Control del dolor e intervenciones no farmacológicas 疼痛控制和非药物干预

Objetivo

Evaluar el uso y el conocimiento global de las intervenciones farmacológicas entre enfermeras clínicas, así como la eficacia del tratamiento para el dolor en un centro médico de atención terciaria.

Métodos

En este estudio descriptivo se utilizaron métodos cuantitativos y recogida de datos transversales involucrando un muestreo de conveniencia de enfermeras. Se utilizó la triangulación con tres métodos de recopilación de datos distintos para otorgarle más rigor (v. el cuadro Definición de términos).

Resultados

Todos los participantes utilizaron una o más intervenciones no farmacológicas. Las intervenciones más utilizadas fueron posicionar y cambiar de posición (97%), aplicación de frío o calor (95,6%) y distracción (92,7%). Las intervenciones menos utilizadas fueron las prácticas espirituales (20,6%), la neuroestimulación eléctrica transcutánea (10,3%) y el masaje superficial (32,4%).

Conclusión

Este estudio proporcionó un primer avance del uso de intervenciones no farmacológicas por enfermeras clínicas e identificó oportunidades para investigaciones futuras y para la formación de estudiantes de enfermería y enfermeras en la práctica. Como las enfermeras creen que no han recibido suficiente formación en métodos no farmacológicos para el tratamiento del dolor, deberían recibir más educación para evaluar y entender el dolor de manera integral, así como el uso de intervenciones no farmacológicas1.

Incorporación de la técnica ecoguiada en la inserción periférica de vías centrales: un nuevo reto para enfermería en cuidados intensivos 生态引导技术在中央通路外周插入中的应用:重症监护护理的新挑战

Objectives

To describe the results of the incorporation of ultrasound guided technique for peripherally inserted central catheters (PICC) in a polivalent intensive care unit.

Methods

Descriptive and comparative observational study. 140 cases were included. Demographic data, type of catheter, use of ultrasound or not, description of the procedure and main complications were collected.

In the descriptive statistics, frequency distributions were performed for qualitative data, and mean and standard deviation for the quantitative variables.

Results

72.9% male and 27.1% female patients. Average age: 63.43 years. 94.5% successful peripheral inserted central catheter (PICC's) insertion. Ultrasound guided technique was used in 48%. The main reason for its use was a difficult venous access or visualization (39%). There were no significant differences between ultrasound guided puncture and blind punture regarding the complications: phlebitis (1.4% vs 2.8%), infections (0.7% vs 0%) and bacteremia (0% vs 0.7%).

Conclusions

The results obtained show that PICC's placement in our intensive care unit is a procedure with high percentage of success and very low incidence of infections, and with positive results after the incorporation of the ultrasound for difficult venous access.
