Research progress on the mutual effects between brown adipose tissue and bone 棕色脂肪组织与骨骼相互作用的研究进展

Brown adipose tissue (BAT) increases energy consumption by directly dissipating stored energy in the form of heat through the role of uncoupling protein (UCP1). Recent studies have found that brown adipocytes may also regulate metabolism through autocrine, paracrine, and endocrine mechanisms. A growing body of evidences have shown that the BAT has a close relationship with bone metabolism, in which BAT secretes a variety of factors to regulate bone metabolism, while bone also secretes a variety of bioactive substances to control BAT function. In addition, BAT may indirectly participate in bone metabolism through muscle-mediated regulation or SNS activity and improvement of body metabolism, thus forming a BAT-skeletal axis. In this paper, we try to explain the relationship between brown adipose tissue and bone, and to discuss their interactive mechanisms. On the consistency of different methods for detecting aldosterone concentration in blood 血液中醛固酮浓度不同检测方法的一致性

Objective To evaluate the consistency of different methods for detecting aldosterone concentration in blood and to establish a reference interval of serum aldosterone concentration by liquid chromatography-tandem mass spectrometry(LC-MS/MS). Methods Concentrations of blood aldosterone were measured by LC-MS/MS, chemiluminescent assays (Diasorin, Domestic A and B systems) and radioimmunoassay (RIA) in 138 healthy adults, 67 patients with essential hypertension and 23 patients with primary aldosteronism. Results Aldosterone concentrations measured by various methods were quiet different(P 0.05) between CLIA(Domestic B) and LC-MS/MS, 0.338(P<0.01) as well as between RIA and LC-MS/MS. Bland-Altman analysis for the measurements showed poor consistency among methods for aldosterone concentrations. The reference interval was 15.2-222.2 pg/ml for serum aldosterone concentrations by LC-MS/MS. Conclusions There are significant differences of blood aldosterone concentrations determined by different methods. Clinically, a specified reference interval might be needed while using different methods. 目的评价不同检测血中醛固酮浓度方法的一致性,建立液相色谱-串联质谱(LC-MS/MS)测定血清醛固酮浓度的参考区间。方法对138例健康成人、67例原发性高血压患者和23例原发性醛固酮增多症患者,采用LC-MS/MS、化学发光法(Diasorin、国产A、B系统)和放射免疫法(RIA)测定血醛固酮浓度。结果CLIA(Domestic B)法测定的醛固酮浓度与LC-MS/MS法测定的醛固酮浓度差异无统计学意义(P 0.05), RIA法测定的醛固酮浓度与LC-MS/MS法测定的醛固酮浓度差异无统计学意义(P<0.01);Bland-Altman分析表明,醛固酮浓度测定方法的一致性较差。LC-MS/MS测定血清醛固酮浓度的参考区间为15.2 ~ 222.2 pg/ml。结论不同方法测定的血醛固酮浓度有显著性差异。临床上,使用不同的方法可能需要一个特定的参考区间。关键词:醛固酮;液相色谱-串联质谱法;化学发光分析;放射免疫检定法

Proteomics analysis of hippocampus and striatum in rats with hyperglycemia using iTRAQ technique 应用iTRAQ技术对高血糖大鼠海马和纹状体的蛋白质组学分析

