Objective: To determine the incidence of influent factors in failure and multiple punctures for the placement of central venous catheter for hemodialysis (CVCHD). Methods: A descriptive analytical study, patients with hemodialysis requirements from February to October 2014 were included. Nurses of the vascular access clinic recorder clinical characteristics, as well as the number of necessary punctures, fail to place the CVCHD and its immediate complications. The analysis of the data was done with descriptive and inferential statistics as corresponded; multiple logistic regression was done. Results: 180 procedures were analyzed, of which 170 hemodialysis catheters (94.4%) were able to be placed. The failure of the CVCHD placement was more frequent in patients with a previous catheter history with an OR 1.46 (95% CI 1.03 - 2.05). We present the case of a 42-year-old man with chronic kidney disease grade 5 in hemodialysis who has been injected for 14 years by non-professional health personnel, subcutaneous material (silicone polymers and paraffin oil) to obtain an appearance athletic subsequently developed an exceptional bone mineral disorder, characterized by: hypercalcemia, deficiency of 25 hydroxy Vitamin D, increase of 1,25-OH-Vitamin D and decrease of intact paratohormone. In addition, calcifications in the places where I apply the material such as: thighs, lumbar area and in target organs such as abdominal aorta, nephrocalcinosis, renal lithiasis and granulomas. This case exemplifies the systemic calcification developed by the interaction between the inflammation of the injected foreign material and the bone mineral metabolism.

Objectives: To know the frequency of presentation of acute tubulointerstitial nephritis (ATIN) in the last three decades, its forms of presentation, etiology, clinical manifestations and findings in the urinary sediment. Material and methods: A descriptive, retrospective cross-sectional study was carried out in Cuban patients who underwent kidney biopsy of own kidneys at the Nephrology Institute, Havana, Cuba, in the period 1988-2018. The information was processed in an automated way (SPSS 22.0). Analysis of frequency distribution, average and the homogeneity test were used. Results: Of 2472 biopsies performed, 77 (3.1%) were diagnosed as ATIN. The frequency of this diagnosis increased, from 1.1% in the period 1988-1998, to 3% in 1999-2008, up to 4.5% in the period 2009-2018 (p = 0.044). The main form of presentation was acute kidney injury (AKI) (83.1%), skin rash, eosinophilia and fever triad was present in 11.7% of patients and the most frequent cause was medication
