Stent fractures are not uncommon in coronary intervention especially in given era of extensive development in technology, which made us to take up challenging cases like highly calcific lesions, chronic total occlusions, left Main interventions etc. Not to forget these challenges comes with risk of complications. Knowing these complications and managing properly is necessary. However, some of complications like, ostial left main stent fractures are so very rare, and really worthy to be reported.

Aortic valve stenosis is a common valvular problem in the aging population. Transcatheter aortic valve implantation (TAVI) is a popular treatment alternative to surgical aortic valve replacement for frail elderly individuals with symptomatic severe aortic valve stenosis. There are multiple research reports documenting the effectiveness of TAVI in elderly, but here we present a case of TAVI for Aortic stenosis (AoS) with accompanying presence of coronary and cerebral occlusion.

Coronary artery anomalies are estimated to be present in 1.8% of the population. Though majority of them are benign, some may lead to angina pectoris, myocardial infarction, cardiac arrhythmias, congestive heart failure and sudden cardiac death, especially in the young. Hence the need arises to identify and treat possible malignant anomalies.

The common femoral artery is often used as a preferred puncture site in various procedures with an incidence of severe complications of less than 0.5%. Various complications of femoral artery puncture have been documented lately due to the exponential growth of interventional cardiology. Femoral artery puncture causing a pseudoaneurysm, communicating arteriovenous fistula and associated venous thrombosis has rarely been reported in literature.

Intravascular lithotripsy converts electrical energy into sonic waves which selectively break calcium facilitating better stent expansion in calcified coronary arteries. There is a small chance of myocardial injury caused by IVL presenting as post-cardiac injury syndrome (PCIS). This is the first case of documented PCIS following the use of IVL.

Transcatheter aortic valve implantation (TAVI) in post-MVR (mitral valve replacement) patients is a technical challenge due to the respective proximity of the aortic annulus and non-compliant prosthetic mitral valve ring. There are only a few case reports in literature which have reported this procedure. We herein describe a case of TAVR in rheumatic heart disease patient having prosthetic mitral valve whose distance between aortic annulus to mitral rim was only 3.8mm.

We present a case of a 20 year old female who suffered from multiple traumatic injuries across the body after a fall from height. Over the course of her hospital stay, the patient developed chylo-hemothorax which then led to the presentation of prolonged asymptomatic bradycardia in the patient. The chylo-hemothorax was managed efficiently with a tube thoracostomy and the other necessary treatment protocols and there was subsequent resolution of the complaints over the passage of days.

A 7year male with heart failure and hypercholesterolemia was found to have severe narrowing of abdominal aorta and proximal left anterior descending artery with left ventricular dysfunction. Apart from lipid-lowering therapy a staged balloon dilation of aorta and coronary intervention were done successfully. This case highlights the clinical spectrum of familial hypercholesterolemia and its management.
