Detection of SARS-CoV-2 in Peritoneal Fluid in Covid-19 Patient Covid-19患者腹膜液中SARS-CoV-2的检测

Background: The mechanisms of excretion of SARS-CoV-2 are actually mostly unknown. We report a case of detection of SARS-CoV-2 in peritoneal fluid in Covid-19 patient. Case presentation: A 73-year-old woman had emergency surgery for bowel obstruction. An oropharyngeal and peritoneal swab for SARS-CoV-2 virus was performed during admission. Both swabs were positive. Discussion: In the literature, in addition to ours, there is only one case of peritonal swab positive for SARS-CoV-2 in patient Covid-19. Conclusion: The presence of SARS-CoV-2 virus at peritoneal level suggests that Covid-19 infection is a systemic disease and that it is necessary to increase the level of consciousness and protection of surgical personnel, especially in emergency surgery situations.

A Symptomatic Cyst of the Falciform Ligament of the Liver: A Case Report and Review of the Literature 有症状的肝镰状韧带囊肿1例报告及文献复习

Falciform ligament cysts are very rare, with only 12 cases being reported so far. Patients may be asymptomatic or may present with symptoms such as abdominal pain and abdominal mass. In this report, we introduce a symptomatic case with abdominal pain. She was diagnosed with falciform ligament cyst using ultrasound imaging confirmed during laparotomy.

Challenges in Management of Hydatid Cyst 包虫病治疗的挑战

A 35-year-old male presented to the outpatient department with dull aching right hypochondrial pain for 10 days, which was associated with fever & chills. On examination, the patient was febrile, icteric with tender hepatomegaly. Total leukocyte count and liver function test were raised. Serum bilirubin was elevated. USG abdomen showed 5x6x6cm, heterogenous, hypoechoiec lesion with internal cystic areas with peripheral calcification in right lobe. Cect abdomen (Figure 1a) showed daughter vesicles and MRI was suggestive of hydatid cyst. Patient underwent laparotomy wherein calcified hard cyst in the 5th & 6th segment of right lobe was found. Cyst fluid and daughter cysts were evacuated (Figure 1b) and 5% povidone iodine was instilled into cyst cavity for about 10min. Cpapitonnage was done by obliterating cyst space with vascularised omentum. Post op was uneventful. Patient was followed up for six months and it was uneventful.

Factors Used by Program Directors in the Orthopedic Sports Medicine Fellowship Match 项目主管在骨科运动医学奖学金比赛中使用的因素

Background: Obtaining an orthopedic sports medicine fellowship position is becoming more difficult, as the number of residents seeking post-graduate training continues to increase. Objective: To identify factors that orthopedic surgery sports medicine fellowship program directors deem valuable in selecting applicants. Methods: A web-based questionnaire was sent to all ACGME accredited sports medicine fellowship program directors in the United States in 2016. The questionnaire was designed to identify the most important criteria in selecting applicants for an interview, and ranking candidates to match into their program. Results: Thirty-five of ninety-one program directors responded. The criteria for offering an applicant an interview were quality of recommendation letter, technical competence, and residency program reputation. Letters of recommendation that held the highest value were from the chief of sports medicine and another sports medicine surgeon in the department. The most important features of the interview were the applicant’s ability to articulate thoughts, the maturity of the applicant, and the ability of the applicant to listen well. The attributes deemed most important in high ranking a candidate included the applicant’s commitment to hard work, quality of the interview, and quality of letters of recommendation. Conclusion: There are identifiable factors considered important by sports medicine fellowship directors when selecting candidates for an interview and ranking them in the match process. With increasing costs of the application process and competition among applicants, this information can be valuable to medical students and residents considering a career in orthopedic sports medicine.

Intestinal Obstruction by Ileosigmoid Knot: Report of Two Cases at Kara Teaching Hospital (Togo) 回肠乙状结肠结致肠梗阻2例报告(多哥卡拉教学医院)

The authors report two cases of acute intestinal obstruction by an ileosigmoid node. Both patients were young, male, one 24 years old and the other 31 years old. They were admitted in emergency for an acute intestinal obstruction. Abdominal x-ray showed hydroaeric levels in favor of hail and colonic occlusion in the 24-year-old patient while it showed right colonic distension associated with central hailic hydroaeric levels in the denier. A biological examen noted hyperleukocytosis in both cases. Because of the intensity of the pain and the hyperleukocytosis, a laparotomy was performed urgently, allowing an ileosigmoid knot to be found in both cases. In the first case, the imprisoned ileum was necrotic. As defection of the knot was impossible, resection of the ileum and sigmoid was performed followed by an ileo-ileal and colorectal anastomosis. In the second case, a defection of the knot had been performed allowing re-coloring of the ischemic ileum. A dolichomegasigmoid was also noted. Consequently colectomy had been performed. The postoperative period was marked by death in the first patient due to ionic disorders likely due to delayed treatment.

Abdominal x-ray showed hydroaeric levels in favor of hail and colonic occlusion in the 24-year-old patient while it showed right colonic distension associated with central hailic hydroaeric levels in the denier. A biological examen noted hyperleukocytosis in both cases. Because of the intensity of the pain and the hyperleukocytosis, a laparotomy was performed urgently, allowing an ileosigmoid knot to be found in both cases. In the first case, the imprisoned ileum was necrotic. As defection of the knot was impossible, resection of the ileum and sigmoid was performed followed by an ileo-ileal and colorectal anastomosis. In the second case, a defection of the knot had been performed allowing re-coloring of the ischemic ileum. A dolichomegasigmoid was also noted. Consequently colectomy had been performed.