Objective To screen the deferentially expressed proteins in hippocampus and striatum in rat models of diabetes mellitus and normal SD rats, and to elucidate the effects of hyperglycemia on central nervous system. Methods SD rats were randomly divided into normal group(n=10)and hyperglycemia group(n=15); rat models of diabetes were induced by the high-fat and high-sugar diet combined with single intraperitoneal injection of streptozotocin. The hippocampus and striatum tissues from hyperglycemia rat and normal SD rat(control group) were selected as specimens. Isobaric tags for relative and absolute quantification(iTRAQ) technique and 2-dimensional liquid chromatography-tandem mass spectrometry(2D-LC-MS/MS) were used for identifying the deferentially expressed proteins. The deferentially expressed proteins were analyzed by bio-informatics for searching candidate proteins.Finally, the candidate proteins were verified by Western blotting. Results A total of 347 proteins showed significantly different expressions, which included 139 up-regulated proteins and 289 down-regulated proteins. These proteins included metabolic glutamate receptor 2, ferritin, leucine repeat protein and complex 2, which are related to Parkinson′s disease. KEEN search indicated that the pathway of dopaminergic synaptic, complement, gap junction, autophagic and Fcgamma R-mediated phagocytosis were involved with these significant differentially expressed proteins. The protein interaction network showed that ferrin, aldehyde/ketone reductase(ARK), Ca2+ -binding protein(CaBP), and protein tyrosine phosphatase(PTP) were located at the crossed nodes. The expression levels of ferrin, ARK, PTP, and CaBP, which were verified by Western blotting, and the results were consistent with those of the quantitative mass spectrometry(P<0.05). Conclusion Deferentially expressed proteins in hippocampus striatum of diabetic rats can be effectively screened by iTRAQ technique combined with 2D-LC-MS/MS. It may provide new clues for the mechanism of neurodegenerative disease. Repeated epistaxis caused by primary hyperparathyroidism with multiple brown tumors: One case report 原发性甲状旁腺功能亢进伴多发褐色肿瘤致反复鼻出血1例

To report the clinical, imaging and pathological feature of a case of primary hyperparathyroidism(PHPT) with multiple brown tumors whose complain was repeated epistaxis, and to conduct a retrospective analysis with relevant literatures. A postmenopausal middle-aged woman, with a history of joint pain, fracture, and kidney stone which were neglected due to the lack of specificity of symptoms for years, went to hospital due to repeated epistaxis. Laboratory results showed high blood calcium, hypophosphatemia, alkaline phosphatase(ALP), and parathyroid hormone(PTH) levels, as well as increased urinary calcium. Imaging examination showed systemically multiple brown tumors and extensive osteoporosis. Meanwhile an abnormal mass was found behind the right thyroid. Taking all findings into account, the diagnosis was made with PHPT, and the histopathology further confirmed that the lesion was a parathyroid adenoma. This case seems to be a rare case of multiple brown tumor caused by parathyroid adenoma, which compressed and destructed nasal blood vessels, thus induced a repeated nasal discharge. After resection of parathyroid adenoma, the patient experienced a transient hypocalcemia and PTH level elevation. After 5-month follow up, part of the brown tumors spontaneously regressed, bone density increased, and her clinical symptoms were mostly relieved. The use of antidiabetic medications in community-dwelling diabetic patients in Shanghai in 2018 2018年上海市社区糖尿病患者抗糖尿病药物使用情况

Objective To investigate the use of diabetes medications and their effects on the community diabetic patients in Shanghai, China, and provide the evidence for the use of antidiabetic drugs in diabetic patients in the region. Methods The data were from a database of a 2018 Survey on Community Diabetes Mellitus in Shanghai, China. There were 4 612 subjects included in this cross-sectional study in 2018. According to the use of antidiabetic drugs, the population was divided into untreated group, single drug group, double drugs combination group and multi-drugs combination group, to compare the fasting blood glucose, glycosylated hemoglobin, BMI and prevalence of diabetic complications in different groups. Results About 70.9% of the 4 612 patients used hypoglycemic agents, 34.8% used metformin, 35.1% used sulfonylureas, 22.9% used alpha glycosidase inhibitors, and 13.8% used insulin. The prevalence of diabetic nephropathy, retinopathy, neuropathy, stroke, and diabetic foot was higher in the combination than in the untreated and single-drug users (P 0.05). Conclusion The most common used antidiabetic drugs in diabetic patients in Shanghai are metformin, sulfonylureas, α-glycosides inbibitor, and insulin. The blood glucose control in diabetic patients in Shanghai community is not good enough. Patients with a longer duration of diabetes, a lower rate of HbA1C at goal, and a higher prevalence of diabetic complications may be more prone to use multiple hypoglycemic drugs. 目的了解上海市社区糖尿病患者糖尿病药物的使用情况及其影响,为该地区糖尿病患者使用抗糖尿病药物提供依据。方法数据来自中国上海市2018年社区糖尿病调查数据库。2018年,共有4612名受试者参与了这项横断面研究。根据抗糖尿病药物的使用情况,将人群分为未治疗组、单药组、双药联合组和多药联合组,比较不同组的空腹血糖、糖化血红蛋白、BMI和糖尿病并发症的发生率。结果4612例患者中约70.9%使用降血糖药物,34.8%使用二甲双胍,35.1%使用磺脲类药物,22.9%使用α-糖苷酶抑制剂,13.8%使用胰岛素。联合用药组糖尿病肾病、视网膜病变、神经病变、中风和糖尿病足的患病率高于未治疗组和单药组(P<0.05)。结论上海地区糖尿病患者最常用的抗糖尿病药物是二甲双胍、磺酰脲类药物、α-糖苷抑制剂和胰岛素。上海社区糖尿病患者血糖控制效果不理想。糖尿病持续时间较长、糖化血红蛋白达标率较低、糖尿病并发症发生率较高的患者可能更倾向于使用多种降糖药物。关键词:HbA1C;空腹血糖;体重指数;抗糖尿病药物

