International integrative primary care outcomes study (IIPCOS-2): an international research project of homeopathy in primary care 国际综合初级保健结果研究(IIPCOS-2):顺势疗法在初级保健中的国际研究项目

Objective: The primary objective is to evaluate the effectiveness of homeopathic treatment compared to conventional treatment for respiratory and ear complaints commonly seen in the primary care setting: runny nose, sore throat, ear pain, sinus pain, and cough.

Method: This study is an international, multi-center, prospective, outcomes study on the effectiveness of homeopathic treatment compared to conventional treatment. The participating investigators will be divided into three groups:

  • Homeopathic treatment;

  • Homeopathic or conventional treatment according to the patient’s preference, either randomized or non-randomized;

  • Conventional treatment.

A total of 2400 patients will be recruited. Consecutive patients, age one month or older, presenting with one of the five chief complaints, and onset of symptoms less than seven days will be included into the study. Prior to enrolment into the trial each patient must provide written informed consent. Patient outcome will be measured using the Integrative Medicine Outcomes Scale (IMOS) at 7-, 14- and 28-d telephone follow-up. In addition, covariate data related to the following will be collected: patient demographics and medical history, patient preference for treatment and willingness to be randomized, health-related quality of life, primary treatment and adjunctive therapies prescribed, adverse events, type and length of consultation, follow-up recommendation, patient compliance, patient satisfaction with treatment, and patient willingness to choose therapy and health care provider again. The main outcome criterion will be the response rate according to the IMOS after 14 days of treatment.

Results: Preliminary interim results will be presented.

Conclusions: Lessons learned from the study will be discussed.

Selection of ten criteria: non-exclusive categories were considered (clinical, pathological, epidemiological, economic and social). The criteria were applied for the clinical domains of medical consultation and homeopathic therapy. Testing the criteria: a scale of three binomial categories was used. `Best' (B) was given 8 ± 10 points, and is the most important group to develop clinical research; `Acceptable' (A), with 5 ± 7 points, is considered to be of moderate importance; `Low' (L), with 1 ± 4 points, is of least importance. The criteria values were one point when predominantly present and zero point if predominantly absent.

A meta-analysis of the homeopathic treatment of pollinosis with Galphimia glauca 顺势疗法治疗花粉症伴青光眼的荟萃分析

OBJECTIVE1) To assess the efficacy of homeopathic prepared Galphimia glauca compared to placebo in the treatment of pollinosis. 2) To estimate the corresponding overall success rate of Galphimia glauca. Meta-analysis of clinical trials.STUDY SELECTION7 randomized double-blind placebo-controlled trials and 4 not placebo-controlled trials (1 randomized and controlled, 1 prospective uncontrolled, 2 retrospective uncontrolled) performed by our study group between 1980 and 1989. An additional MEDLINE search revealed no further trials on this topic. Exclusion and inclusion criteria were identical over all trials. In total, 1038 ambulatory patients that suffered from acute pollinosis (752 in placebo-controlled trials) entered the analysis.MAIN OUTCOME MEASURESRelative frequency and relative risk for showing noticeable and soothing relief in ocular symptoms as assessed by the patient. The overall rate of improved eye-symptoms is about 1.25 (CI: 1.09 to 1.43) times higher in the verum than in the placebo group. Verum success rate is estimated by 79.3% (CI: 74.1% to 85.0%). Across the single studies the results were highly comparable except for the study run in 1985. A significant superiority of Galphimia glauca over placebo is demonstrated. Estimates of verum success rates are comparable with those of conventional antihistaminics, but no side effects occurred. As not all of the single studies were analyzed by intention to treat analysis the results may be biased.

An evaluation of Coffea cruda effect on rats 咖啡原料对大鼠的作用评价

To investigate the effect of the homeopathic medicine Coffea cruda on sleep pattern, it was orally administered to rats at the beginning of their waking period. EEG from the parietal region was recorded during their next sleep cycle. Applying an FFT algorithm, spectral in the band, 0.5–2.5 Hz, was chosen as a marker parameter, evaluated for control and verum groups using a double-blind protocol. Power in the verum group was statistically higher than baseline value, it was not statistically different in the control group. The results indicate that an enhancement in EEG slow delta activity is associated with Coffea cruda.

Improving the accessibility and quality of clinical and patient information in the patient’s medical notes 提高患者病历中临床和患者信息的可访问性和质量

Objectives To improve the accessibility and quality of information in the medical notes including: the patient’s general and homeopathic history at a first consultation, the rationale for the homeopathic prescription, the evaluation of the prescription, the evaluation of the progress of the patient at follow-up visits, and the usage of conventional medication. Information needs to be accessed easily from the medical notes and needs to be of sufficient quality to be useful to clinical staff and others. This information is one source for measuring quality in clinical audit, which is an important part of the new concept of Clinical Governance which is now a requirement for British public sector hospitals. Various doctors had highlighted the difficulty in finding information easily from the notes, particularly if the patient was new to them.
