays a role in the diagnosis and exploring disease pathogenesis of IBD,as well as its targeted therapy.This article reviewed the aberrant miRNA expressions in patients with IBD in order to clarify the role of miRNA in the pathogenesis and development of IBD,and to further provide the evidence for its clinical application.

Background:Leptin may be involved in the formation and development of liver fibrosis.AngiotensinⅡtype 1 receptor(AT1R) antagonist can decrease the synthesis and secretion of leptin in adipose tissue in rats.Aims:To evaluate the effect of valsartan,an AT1R antagonist,on expression of leptin in rats with immunodamaged hepatic fibrosis.Methods: Thirty-six Sprague-Dawley rats were randomly divided into normal control group,model group and valsartan prevention group.In the latter two groups,liver fibrosis model was constructed by intraperitoneal injection with hog serum.Valsartan (30 mg/kg) was intragastrically administered daily after the construction of liver fibrosis in valsartan prevention group.At the 10th week,all the rats were sacrificed.The degree of fibrosis and collagen area were evaluated by HE and Sirius red staining.Expression of leptin was determined by immunohistochemistry staining.Results:As compared with normal control group,the degree of fibrosis(P0.05),collagen area(10.08%±2.01%vs.0.62%±0.31%,P0.01) and expression of liver tissue leptin(5.05±2.91 vs.0.44±0.27,P0.05) were obviously increased in model group.After intervention with valsartan,all the above-mentioned indices significantly ameliorated.Conclusions:Leptin can induce liver fibrosis,and AT1R antagonist valsartan can delay the development of liver fibrosis via the reduction of leptin expression in liver tissue, which may be one of the mechanisms of its anti-fibrosis effect.

Gallstone ileus is an uncommon mechanical bowel obstruction caused by impaction of one or more gallstones in the intestine. The ileum is involved in about 80% of cases. Difficult and delayed diagnosis and old age of the patients account for its high mortality. We herein report one case with gallstone ileus diagnosed by ultrasound preoperatively in which the patient survived after surgery with only a few complications related to the patient's debilitated medical problems. Emphasis is also put upon how to deal with acute abdomen in elderly patients; in addition to traditional roentgenography, ultrasound may afford a tool to detect gallstone ileus.

Hookworms usually reside in the small intestine, so they are rarely discovered during routine upper gastrointestinal endoscopy (UGIE). We herein report two cases of hookworm infestation diagnosed by UGIE. One of the two cases combined with gastric adenocarcinoma and no parasite ova was detected in stool examination. Ancylostoma duodenale and Necator americanus can infect humans. In Taiwan, N. americanus is prevalent in the north and the east, and A. duodenale is prevalent in the south and the central region. The infection rate in Taiwan is less than 1%. The adult hookworm resides mainly in the jejunum and may be refluxed toward the duodenum and the stomach during bowel contraction. Endoscopically, hookworms are shining red in color, about 10mm in length, and a couple of female and male worms usually hug together tightly. There is no report to suggest the relation between hookworm infestation and gastric malignancy in the literature.