There were 4 612 subjects included in this cross-sectional study in 2018. According to the use of antidiabetic drugs, the population was divided into untreated group, single drug group, double drugs combination group and multi-drugs combination group, to compare the fasting blood glucose, glycosylated hemoglobin, BMI and prevalence of diabetic complications in different groups. \u0000 \u0000 \u0000Results \u0000About 70.9% of the 4 612 patients used hypoglycemic agents, 34.8% used metformin, 35.1% used sulfonylureas, 22.9% used alpha glycosidase inhibitors, and 13.8% used insulin. The prevalence of diabetic nephropathy, retinopathy, neuropathy, stroke, and diabetic foot was higher in the combination than in the untreated and single-drug users (P 0.05). \u0000 \u0000 \u0000Conclusion \u0000The most common used antidiabetic drugs in diabetic patients in Shanghai are metformin, sulfonylureas, α-glycosides inbibitor, and insulin. The blood glucose control in diabetic patients in Shanghai community is not good enough. Clinical significance of LC-MS/MS aldosterone in screening for primary aldosteronism LC-MS/MS醛固酮筛查原发性醛固酮增多症的临床意义

Objective Retrospective analysis of clinical data of patients with primary aldosteronism in the past 2 years, to assess the value of liquid chromatography-mass spectrometric method (LC-MS/MS) aldosterone to renin ratio(ARR) and saline infusion test and its optimal cutoff in the diagnosis of primary aldosteronism(PA). Methods A total of 124 healthy volunteers were recruited to determine the normal reference range for LC-MS/MS aldosterone. 375 patients with high risk of primary aldosteronism were enrolled during March 2016 to November 2017. Among them, 241 patients underwent the saline infusion test. Receiver operating characteristic (ROC) analysis was performed to compare the diagnostic accuracie and the cutoff value of random ARR and post saline infusion test. Results The average level of LC-MS/MS aldosterone in healthy volunteers was(9.16±6.73)ng/dl (95% CI 2.34-22.18 ng/dl). Among 375 patients with high risk of PA, 143 patients with PA and 232 patients with essential hypertension (EH) were diagnosed. The area under the ROC curve(AUC ROC) of random ARR was 0.837(0.787-0.886), the cutoff point was 13.84 (ng/dl)/(ng/ml·h), with a sensitivity of 78.3% and a specificity of 79%. Using post-test plasma aldosterone concentration (PAC) for diagnosis, AUC ROC of the saline infusion test was 0.807(0.747-0.867), the cutoff point was 4.29 ng/dl, with a sensitivity of 90.6% and a specificity of 69.6%. Conclusion The normal range of LC-MS/MS aldosterone is 2.34-22.18 ng/dl. In our research, random ARR value above 13.84(ng/dl)/(ng/ml·h) can be the cutoff point in screening the suspected PA patients.PAC post- saline infusion test more than 4.29 ng/dl is recommended to confirm PA. Association between glucagon-like peptide-1 receptor gene polymorphisms and metabolic markers in type 2 diabetic patients 2型糖尿病患者胰高血糖素样肽-1受体基因多态性与代谢标志物的关系

Objective To investigate the distribution of polymorphisms of glucagon-like peptide-1 receptor gene (GLP-1R) rs10305420 and rs3765467 in Chinese Han type 2 diabetic patients, and the effects on body weight, blood glucose and serum lipid levels. Methods Two SNPs of GLP-1R rs3765467 and rs10305420 were genotyped by Sanger dideoxy termination sequencing method. The racial difference and the association between the gene polymorphisms and the metabolic markers including BMI, serum lipids and blood glucose were analyzed. Results The distribution of gene polymorphisms was consistent with the Hardy-Weinberg equilibrium. High-density lipoprotein (HDL-C) levels were significantly lower in the rs10305420 T allele carriers than in the CC genotype (1.00±0.18 vs 1.09±0.22, P=0.02). The triglyceride (TG) level of the rs3765467 A allele carrier was higher than that of the GG type (2.75±2.19 vs 2.07±1.36, P=0.03). The allele frequency of rs10305420 C/T was highly statistically significant compared with European population (P=0.000 3). The allele frequency of rs3765467 G/A was statistically different from that of European and African populations (P<0.01). Conclusions The two genetic polymorphisms were significantly associated with lipid levels in patients with type 2 diabetes, and there was a significant racial difference in the frequency distribution of the two variants. Screening and analysis of the serum thyroid stimulating hormone (TSH) of rural pregestational women In Yunnan province 云南省农村育龄妇女血清促甲状腺激素的筛查与分析

Objective Through screening the serum TSH in rural pregestational women, fo find out the high incidence area and high risk population of thyroid disease in Yunnan province, to reduce the probability of infertility and abnormal pregnancy and provide important reference basis for prenatal aristogenesis. Methods The serum TSH level of 30 163 rural pregestational women(20-49 yeas old) which from 16 different areas and 16 different ethnicities of Yunnan province was detected by using TSH3-Ultra (TSH3-UL) reagent on a Siemens centar-XP automatic chemiluminescence apparatus. Results 1 797 women were found with abnormal serum TSH level, the abnormal rate was 5.96%. Among them, the proportion of women with decreased TSH was 3.09% and increased TSH was 2.87%, compared with the normal reference range. And there was a significant difference of abnormal serum TSH rate between pregestational women from different areas and ethnicities. Moreover, in the 16 tested areas, the highest abnormal serum TSH rate was 8.90% from Ximeng county, the lowest was 3.60% from Diqing state. However in the 16 tested ethnicities, the highest abnormal serum TSH rate was 9.73% from Lisu ethnic minority and the lowest was 3.60% from Tibetan ethnic minority. Conclusions The results inferred that ethnic and regional factors both to some extent affect the prevalence of thyroid disease in pregestational women and it is a small probability event that high occurrence of TSH decreased or increased situation simultaneously happened in one area or one ethnicity. 目的通过对云南省农村妊娠妇女血清TSH的筛查,找出云南省甲状腺疾病的高发地区和高危人群,降低不孕和异常妊娠的概率,为产前优生提供重要参考依据。方法采用TSH3-Ultra (TSH3-UL)试剂,在西门子centar-XP型自动化学发光仪上对云南省16个不同地区、16个不同民族的30 163例农村孕妇(20 ~ 49岁)血清TSH水平进行检测。结果1 797例妇女血清TSH异常,异常率为5.96%。其中,与正常参考范围相比,TSH下降的比例为3.09%,TSH升高的比例为2.87%。不同地区、不同民族孕妇血清TSH异常率差异有统计学意义。16个检测地区中,西蒙县TSH异常率最高,为8.90%,迪庆州最低,为3.60%。16个民族中,傈僳族血清TSH异常率最高,为9.73%,藏族最低,为3.60%。结论民族和地区因素均对妊娠妇女甲状腺疾病的发生有一定的影响,在一个地区或一个民族同时发生高TSH降低或升高的情况是小概率事件。关键词:促甲状腺激素;孕前的女性;农村地区;云南少数民族

Progress in the effects of sex steroids on bone mineral density and morphology 性类固醇对骨矿物质密度和形态影响的研究进展

Sex steroids play an important role in the growth, development, and maintenance of the bone. Sex differences are observed in bone mass growth. Bone structure and strength in young males are stronger than young females, which are results of the fully utilization of androgen containing testosterone, its aromatization and its reduction products. As male and female growing older, the reduction in bone mass of males happens later than that of females, which are result of the more moderate decrease of estrogen in males. Our review is to summarize the effects of bone metabolism differences between males and females based on several clinical pathological models and to review the latest researches in this field.
